Descriptive and Correlational Analyses between Uric Acid Levelsin Serum of the Patients with Cercbral Infanction and the Risk Factors of Carotil Atherosclerosis Plaqne

Objective:
For cerebral infarction uric acid level and relationship between risk factors of carotid atherosclerotic plaques were analyzed.
methods:
a retrospective analysis of Oct. 2015 to Oct. 2016 in our hospital make a diagnosis and give treatment for cerebral infarction clinical data of 160 patients, according to the inspection result into the stability of carotid ultrasound plaque group, the stability of plaque and plaque group, 66 cases, 40 cases and 54 cases respectively, compare the three groups of uric acid levels, and with the help of double function colour to exceed, nature and intima-media thickness in patients with carotid plaques to detect them.
Results:
The results of carotid atherosclerotic plaque risk factors including carotid intima-media thickness, uric acid, glycated hemoglobin level, blood sugar 2 hours after meal;Serum uric acid level unstable plaque group were higher than stability of plaque and plaque group (P < 0.05);Uric acid and carotid intima middle thickness and high density lipoprotein cholesterol negative correlation, and triglycerides appear positive correlation (P < 0.05);Serum uric acid level for cerebral infarction carotid unstable plaque independent risk factors.
Conclusion:
human serum uric acid level for cerebral infarction carotid atherosclerotic plaque formation risk factors, and as an independent risk factor of stability plaques, according to the uric acid level in patients with clinical and ultrasonic diagnosis implement effective prevention measures, to promote patients to alleviate.

Glycated Hemoglobin Joint fasting Glucose Detection Application Value in Screening for Gestational Diabetes

Objective:
To study the a1c and fasting plasma glucose in two ways the effectiveness of the joint inspection of gestational diabetes screening.
methods:
Select 2014 to 2015 in our hospital examination of 582 cases of pregnant women, select the check out 50 cases of gestational diabetes patients as observation group, at the same time, choose the same check the health of pregnant women 50 cases as control group, compared two groups of patients with a1c and fasting plasma glucose levels, and analysis of two types of inspection to the client of gestational diabetes screening results.
Results:
Observation group of patients serum glycosylated hemoglobin and fasting plasma glucose levels were significantly higher than that of control group (P < 0.05).Negative predictive value of gestational diabetes fasting glucose was 43.16%, positive predictive value was 92.38%, specificity of 97.16%, 81.25% accuracy, sensitivity is 43.36%;Glycated hemoglobin for gestational diabetes negative predictive value of 81.97%, positive predictive value was 94.74%, specificity of 98.25%, 93.16% accuracy, sensitivity of 82.25%.
Conclusion:
Glycosylated hemoglobin examination negative predictive value, accuracy and sensitivity of gestational diabetes were significantly higher than that of fasting plasma glucose, two ways of joint inspection can be a good screening method of gestational diabetes.

The Clinical Value of Ultrasonic Root Canal Irrigation for Patients with Periapical Inflammatory

Objective:
To investigate the ultrasonic root canal irrigation in the treatment of patients with locally periapical inflammation and clinical results.
methods:
100 cases of apical periodontitis patients admitted to hospital from June 2012 to June 2015, according to randomized, divided into two groups, 50 cases in the observation group used ultrasonic root canal irrigation operation, the control group using a handheld K file type before and after treatment were compared intervention, root canal colonies, before and after the intervention inner root canal endotoxin levels and the degree of pain the patient, filling two statistical quality and clinical effectiveness.
Results:
After the intervention groups root canal colonies were significantly less than before the intervention (P<0.05), and the intervention group was observed after root canal colonies after the intervention than in the control group (P<0.05), two groups within the root canal after intervention endotoxin levels and pain scores were lower than before the intervention (P<0.05), and endotoxin levels after the intervention group was observed within the root canal and post-intervention pain severity score lower than the control group (P<0.05), after treatment in the observation group appropriate charge rate was significantly higher (P<0.05), clinical healing rate of the observation group was significantly higher (P<0.05), non-healing rate was significantly lower than the control group (P<0.05).
Conclusion:
Ultrasonic root canal irrigation technique can effectively reduce the apical root canal colonies and endotoxin levels in patients with peripheral inflammation, relieve pain in patients, improve clinical outcomes.

Danhong Injection on Serum TNF-α NF-κB and IL-1β in Elderly Patients with Congestive Heart Failure

Objective:
To investigate the effect of Danhong injection on the serum TNF-α,NF-κB and IL-1β in elderly patients with heart failure.
method:
60 patients with chronic heart failure from our hospital were selected and randomly divided into the control group and the experiment group. The patients in the control group were treated by basic anti heart failure therapy. Patients in the experiment group were treated on the base of the control group with the Dan Hong injection. The serum TNF-α,NF-κB , IL-1βlevel and TCM syndrome efficacy and quality of lifewere were tested and compared before and after treatment.
Results:
Compared with the control group, the serum TNF-α,NF-κB and IL-1βlevel were lower(P<0.05). Compared with the control group, the TCM syndrome integral and Minnesota life quality integral were lower,(P<0.05) .
Conclusion:
The serum levels of TNF-α,NF-κB and IL-1β can significantly reduce the serum levels of and IL-1β in elderly patients with heart failure, which is important to improve the clinical symptoms and improve the quality of life of patients with heart failure.

The Relationship between Ghrelin Gene Polymorphism and Essential Hypertension in Patients with Risk Factors

Objective:
To investigate the relationship between the human growth hormone releasing peptide (Ghrelin) and the essential hypertension.
methods:
80 cases of patients with essential hypertension were studied and 80 cases of healthy were selected from October 2014 to October. The Ghrelin gene polymorphism assays of two groups were used with TaqMan-PCR method and Ghrelin gene polymorphism in patients with risk factors for hypertension were analyzed.
Results:
Compared with the control group, patients with hypertension CC, CA, AA genotype frequency distribution was 68.75%, 17.50%, 13.75%, allele C, A frequencies were 0.185,0.875, the control group were 18.75% , 35.00%, 46.25%, allele C, a frequencies were 0.125,0.815. Ghrelin gene CC type serum triglycerides in patients with hypertension (TC), low density lipoprotein (LDL-C), total cholesterol (TG), systolic blood pressure (SBP), diastolic blood pressure (DBP), waist-hip ratio (WHR), body mass (BMI) were significantly higher than those of CA and AA genotype (P <0.05), whereas HDL-C and lower than the CA type AA type (P<0.05). Chi square and Logistic multivariate analysis showed that the Ghrelin gene type CC was associated with TC abnormalities, LDL-C abnormalities, SBP abnormalities, DBP abnormalities, and BMI abnormalities.
Conclusion:
Ghrelin gene polymorphism and risk factors lead to high blood pressure, such as abnormal TC, LDL-C exception, SBP abnormality, DBP abnormality, BMI and abnormal, and therefore, Ghrelin gene polymorphism may be involved in essential hypertension pathogenesis.

