Expression and Clinical Correlation Analysis of EGFR and Ki-67 in Lung Adenocarcinoma with Micropapillary Structure

Objective:
To investigate the relationship between epidermal growth factor receptor (EGFR) gene mutation and the expression of Ki-67 protein in lung adenocarcinoma with micropapillary structure (MPP) and its clinical significance.
Methods:
The clinical data of MPP lung adenocarcinoma in our hospital from 2013 to 2017 were collected. The mutation of EGFR gene in 80 patients was detected by gene sequencing, and the expression of Ki67 was detected by immunohistochemistry.
Results:
Of the 80 specimens, 50 cases were detected EGFR mutation, the total detection rate was 62.5%. Among them, the mutation rate of EGFR gene was high in non smoking and female. It was not related to age, tumor size, metastasis and staging; the expression rate of Ki67 protein was 63.8%. The expression of Ki67 and sex, age, smoking history, tumor size, lymph node metastasis and staging were all. No correlation; EGFR and Ki67 expression were positively correlated.
Conclusion:
The mutation rate of EGFR in MPPAC is higher than that in other subtypes of lung adenocarcinoma. It is highly expressed in female, non smoking, tumor size over 3cm. The expression rate of Ki67 in MPPAC was higher than that in other subtypes of lung adenocarcinoma, and it was highly expressed in patients with tumor diameter greater than 3cm. In MPPAC, the expression of EGFR gene and Ki67 protein is positively correlated. The positive rate of both of them is related to the size of tumor. It is speculated that the mutation of EGFR gene leads to abnormal overexpression of Ki67 and promotes the proliferation of tumor cells. In the Sanya group with different MPP ratios, the EGFR gene mutation rate and the positive rate of Ki67 protein expression were not related to the proportion of MPP.

The Relationship and Efficacy of the First-line Chemotherapy combined with Bevacizumab to K-ras Gene Status in Advanced Colorectal Cancer

Objective:
To investigate the relationship and efficacy of the first-line chemotherapy combined with bevacizumab to K-ras gene status in advanced colorectal cancer (CRC).
Methods:
A retrospective analysis of 56 patients with CRC was conducted in this study, detected of K-ras gene status and treated with first-line chemotherapy combined with bevacizumab. The relationship between the mutation status of the K-ras gene and the response rate (RR), the progression free survival (PFS) and the total survival (OS) of the treatment was observed separately.
Results:
K-ras gene mutations were found in 58.2% of the patients with CRC.The K-ras gene mutation group transferred to the lung and liver more than the K-ras gene wild-type group, and the difference was statistically significant (P<0.05). There was no significant difference in RR rate and PFS between the two groups (P=0.61,0.83). The K-ras gene mutation group in OS was less than the wild type group, but the difference was not statistically significant (P=0.93). K-ras gene mutation was not a prognostic factor for PFS (P=0.86) and OS (P=0.17) in the Cox survival analysis model.
Conclusion:
The mutation status of the K-ras gene is not a prognostic factor of efficacy of the first-line chemotherapy combined with bevacizumab in CRC.

Expression of Multidrug Resistance Associated Protein 1 in Osteosarcoma and the Relationship with Pathological Characteristics

Objective:
To investigate the expression of multidrug resistance associated protein 1 (MRP1) in osteosarcoma and its relationship with pathological features of osteosarcoma.
Methods:
80 cases of osteosarcoma admitted to our hospital from January 2016 to January 2017 were selected as the research group. Meanwhile, 20 healthy persons in the same period were selected as control group. The difference between MRP1 in the two groups was compared, and the relationship between MRP1 and tumor stage and tumor size was analyzed.
Results:
The positive rate and positive rate of MDR1 in osteosarcoma patients were significantly higher than those in normal bone tissue (P<0.05). The positive expression of MDR1 and P-gP in grade III patients and grade IV patients were significantly higher than those in grade I and grade II patients (P < 0.05). The positive rate of MDR1 and P-gP expression in large tumor patients and the positive rate were significantly lower than those in patients with small tumors (P < 0.05).
Conclusion:
The expression of MDR1 is positively related to the pathological grade of osteosarcoma patients, and there is a negative correlation with the size of the tumor. It is necessary for the medical staff to pay attention to the clinical treatment.

