重度颅脑外伤应用高压氧治疗的疗效及相关因素分析

Abstract Objective: To investigate the clinical efficacy and related influencing factors of hyperbaric oxygen therapy for severe traumatic brain injury. Methods: Fifty-six patients with severe craniocerebral trauma admitted to our hospital during April 2015 to February 2018 were all treated with hyperbaric oxygen therapy. Their clinical data were retrospectively analyzed and patients were prospectively analyzed with Disability Rating Scale (DRS). Efficacy was assessed and statistical analysis was performed on the relevant influencing factors. Results: Compared with before treatment (25.1±4.7 points), the DRS score (12.7±5.6 points) decreased significantly after treatment (P<0.05); the frequency of hyperbaric oxygen therapy, tracheotomy, basic diseases, The injury site and the number of craniocerebral operations did not significantly affect the long-term outcome of hyperbaric oxygen therapy in severe traumatic brain injury (P>0.05), but the time of hyperbaric oxygen therapy, GCS score on admission, and neurological complications affect this The main factors of long-term curative effect of patients (P<0.05) were treated with hyperbaric oxygen therapy within 29 days after injury. The patients' DRS score was significantly better than that after 30 days. The difference was significant (P<0.05). Conclusion: Hyperbaric oxygen therapy for patients with severe traumatic brain injury can effectively improve the prognosis of patients, including hyperbaric oxygenation time, GCS score at admission,Neurological complications are the main factors affecting the long-term efficacy of patients. The timing of hyperbaric oxygen therapy is the most significant.