Effects of Dexmedetomidine PCIA on Inflammatory Reaction in Esophageal Cancer Surgery

Objective:
To investigate the effects of dexmedetomidine patient-controlled intravenous analgesia on inflammatory reaction in esophageal cancer surgery.
methods:
80 patients (aged 50~70, ASAⅠ~Ⅱ) scheduled for esophageal cancer surgery were randomly divided into 2 groups (in each group): morphine group (group M, n=40) and dexmedetomidine and morphine group (group DM, n=40). PCIA were used in both groups after surgery. Group M were pumping morphine with 0.3mg/kg/h, group DM were pumping dexmedetomidine 1μg/kg/d and morphine 0.3mg/kg/h,both of them were dilutioned to 150 ml with 0.9% normal saline.The expressions of IL-1 and TNF-α in venous blood were tested on the preoperative 1 day (T

Influences of Dexmedetomidine on the Early Cognitive Function and Postoperative Agitation in the Stage of Analepsia in Elderly Patients with General Anesthesia

Objective:
To investigate the effects of dexmedetomidine on the early cognitive function and postoperative agitation in the stage of analepsia in old patients with general anesthesia.
methods:
A total of 60 eligible patients were collected and randomly divided into observation group and control group, with 30 patients in each group. All the patients were given the same anesthesia, and the observation group used dexmedetomidine additionally, while the control group used the same volume of 0.9% sodium chloride injection. The postoperative cognitive function and agitation in the stage of analepsia after anesthesia were observed.
Results:
The two groups had insignificantly different operative times, anesthesia times, and times to recovery of spontaneous breathing (P>0.05); but the observation group had significantly shorter analepsia time than those in control group (P<0.05). The incidence of agitation in the stage of analepsia of the observation group was 16.67%, and was significantly lower than that of the control group (53.33%)(P<0.05). On 1d and 7d after the surgeries, the incidences of postoperative cognitive dysfunction of the observation group were 13.33% and 3.33%, respectively, and were significantly lower than those of the control group (30.00% and 16.67%, respectively)(P<0.05).
Conclusion:
Dexmedetomidine can effectively prevent from postoperative agitation in the stage of analepsia and lighten the effects of the anesthesia on the cognitive function in old patients with general anesthesia.

Expression of CD133 β-catenin and hTERT in Gastric Carcinoma and the Clinical Relevance

Objective:
To study the expression of CD133, β-catenin and hTERT in gastric carcinoma and the clinical relevance.
methods:
CD133 β-catenin and hTERT expression in tissue samples of 60 cases of gastric cancer (group A), 60 cases of precancerous lesions (group B) and 40 cases of chronic non-atrophic gastritis (group C) admitted in our hospital from February 2015 to March 2016 were detected by immunohistochemical method and analyzed.
Results:
The results of immunohistochemistry showed that the positive rates of CD133, β-catenin and hTERT in gastric cancer and precancerous lesions were significantly higher than those in gastritis, the difference was statistically significant (P < 0.05), while the positive rate of gastric cancer was significantly higher than that of cancer precancerous lesion. The expression of A, B group CD133, β-catenin beta and hTERT protein was significantly higher than that in group C, while the A group was significantly higher than that in B group, the differences were statistically significant (P < 0.05); CD133 protein in cancer tissues, β-catenin protein and hTERT protein in higher than that without lymph node metastasis in lymph node metastasis and expression. In the depth of invasion in T3+T4 was higher than that in T1+T2. The difference was statistically significant (P < 0.05). The correlation analysis showed that the expression levels of CD133, β-catenin and hTERT were positively correlated.
Conclusion:
The expression of CD133,β- catenin and hTERT are closely related to the stomach cancer occurrence, development, invasion and metastasis, which can be used as a clinical reference indexes of early diagnosis and prognosis of gastric cancer.

Significance of Monitoring Urinary Micro Albumin in Patients with Hypertensive Cerebral Hemorrhage During Acute Phase

Objective:
To observe the levels of urinary albumin (ABLU) in patients with acute hypertensive cerebral hemorrhage in acute stage, to study the correlation between the patients with hypertensive cerebral hemorrhage and the amount of hematoma, the degree of neurological deficits and short-term outcome of cerebral hemorrhage.
methods:
40 cases of spontaneous cerebral hemorrhage were detected at 24h, 72h (3D), 120h (5d) ABLU content, hematoma volume calculation shows at admission and the incidence of 3~5 CT in the brain, with the degree of neural function defect score scale for patients on admission, with 30 days of onset with life or death as the outcome in patients with short-term; control group 40 cases of physical examination center of the hypertension patients, detection of ABLU; statistical analysis on the above data.
Results:
The concentration of ABLU in hypertensive intracerebral hemorrhage group was significantly higher than that in control group. There was a significant difference in the amount of ABLU, the amount of hematoma and the admission score between the 1 groups (14/40) and the survival group (26/40).
Conclusion:
The concentration of ABLU in patients with hypertensive cerebral hemorrhage is a certain reference value for early diagnosis of the severity of the disease, short-term prognosis and so on.

Relationship between APACHEII Score and Serum ET IGF-Ⅱ CNP Levels and Prognosis in Patients with Traumatic Brain Injury

Objective:
To study the relationship between APACHEII score and serum ET, IGF-, CNP levels and prognosis in patients with traumatic brain injury.
methods:
80 cases of craniocerebral trauma patients admitted in our hospital from Oct. 2011 to Dec. 2015 were selected and divided into mild group (GCS 13~15 points), moderate group (GCS 9~12) and severe group (GCS 3-8 minutes) according to Glasgow Coma Scale (GCS). Endothelin (ET), insulin like growth factor II (IGF-II) and C-type natriuretic peptide (CNP) were collected at admission and the content of serum was measured. General condition was evaluated by Acute Physiology and Chronic Health Evaluation II (APACHE II); 6 months after trauma, prognosis was evaluated by Glasgow Outcome Score (GOS).
Results:
Patients with different severity of craniocerebral trauma of APACHE II score has significant difference (P < 0.05); moderate group and the severe group patients with APACHE II score was lower than that of the mild group and severe group patients with APACHE II score lower than the medium group. Different severity in patients with traumatic brain injury serum ET, IGF-II and CNP levels have significant difference (P < 0.05); moderate group and the severe group patients serum ET levels higher than mild group, IGF-II, CNP levels lower than mild group, severe group patients serum ET level is higher than the medium group, IGF-II, CNP below medium group. A good prognosis in patients with traumatic brain injury, serum levels of ET was significantly lower than that of poor prognosis in patients with traumatic brain injury, IGF-II, CNP was significantly higher than that of poor prognosis in patients with traumatic brain injury, APACHE II score was significantly higher than that of poor prognosis in patients with traumatic brain injury.
Conclusion:
The serum ET, IGF- and CNP score is correlated with the severity of brain injury, which could be used for the evaluation of short term and long term prognosis.

The Relationship between Serum Homocysteine and Cognitive Impairment in Patients with Epilepsy

Objective:
To study the relationship between serum homocysteine (Hcy) and cognitive impairment (MCI) in patients with epilepsy.
methods:
120 patients with epilepsy were divided into MCI group (n = 51) and non MCI group according to the impairment of cognitive function (n = 69). The Hcy, S-adenosine methionine (SAM), S-adenosine homocysteine (SAH) levels of were determined, and the Hcy levels and simple mental state assessment scale (MMSE) score were detected.
Results:
The serum level of Hcy and SAH of patients in MCI group were significantly higher than that in MCI group (P < 0.05); SAM serum level in patients with MCI group was obviously lower than the MCI group (P < 0.05); while serum Hcy levels < 15.0 umoL/L, MMSE score and serum Hcy concentration in patients had no significant correlation (r = 0.19, P > 0.19); When serum Hcy levels acuity 15.0 mu moL/L, MMSE score and serum Hcy concentration had negative correlation (P < 0.05).
Conclusion:
There are some correlations between serum Hcy and epilepsy patients with MCI. Clinical diagnosis can be carried out accordingly it.

Mechanism and Expression Significance of Calcium Binding Protein S100A12 in Patients with CHF

Objective:
To study mechanism and expression significance of calcium binding protein S100A12 in patients with chronic heart failure (CHF).
methods:
58 patients with CHF who received treatment in hospital from October 2015 to July 2016 were selected as the study subjects, chose another same period of physical examination in the hospital without the occurrence of heart failure of volunteers as a control group (50 cases), compared the two groups of clinical detection indicators, patients with different cardiac function in patients with S100A12 levels and echocardiographic indicators, analysis of the correlation between S100A12 level and cardiac function classification and clinical indicators.
Results:
The levels of white blood cell count, red blood cell count, Hs-CRP, HR, LVEDD, LVESD and LVMI in the observation group were significantly higher than those in the control group, the difference was statistically significant (P < 0.05). With the increase of the degree of patients with heart failure, the LVEDD, LVESD, LVMI and S100A12 levels were significantly increased, the difference was statistically significant (P < 0.05). According to the Spearman method, the correlation of S100A12, LVESD, LVEDD, LVMI, HR, Hs-CRP, white blood cell count and red blood cell count were positively correlated.
Conclusion:
Patients with CHF of S100A12 expression and cardiac function and inflammatory markers is very closely linked to clinical as the monitoring index, thus better adjuvant therapy.

Deep Vein Thrombosis and D-dimer in Patients with Femoral Shaft Fracture

Objective:
To investigate the factors associated with the incidence of deep vein thrombosis (DVT) and D-dimer in the patients with femoral shaft fracture.
methods:
Enrolled in the study were 138 patients with femoral shaft fracture who had been treated from June 2014 to December 2015. They were reseanched according to the presence or absence of DVT. The D-dimmer levels were examined for all the cases on the first day after admission,the day before surgery,the 1st, 3rd, and 5th day after surgery.
Results:
In this cohort, DVT occurred in 23 cases. 8 cases(5.80%)occurred in 1 to 3 days after injury, and 8 (5.80%) cases were born at postoperative day second. The level of posttraumatic D-dimer was higher than the normal value. In 1 day before surgery, the D-dimer in thrombus group was significantly higher than non thrombus group, the difference was statistically significant (P<0.05), and third days after the surgery D-dimer level decreased gradually. When the D-dimer critical value was 2.31ug/mL, its sensitivity was 0.631 and its specificity 0.509 in the diagnosis of DVT; when the D-dimer critical value was 4.63ug/mL, the sensitivity was 0.481 and the specificity was 0.807 in the diagnosis of DVT.
Conclusion:
Perioperative DVT may happen in the patients with femoral shaft fracture, mostly involving the near end. Peak time of thrombus was 3 days before injury and 1 day after surgery in both groups. The incidence of DVT may be associated with the injury violence. The appropriate critical value of D-dimer may be 3.6 mg/L In 1 days before surgery. The change trend of D-dimer was similar to that of thrombosis

Factors Patients with Acute Coronary Syndrome Bleeding

Objective:
To investigate the factors associated with acute coronary syndrome patients with hemorrhage.
method:
A retrospective analysis from January 2012 to December 2015 admitted 940 patients with acute coronary syndrome patients divided into two groups according to whether the hemorrhage, medical records and reports of medical instruments and analysis of relevant factors during the treatment of patients with hemorrhage.
Results:
There are two sets of gender factors in patients with significant difference was statistically significant (P <0.05); factor of age, history of diabetes, history of hypertension, high cholesterol history, history of myocardial infarction, peptic ulcer history, history of heart failure, GPⅡb / receptor antagonists exist factors associated with postoperative use of heparin factors Ⅲ significant difference was statistically significant (P <0.01); sex, age, history of diabetes, history of hypertension, high cholesterol history, history of myocardial infarction, digestion Road ulcer history, history of heart failure, GPⅡb / Ⅲ receptor antagonist use ORmin factors associated with postoperative heparin factor> 1, is a risk factor in acute coronary syndrome patients with hemorrhage.
Conclusion:

Effects of General Anesthesia and Epidural Anesthesia on Postoperative Cognitive Function in Elderly Patients with Orthopaedic Surgery

Objective:
To observe and compare the general anesthesia and epidural anesthesia effects on short-term cognitive function in elderly patients with orthopaedic surgery.
methods:
95 cases of orthopedic surgery elderly patients were randomly divided into two groups: control group (n = 47) line of general anaesthesia, the observation group (n = 48) line of epidural anaesthesia, the anesthesia of arterial blood pressure and heart rate, open and tube drawing and response time, contrast before anesthesia, after anesthesia, 6 h, 12 h, 24 h, 72 h of simple mental health scale (MMSE) score, statistics of two groups of patients with postoperative cognitive dysfunction (POCD) were compared before and after anesthesia.
Results:
For the arterial blood pressure, heart rate, there was no significant difference between the two groups before and after anesthesia, during operation, 30min, and at the end of the operation (P > 0.05); For opening eyes timing, extubation and response time, there was no significant difference (P > 0.05); The 24h score of MMSE in the observation group was higher than that in the control group (P < 0.05); The POCD incidence rate of 6h and 12h in the observation group was lower than that in the control group (P < 0.05).
Conclusion:
Compared with general anesthesia, the effect of epidural anesthesia on postoperative cognitive function in elderly patients with Department of orthopedics is more slight.

To Explore the Best time of Hemabate in Prevention and Treatment of Postpartum Hemorrhage

Objective:
To study the best time of hemabate in prevention and treatment of postpartum hemorrhage, to provide basis for clinical diagnosis and treatment.
methods:
114 cases of cesarean section with bleeding tendency from February 2014 to February 2016 in our hospital were selected, the patients were randomly divided into group I, group II and group III, 38 cases in each group, group I injection of oxytocin and hemabate immediately after fetal delivery, group II injection of the oxytocin immediately after fetal delivery, application of hemabate when uterine atony, group III injection of oxytocin immediately after fetal delivery, not using hemahate, the bleeding intraoperative, back to the ward to postoperative 2H and 2H to postpartum 24h were compared between the three groups.
Results:
The bleeding during operation of group I significantly lower than II group and III group (P < 0.05), there were no significant amount of intraoperative blood loss in group II and group III (P > 0.05); back to the ward to postoperative 2H bleeding of group I and group II were significantly lower than those in the group III (P < 0.05), group I and group II were not statistically significant (P > 0.05), 2H to postpartum 24h bleeding of group I and group II were significantly lower than those in the group III (P < 0.05), group I and group II were not statistically significant (P > 0.05).
Conclusion:
Fetal delivery immediately injection of oxytocin and Pei could effectively reduce the amount of postpartum hemorrhage, it is the best time to prevent postpartum hemorrhage.

Objective:
Retrospective analysis to clinic effect of angiotensin II receptor antagonist (ARB) Aprovel in the treatment of Vasovagal Syncope, and observe and evaluate the clinical curative effect with β blocker metoprolol.
methods:
86 cases were diagnosed by tilt table test, and retrospective analyzed the patients respectively in treatment of Aprovel and Metoprolol. Recorded the patients baseline and the tilt test result after 3 months, further to observe the negative conversion rate and drug side effects.
Results:
Compared with those before treatment, patients in the two groups have been greatly improved, while the metoprolol group Hutt negative rate is slightly better than Aprovel group, the difference is statistically significant (P<0.05). In adverse reaction respect, Aprovel group was slightly better than metoprolol group, the difference is statistically significant (P<0.05).
Conclusion:
ARB has a certain effect on the treatment of patients with Vasovagal Syncope. It is a kind of alternative treatment method with less adverse reactions than β blocker.

The Effect of Total Thyroidectomy for Primary Hyperthyroidism and its Influence on Patients with Cardiac Function and Liver Function

Objective:
To observe the clinical application of thyroid full cut method for the treatment of primary thyroid function hyperfunction and its influence on patient's heart and liver function.
methods:
98 cases of primary hyperthyroidism in our hospital from July 2014 to August 2015 were randomly divided into control group and observation group (n = 49). Patients in the control group were treated with subtotal thyroidectomy, while the patients in the observation group were treated with total thyroidectomy. The clinical curative effect and impact on the patient's heart and liver function of patients in two groups were compared.
Results:
After treatment, the transient hypothyroidism incidence of patients in the observation group was significantly higher than the control group (P<0.05), and the recurrence rate of postoperative bleeding and operation time were significantly lower than the control group (P<0.05). Cardiac function and liver function indexes of patients in the two groups were no significant difference (P>0.05).
Conclusion:
Clinical application of total thyroidectomy in the treatment of primary hyperthyroidism has significant effect, can significantly improve the heart and liver function.

Objective:
To analyze and study the clinical effect of transcatheter selective cerebral arterial thrombolysis in the treatment of patients with acute cerebral infarction.
methods:
48 patients with acute cerebral infarction who were treated with transcatheter selective cerebral arterial thrombolysis in our hospital from June 2014 to May 2016 were selected as the observation group,48 patients who were treated with intravenous thrombolysis at the same time were selected as the control group, then the total effective rates at different time after the treatment, nerve function indexes before and after the treatment of two groups were compared.
Results:
The total effective rates of observation group at 1st,2nd and 4th week after the treatment were all higher than those of control group, the serum expression levels of nerve function indexes at 1st, 2nd and 4th week after the treatment were all lower than those of control group, all P<0.05,the results of two groups all had significant differences.
Conclusion:
The clinical effect of transcatheter selective cerebral arterial thrombolysis in the treatment of patients with acute cerebral infarction is better,and its improvement role for the nerve function of patients is more obvious, so the application value of treatment in the patients with acute cerebral infarction is higher.

Analysis on Efficacy and Prognostic Factors of Craniotomy at Different Time and Endovascular Therapy for Subarachnoid Hemorrhage Caused by Ruptured Aneurysm

Objective:
To analyze the difference for efficacy and prognosis of craniotomy at different time and endovascular therapy for SAH.
methods:
The clinical data of 152 patients with subarachnoid hemorrhage caused by ruptured aneurysm were analyzed retrospectively, and were divided into two groups according to Hunt-Hess classification, the low grade group(n=87, Ⅰ~Ⅲ level)and the high grade group(n=65, Ⅳ~Ⅴlevel). The GOS was applied to evaluate the prognosis of patients. The effect of craniotomy in the early process and endovascular intervention of two groups was compared.
Results:
In the low grade group, the good rate of craniotomy and endovascular intervention in the early process was higher than that of delayed treatment(P<0.05). In the high grade group, the good rate of craniotomy and endovascular intervention of delayed treatment was higher than that of early treatment(P<0.05). The difference for the good rate of craniotomy and endovascular intervention between low grade and high grade was not evident(P>0.05).
Conclusion:
Operation time is an important factor influencing treatment effect of subarachnoid hemorrhage caused by ruptured aneurysm, but operation method has no correlation with treatment effect. Taking craniotomy and endovascular intervention actively can effectively improve the prognosis.

The Anesthesia Advantage of Intranasal Dexmedetomidine(DEX) Application Used in Pediatric Anesthesia Induction Period Undergoing the Lower Abdomen and Lower Extremity Surgery

Objective:
To observe the anesthesia advantage of intranasal dexmedetomidine(DEX) application used in pediatric anesthesia induction period undergoing the lower abdomen and lower extremity surgery.
methods:
We enrolled 40 patients who were undergoing the general anesthesia for lower abdomen and lower extremity elective surgery, which were randomly devided into A(n=20) and B(n=20) groups , intranasal dexmedetomidine 1ug/kg 30min before operation in group A, physiological saline instead in group B. 1mg /kg propofol was infused intravenously every time who could not smoothly into the operating room of children as well as the induced intolerance to oxygen mask of children. 1. Record each patient general information respectively; 2. Sedation status when separating from their parents and mask induction scores were assessed, the dose of propofol during induction period were recorded ; 3. Mean arterial pressure(MAP) ,Heart rate(HR) and saturation of pulse oximetry (SPO
2
) were recorded at different time points as follows: baseline values were recorded before intranasal dexmedetomidine (T0), transferred into operation room (T1), immediately inserting the laryngeal mask (T2) , at 5 min after intubation(T3) and operation immediately (T4); 4. The time of operation, induction , extubation and anesthesia awake were recorded; 5. The adverse events such as hypoxemia , bradycardia , reflection of tussis , laryngismus and tachycardia were recorded.
Results:
1.The general situation have no statistical significance (P>0.05); 2. Compared with group B, sedation status when separating from their parents are higher in A group , the consumption of propofol are less than B group , mask induction scores are lower in A group(P<0.05); 3. The influence of two groups on the MAP, HR and SPO
2
are difference (P<0.05), the A group was relatively stable than the B group. Compared with T0 , the MAP, HR and SPO
2
of group A at all points are no difference (P>0.05), the MAP of group B at T1 are lower, at T2 are higher significantly (P<0.05) . The HR of group B at T1,4 are higher significantly (P<0.05). The SPO

of group B at T1 are lower significantly (P<0.05). 4. The time of operation, extubation and anesthesia awake have no statistical significance (P>0.05), the induction time of A group are shorter than B group(P<0.05); 5. The two groups have no hypoxemia and bradycardia , compared with group B, the emergence reflection of tussis , laryngismus and tachycardia are lower in group A(P<0.05).
Conclusion:
Intranasal dexmedetomidine(DEX) application used in pediatric anesthesia induction period undergoing the lower abdomen and lower extremity surgery could significantly reduce induction time and reduce adverse reaction, increase comfort and safety of children in the transfer process, it has a relatively stable hemodetomidine during induction.

Clinical Efficacy and Effects on Postoperative Quality of Life of MIPPO in Treatment of Adult tibial Shaft Fracture

Objective:
To investigate the clinical efficacy and effects on quality of life of micro-invasive percutaneous plating (MIPPO) in treatment of tibial shaft fracture.
methods:
Totally 60 eligible patients were selected and randomly divided into observation group and control group, with 30 patients in each group. The control group was treated with common open reduction with anatomical plate, while the observation group treated with MIPPO, and the basic conditions of the surgeries, postoperative qualities of fracture healing, and qualities of life were observed in the patients of the two groups.
Results:
The patients of the two groups had insignificantly different operative times (P>0.05); but the observation group had significantly better incision length, total blood loss, length of stay, and time to healing than the control group (all P<0.05). Of the observation group, excellent clinical efficacy was achieved in 12 patients, good clinical efficacy achieved in 15 patients, moderate clinical efficacy achieved in 2 patients, and poor clinical efficacy achieved in 1 patient; while of the control group, excellent clinical efficacy was achieved in 8 patients, good clinical efficacy achieved in 14 patients, moderate clinical efficacy achieved in 6 patients, and poor clinical efficacy achieved in 2 patients; and the differences were statistically significant (P<0.05). At 6m after the surgeries, the observation group had significantly better SF-36 scores than the control group (all P<0.05).
Conclusion:
MIPPO in treatment of tibial shaft fracture has a good clinical efficacy, can promote fracture healing and elevate quality of life, and is effective.

Improvement Role of Rehabilitation of Continuous Passive Motion Training of Sitting and Supine on PostoperativePatients with Lower Limb Bone Joint

Objective:
To investigate the role of sitting and supine continuous passive motion training on rehabilitation of lower limb bone joint after operation.
method:
A retrospective analysis of clinical data of 151 cases of lower limb joint injury patients in our hospital September 2012 to 2014 September, underwent surgical treatment, divided into control group (73 cases) and observation group (78 cases) were given conventional postoperative rehabilitation training and traditional rehabilitation training combined with sitting and supine continuous passive motion training.
Results:
Compared with the group before treatment, the two groups of flexion and extension activity were significantly improved, P < 0.05; after treatment in observation group, the joint flexion and extension activity were significantly higher than the control group, P < 0.05. Compared with the group before treatment in two groups of 6 minutes walking distance and 10 meter walking time were significantly improved, walking distance increased, walking time significantly shortened, P < 0.05; the observation group after treatment 6 minutes walking distance and 10 meter walking time were significantly shorter than or greater than the control group, P < 0.05. Compared with this group before treatment, two groups of balance ability score were significantly improved, P < 0.05; after treatment, the observation group balance ability score was significantly higher than the control group, P < 0.05.
Conclusion:
The implementation of sitting and supine continuous passive motion training can get ideal effect for patients with lower limb bone joint after operation, improve the motor function and balance ability, promote the rehabilitation of patients after operation.

The Application Effect of Spinal Epidural Anesthesia Combined Continuous Epidural Anesthesia in Total Hysterectomy

Objective:

methods:
86 patients with total hysterectomy admitted in our hospital from December 2013 to February 2015 were selected. All the patients were randomly average divided into anesthesia group and epidural anesthesia group. Anesthesia group patients were taken anesthesia of combined spinal epidural anesthesia, epidural anesthesia group patients were taken anesthesia of continuous epidural anesthesia, the mean arterial pressure and heart rate were recorded, anesthetic effect of patients were compared.
Results:
Joint anesthesia onset time of anesthesia was significantly shorter than the epidural anesthesia group, P <0.05, two groups of patients no significant difference in operative time, the amount of anesthetic in anesthesia group was significantly lower than epidural anesthesia group, P <0.05. Different time, anesthesia group MAP, HR and oxygen saturation smooth change, the change was significantly lower than the epidural anesthesia group, P <0.05. The total fine rate observed on Narcotic was significantly higher than epidural anesthesia group, P <0.05, a muscle relaxant anesthesia group was significantly better than the epidural anesthesia group, P <0.05, two groups of patients with adverse events was no significant difference, P> 0.05.
Conclusion:
The method of spinal epidural anesthesia combined continuous epidural anesthesia in total hysterectomy is simple, safe and effective.

The Effects of Folic Acid on Homocysteine Level and Atherosclerosis in Elderly Patients with H-type Hypertension

Objective:
To observe the effects of folic acid on homocysteine level and atherosclerosis in elderly patients with H-type hypertension.
Methods:
112 patients with H-type hypertension who were diagnosed and treated in our department from March 2015 to December 2015 were randomly divided into the control group and the observation group (56 cases). The patients in the control group were treated with routine antihypertensive therapy. The observation group was treated with folic acid on the basis of the control group. The effects of treatment on homocysteine level and atherosclerosis were compared between the two groups.
Results:
Compared with the control group, the total effective rate of the observation group was significantly higher than that of the control group (P <0.05). There was no significant difference in plasma Hcy, baPWV, SBP and DBP between the two groups before treatment (P> 0.05). The levels of plasma Hcy, baPWV, SBP and DBP in the two groups after treatment were significantly lower than those before treatment (P <0.05). The decrease of the indexes of the observation group was significantly higher than that of the control group (P <0.05).
Conclusion:
The clinical application of folic acid in the treatment of elderly patients with H-type hypertension can significantly improve the clinical treatment efficiency, improve plasma Hcy levels and atherosclerosis state, reduce blood pressure, can be applied in clinical practice.

Clinical Observation on 36 Cases of Adolescent Idiopathic Scoliosis Treated with Three Integrated Methods

To observe the clinical effect and adverse reaction of three integrated methods with traditional Chinese chiropractic technique, personal dimensional traction combined with orthopedic brace for adolescent idiopathic scoliosis.
methods:
80 cases with adolescent idiopathic scoliosis, according to the consulting order, were divided into treatment group (the single number) and control group (the double number). The treatment group was carried out the treatment by three integrated
methods:
traditional Chinese chiropractic technique, personal dimensional traction combine with orthopedic brace. The control group was treated only by Boston orthopedic brace. Each group was treated for three months. The changes of Cobb angle, VAS score before and after the treatment, the clinical effects and whether adverse reactions were observed. All the data were analyzed by SPSS19.0.
Results:
Cobb angle and VAS score in treatment group were obviously lower than control group,there was a statistical significance(P<0.05); the cure and improvement rate in treatment group was obviously higher than the control group. There was no adverse reaction in both groups.
Conclusion:
The three integrated methods with traditional Chinese chiropractic technique, personal dimensional traction combined with orthopedic brace for adolescent idiopathic scoliosis has a good efficacy. There is no adverse reactions and it is worthy of further clinical promotion.

To discuss the relationship between micro-inflammatory state and malnutrition in patients with diabetic nephropathy.
methods:
120 patients of different stages of diabetic nephropathy were observed,we measured the level of nutrition and micro-inflammatory markers such as GLU,ALB,CREA,BUN,CRP,IL-6,TNF-αbefore treatment to analyse the relationship between micro-inflammatory status and nutrition status.
Results:
Nutrition and micro-inflammatory status were different in different stages of diabetic nephropathy(p<0.05),the level of nutrition and inflammatory markers were gradually increased along with the progress of DN.There was negative correlation between CRP and ALB,preALB,positive correlation between CRP and IL-6,TNF-α(p<0.05);Also positive correlation was found between ALB and preALB,CREA,age(p<0.05).
Conclusion:
The micro-inflammatory status was correlated with malnutrition,micro-inflammatory can aggravate malnutrition,and micro-inflammatory can be influenced by malnutrition,clinic attention should be paid.

Results After treatment, the patient's treatment, the total effective rate was higher than that of control group, and P < 0.05, the experimental group of cough and expectoration vanished time, antibiotics application, lung inflammation, basic absorb the time and the length of hospital stay in patients were significantly shorter than the control group, and P < 0.05, the difference is statistically significant. Conclusion In patients with cerebral hemorrhage of severe lung infection after the implementation of flexible bronchoscopy mirror alveolar lavage treatment, the treatment effect is remarkable, suggested that clinical popularization and application.

Comparative Analysis of Clinical Efficacy of Three Different Surgical Methods in the Treatment of Patients with Severe Mixed Hemorrhoids

Objective:
To investigate the clinical efficacy of three different surgical methods in the treatment of patients with severe mixed hemorrhoids.
methods:
90 cases with severe mixed hemorrhoid from January 2014 to March 2016 in the Department of general surgery were selected and divided into three groups according to admission time sequence with 30 cases in each group. Group A were treated by rubber ring ligation combined with Milligan surgery, and group B by Xiaozhi spirit injection joint strip plunge into art and group C by Milligan Morgan operation. The clinical efficacy surgery related indicators, complications, postoperative recurrence rate and satisfaction degree were observed and compared among the three groups.
Results:
The clinical curative effect were no statistical difference among the three groups of patients (P>0.05). The operation time, bleeding volume and hospitalization time of group A and group B were both less than those in group C (P<0.05), there were on statistical difference between group A and group B (P>0.05). The wound healing time in group B was significantly less than that of group A and C group (P<0.05) and the wound healing time in group A less than that in group C (P<0.05). The complications of group A was significantly less than that in group B and group C (P<0.05) and the complications of group B were less than it in group C (P<0.05). The pain score of B group at each time were significantly lower than that of group A and group C (P<0.05), and the pain score of group A were lower than that in group C (P<0.05); the recurrence rate and satisfaction of patients in group A were better than those in group B and group C (P<0.05).
Conclusion:
Rubber ring ligation combined with Milligan Morgan and Xiaozhi spirit injection joint strip plunge into art are effective in the treatment of severe hemorrhoids. Rubber ring ligation combined with Milligan Morgan has the advantages of simple operation and less complications and Xiaozhi spirit injection joint strip plunge into arthas less pain and wound healing fast.

Objective:
To study the C - reactive protein (CRP), serum prealbumin (PA), serum calcitonin original joint detection (PCI) in the diagnosis of infectious disease clinical value.Selection
Methods:
our hospital between 2015 and 2016 of 108 patients with infectious diseases were divided into viral infection group of 43 patients with bacterial infection group 65 examples, selecting the healthy physical examination were 50 cases for the same period as the healthy control group.
Results:
CRP, PCI in patients with bacterial infection group was obviously higher than that of other two groups (P < 0.05), PA significantly lower than the other two groups (P < 0.05), the level of CRP, PCI virus infection group was obviously higher than that of control group (P < 0.05), the virus infection group and control group in the PA has no obvious difference (P > 0.05).Respectively by PCT > 5 ng/ml, CRP > 50 mg/L, PA < 180 mg/L for the standard statistical positive rate, positive rate of patients with bacterial infection group were significantly increased (P < 0.05).
Conclusion:
the serum PCT and CRP and PA indexes can be used for clinical diagnosis of infectious disease, can be a distinction between infection of pathogenic bacteria, but joint detection methods with higher accuracy.

Clinical Significance of Serum Adiponectin of Levels in Patients with Acute Coronary Syndrome

Objective:
To study the clinical value of serum adiponectin of levels in patients with acute coronary syndrome.
methods:
40 cases of ACS patients treated in our hospital from 2015 to 2016 were selected as observation group, and 40 cases of healthy as healthy control group. IMD and hypersensitive c-reactive protein (hs - CRP) indicator, with the result of coronary angiography, the Gensini score assessment of coronary artery stenosis with Pearson correlation analysis method to evaluate the IMD and the correlation of the indicators to be detected.
Results:
IMD and hs - CRP levels of observation group patients were significantly higher than that of control group (P < 0.01), serum level of IMD and Gensini score and hs - CRP levels were maintaining positive correlation (P < 0.01).
Conclusion:
The level of serum IMD in patients with ACS is significantly higher than that in healthy people, which is closely related to the progression of coronary artery disease. It could be used as an effective marker for ACS diagnosis and prognosis evaluation.

Hydrochloric Acid Treatment for ek, EGFR Mutation Status Clear Advanced Non-small Cell Lung Cancer Clinical Observation

Objective:

Clinical Application of Drip Bipolar Coagulation in Thyroidectomy

Objective:
To explore the effect of drip bipolar coagulation on parathyroid glands and recurrent laryngeal nerve protection in thyroidectomy.
method:
70 patients underwenttotalthyroidectomy were divided into research group and control group. Patients in the research group were treated by harmonic scalpel and drip bipolar coagulation operation thyroidectomy, and patients in the control group were treated by harmonic scalpel operation thyroidectomy. The operative time, bleeding, palsy of recurrent laryngeal nerve, the blood calcium and postoperative PTH were compared between the two groups.
Results:
There was no difference of the operative time and bleeding in the two groups(P >0.05). The complications of transient function insufficiency of parathyroid and transient palsy of recurrent laryngeal nerve were more in the control group (14.3% and 17.1%) than in the research group (2.8% and 5.7%) with statistical difference (P<0.05). There were no permanent function insufficiency of parathyroid and permanent palsy of recurrent laryngeal nerve in the two groups.
Conclusion:
Drip bipolar coagulation for thyroidectomy can effectively reduce the surrounding temperature of the tissue, and can protect the parathyroid and recurrent laryngeal nerve.

Clinical Efficacy of Budesonide Aerosol Inhalation and Influence on Serum Inflammatory Factor Levels in Children with Mycoplasma Pneumonia

Objective:
To investigate the clinical efficacy of budesonide aerosol inhalation and influence on serum inflammatory factor levels in children with mycoplasma pneumonia.
Methods:
Totally 100 eligible children were selected and randomly divided into observation group and control group according to the admission numbers, with 50 children in each group. Both groups were given conventional therapies, while the observation group used budesonide spray inhalation additionally. The changes in clinical symptoms and physical signs of the children were observed in the treatment period, and the serum levels of interferon-γ (IFN-γ), tumor necrosis factor -α (TNF-α), interleukin-2 (IL-2), interleukin-6 (IL-6), interleukin-10 (IL-10), and interleukin-13 (IL-13) were determined before treatment and after 7 d of treatment, respectively.
Results:
For the observation group, 32 children were cured, 14 children were responsive to the therapy excellently, 3 children were improved, and 1 child was unresponsive to the therapy; while for the control group, 25 children were cured, 11 children were responsive to the therapy excellently, 9 children were improved, and 5 children were unresponsive to the therapy; and the differences were statistically significant (P<0.05). The observation group had significantly shorter times to disappearances of fever, cough, asthma, and rales in lungs than the control group (P<0.05). Before treatment, the two groups had insignificantly different serum levels of proinflammatory factors such as IFN-γ, TNF-α, and IL-6 (P >0.05); after 7 d of treatment, both groups had significantly decreased serum levels of IFN-γ, TNF-α, and IL-6 compared with those before treatment, but the observation group had significantly greater decreasing extents than the control group (P<0.05). Before treatment, the two groups had insignificantly different serum levels of antiinflammatory factors such as IL-2, IL-10, and IL-13 (P >0.05); after 7 d of treatment, both groups had significantly increased serum level of IL-2 and significantly decreased serum levels of IL-10 and IL-13 compared with those before treatment, but the observation group had significantly greater improving extents than the control group (P<0.05).
Conclusion:
Budesonide aerosol inhalation can rapidly relieve the clinical symptoms and physical signs, promote normalization of serum levels of antiinflammatory and proinflammatory factors, and elevate the clinical efficacy in children with mycoplasma pneumonia.

Short term Efficacy of Laparoscopic Surgery in Elderly Patients with Rectal Cancer

Objective:
To study the short-term effects of laparoscopic surgery for rectal cancer in elderly patients.
methods:
88 cases of rectal cancer in elderly patients admitted to our hospital from September 2013 to September 2015 were enrolled in the study. All patients were divided into two groups according to the order of admission, each group had 44 cases. Among them, 44 cases were treated by open surgery as control group; the other 44 cases were treated by laparoscopic operation as observation group. The operation condition, postoperative recovery indicators and complications of the two groups of patients were observed.
Results:
The operation time of the observation group was longer than that of the control group, the amount of bleeding, the number of lymph nodes and the length of stay in hospital were better than those in the control group (P<0.05). Postoperative exhaust time, eating time, catheter indwelling time and the time of getting out of bed in the observation group were shorter than those in the control group (P<0.05). The incidence of complications in the observation group and control group were 6.82% and 18.18% respectively, and the incidence rate of the observation group was significantly lower (P<0.05.).
Conclusion:
For rectal cancer patients, compared with open surgery, in addition to longer operation time, laparoscopic surgery has less trauma, faster recovery, more lymph node clearance, less postoperative complications, etc. It is worthy of clinical application and promotion.

Effect of Analgesia and Sedation on Cerebral Metabolism in Patients with Severe Subarachnoid Hemorrhage

Objective:
To integrative clinical effect of analgesia and sedation on cerebral metabolism in patients with severe subarachnoid hemorrhage.
methods:
55 patients with severe subarachnoid hemorrhage were chosen for this investigation. Fentanyl citrate injection, midazolam injection and propofol injection were used for the treatment of severe subarachnoid hemorrhage. The carotid, jugular and central venous oxygen saturation before, during and after analgesia and sedation treatment were detected.
Results:
Before sedation and analgesia treatment, the carotid, jugular and central venous blood oxygen saturation were (0.86±0.19), (0.46±0.07) and (0.48±0.05). 2h after analgesia and sedation treatment started, carotid artery oxygen saturation decreased significantly (t=4.63, P < 0.05); while the jugular vein (t=7.62, P < 0.05) and central vein (t=7.53, P < 0.05) oxygen saturation increased significantly. After analgesia and sedation treatment, carotid artery oxygen saturation decreased, while jugular vein and central venous oxygen saturation increased, and the results had no significant difference with the results before treatment (P > 0.05).
Conclusion:
The use of analgesia and sedation treatment of patients with severe subarachnoid hemorrhage can not only reduce the body metabolism, but also reduce the cerebral metabolism. Whether analgesia and sedation treatment can improve the prognosis of severe subarachnoid hemorrhage needs further study.

Comparison the Effect of Different Doses of Dezocine Combined with Ropivacaine in Gynecological Patients with Patient Controlled Epidural Analgesia after Operation

Objective:
To compare the effects of different doses of dezocine combined with ropivacaine in gynecological patients with postoperative epidural analgesia, and occurrence of adverse reactions, in order to dezocine combined with ropivacaine in epidural analgesia after gynecological surgery to find the best regimen.
methods:
From January 2014 to December 2015, 120 cases of gynecologic surgery in our hospital were selected, Four groups were randomly divided into four groups using the program, Statistical analysis of VAS pain score and the incidence of adverse reactions were different.
Results:
The postoperative VAS pain score of D group was compared with that of group A, the difference was not statistically significant (p>0.05); But B group and C group postoperative VAS pain score than A group postoperative pain scores were significantly lower (P<0.05, P<0.01). The incidence of headache, nausea and skin itching in group C was slightly lower than that in group D, the difference was not statistically significant (p>0.05), but the difference in the incidence of vomiting was statistically significant between the two groups (P<0.05); The incidence of skin itching, nausea, postoperative headache, vomiting and other adverse reactions in group A and group B were lower than that in group D, and the difference was statistically significant (P<0.05, P<0.01).
Conclusion:
Dezocine combined with ropivacaine in gynecological patients with postoperative epidural analgesia has a good analgesic effect, and with the increasing dose of dezocine analgesia effect is more obvious, but the incidence of adverse reactions was also increased. By changing the dosage of dezocine, and achieve the analgesia requirements or reduce the incidence of adverse reactions of the requirements, to provide personalized combination therapy for different patients.

Study on Correlations between Hyperhomocysteinemia as Well as Atherosclerosis and Acute Cerebral Infarction

Objective:
To study the correlation between hyperhomocysteinemia (Hhcy) and acute cerebral infarction and to explore the relationship between hcy level and acute cerebral infarction.
methods:
96 patients with acute cerebral infarction and 90 normal healthy subjects were selected as objects. The levels of hcy were comparatively analyzed.
Results:
The serum hcy level of patients with acute cerebral infarction(23.56±4.65) μmoL/L was significantly higher than that of the healthy group(11.28±2.19) μmoL/L; The Hhcy patient number of the acute cerebral infarction group was 46, and its proportion was 47.9%, while the Hhcy patient number was 10 of the healthy group and its proportion was 11.1%, that has a significant difference (P <0.05).
Conclusion:
The Hhcy may be one of independent risk factor in acute cerebral infarction; the hcy level and the incidence of Hhcy of patients with acute cerebral infarction are significantly higher than normal people.

Influence of Different Anesthesia Methods on postoperative Pulmonary Infection in Elderly Patients Undergoing Upper Abdominal Surgery

Objective:
To observe the effects of different anesthetic methods on postoperative pulmonary infections in the elderly patients with upper abdominal surgery.
methods:
A total of 102 cases of elderly patients with upper abdominal surgery from April 2014 to April 2016 were enrolled. They were randomly divided into epidural block combined with general anesthesia (group A), inhalation anesthesia group (group B) and total intravenous anesthesia group (group C) with 34 cases each. The extubation time, recovery time, incidence of postoperative pulmonary infections were observed and compared among the three groups.
Results:
The extubnation time, recovery time and incidence of postoperative pulmonary infection in group A were significantly better than those in both group B and group C (p<0.05), but there was no significant difference in those between group B and group C (p>0.05). The level of interleukin-6(IL-6) and interleukin-8(IL-8) were increased apparently in the 6h after the surgery of the three groups (p<0.05), but T lymphocyte subsets CD4+/CD8+ ratio reduced apparently (p<0.05) compared with those before anesthesia. 3d after the operation, the level of IL-6, IL-8 and CD4+/CD8+ ratio in group A restored to preoperative level while those in group B and C did not(p<0.05).
Conclusion:
Epidural block combined with general anesthesia has a positive effect on postoperative recovery of consciousness and reduces the incidence of (postoperative) pulmonary infection.

Study on the Correlation between CT Manifestations and Pulmonary Function in Patients with Chronic Obstructive Pulmonary Disease

Objective:
To study the correlation between CT manifestations and lung function in patients with chronic obstructive pulmonary disease.
methods:
All the data of 120 patients with chronic obstructive pulmonary disease admitted in our hospital from October 2014 to October 2016 were analyzed. According to the CT characteristics of M, E and A model, with CT image pixel index, the analysis of each type pixel index and types of CT manifestations and lung function index correlation.
Results:
The pixel index of type A was lower than M, E. The pixel index and residual volume/total lung of type E, A were positively correlated (P < 0.05).
Conclusion:
The clinical analysis of the correlation between CT expression and pulmonary function test can provide the basis for the prevention and treatment of chronic obstructive pulmonary disease.

Effects of Dexmedetomine on Closed-loop Muscle Relaxant Injection System Using Cisatracurium

Objective:
To investigate the effect of dexmedetomine (DEX) on close-loop muscle relaxant injection system using cisatracurium (CIS) in laparoscopic cholecystectomy (LC).
Methoyds:
60 patients undergoing LC under general anesthesia with tolal intravenous anesthesia were randomly divided into three groups: group A, normal saline (NS)+3ED95CIS; group B, DEX+3ED95CIS; group C, DEX+4ED95CIS 20 case in each group. In group B and C, a bolus close of 1.0μg/kg DEX was infused before 15 minutes of induction, followed by continous infusion of 0.5μg/kg/h untill 20 min before the operation ended. Normal saline was infused in the same way in group A. The onset time, intubation condition, the first-time to add medicine, clinic effect time, the time from stopped close-loop muscle relaxant to recovery of TOF to 0.2,0.5,0.7,0.9, and recovery index (RI) of CIS.
Results:
There were no statistical significant on onset time, intubation condition, the first time to add medicine, clinical cluration, the time from stopped close-loop muscle relaxant to recovery of TOF to 0.2,0.5,0.7,0.9 and RI of CIS in groupAand group B. Comparison between group C and group A, onset time of group C was shorter than in group A (P<0.05), and first add medicine time longer than group A (P<0.05). Comparison between group B and group C, onset time of group C shorter than group B, first add medicine time longer than group B (P<0.05). The intubation condition in group C was better than in group B (P<0.05).
Conclusion:
DEX can not affect metablism and effect of CIS with closed-loop muscle relaxant injection, but it can improve the condition of intubation. Use DEX conbined with CIS was safe to the patient.

The Effects of Two Methods on Dental Fear in Children

Objective:
To observe the effects of the application of full psychological intervention and local anesthetic method on dental fear in children in the oral clinica treatment for child dental diseases.
method:
120 diseased children, including 64 female cases and 56 male cases, who had been diagnosed as having superficial or deep caries of dentin were randomly divided into three groups with 40 cases in each group. Group A(treated with psychological intervention): in coordination with the six-handed operation, the circuit nurse applied psychological intervention to the full process (the early, middle and late terms) of the treatment for the diseased children. Group B(treated with local anesthetic method): in coordination with the six-handed operation, the doctor performed local anesthesia on the decayed teeth of the diseased children. Group C(blank control group): the six-handed operation was used in coordination with the doctor’s routine work and no intervention was performed on dental fear in children. All the three groups received the routine filling treatment. The clinical effects and the occurrence of dental fear in each group were observed.
Results:
The time for the doctor’s cavity preparation in Group A and Group B is different from that in Group C(F﹦170.94,P<0.05); The diseased children in Group A showed the best cooperation(F﹦88.07,P<0.05); the doctor’s degree of satisfaction in Group A and Group B is different from that in Group C(H﹦83.17,P<0.05); the relatives of the diseased children in Group A showed the highest degree of satisfaction(H=95.45,P<0.05); the filling damage rate one month after the operation in Group A is the lowest, which is 5%; the occurrence rate of dental fear in Group A is the lowest(H﹦82.57,P<0.05).
Conclusion:
full psychological intervention has more apparent effect in the prevention of dental fear in the treatment for child dental diseases with better social benefits.