Clinical Efficacy of Compound Xuanju Capsule in Treatment of Chronic Prostatitis and the Influence on Serum MIP-2 and SIgA Levels

Objective:
To investigate the clinical efficacy of compound Xuanju capsule in the treatment of chronic prostatitis and the influence on the serum MIP-2 and SIgA levels.
Methods:
130 cases of patients with chronic prostatitis treated from September 2014 to March 2016 in our hospital were selected and divided into the observation group and the control group according to the random number table, with 65 cases in each group. The control group was given a daily moxifloxacin hydrochloride tablets and prostate massage once; the observation group was given complex Xuanju capsules on the basis of control group, three times a day, three tablets each time. The changes of serum MIP-2 and SIgA levels were compared between the two groups before and after treatment.
Results:
The overall effective rate was significantly higher in the observation group than that in the control group, and the NIH-CPSI score after treatment was significantly lower than that of the control group [(10.93±3.20) vs (19.68±4.27)](P<0.05). The levels of serum MIP-2 and SIgA in both groups were significantly lower than those in the control group before and after treatment, the serum MIP-2 and SIgA levels in the observation group were lower than those in the control group [(4.17±0.61)ng/mL vs (6.20±1.04) ng/mL];[(66.51±40.16) μg/mL vs (91.57±48.06)μg/mL] (P<0.05). The secretion of white blood cells in the two groups was significantly decreased after treatment, and the secretion of the observation group was significantly lower than that of the control group [(15.38±11.39)n/HP vs(21.74±10.59)n/HP] (P<0.05).
Conclusion:
Compound Xuanju capsule could significantly improve the clinical effect of chronic prostatitis patients, which could promote the recovery of the disease and effectively reduce the expressions of serum MIP-2 and SIgA.

Effects of Crimomycin Combined with Moxifloxacin Intrathoracic Injection on the Serum and Pleural Effusion Interferon-γ TNF-α CEA and ADA Levels of Patients with Tuberculous Pleurisy

Objective:
To research the effect of crimomycin combined moxifloxacin intrathoracic injection on the serum and pleural effusion interferon-γ(IFN-γ), tumor necrosis factor-α(TNF-α), carcinoembryonic antigen (CEA), adenosine deaminase (ADA) levels of patients with tuberculous pleurisy.
Methods:
98 cases of patients with tuberculous pleurisy admitted to our hospital from April 2013 to April 2016 were selected and randomly divided into two groups, the control group was treated with crimomycin, while the experimental group was treated with moxifloxacin based on the control group. The clinical curative effect, changes of serum and pleural effusion IFN-γ, TNF-α, CEA, ADA, forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), pleural thickness levels before and after treatment and incidence of adverse reactions were compared between two groups.
Results:
The total effective rate of experimental group was higher than that of the control group (P<0.05). Serum and pleural effusion IFN-γ, FVC and FEV1 in two groups were significantly higher than those before treatment, and the above indexes were significantly higher in the experimental group than in the control group. Serum TNF- α, CEA, ADA and pleural thickness in two groups were significantly lower than before treatment, and the above indexes were significantly lower than those in the control group. There were statistically significant differences in the interactions between the two groups, between different time points, between groups, and between different time points (P<0.05). There was no statistically significant difference between the two groups (P>0. 05).
Conclusion:
The curative effect of crimomycin combined moxifloxacin intrathoracic injection was bettetr than monocolomycin alone with tuberculous pleurisy patients in the treatment of patients with tuberculous pleurisy, whcih can effectively regulate the serum and pleural effusion levels of interferon-γ, TNF-α, CEA, ADA.

Observation on Caltrate D Combined with Zoledronic Acid Injection in the Treatment of Chronic Low Back Pain with Primary Osteoporosis

Objective:
To study effect of caltrate D combined with zoledronic acid injection in the treatment of primary osteoporosis patients with chronic low back pain.
Methods:
this study was limited to 80 patients with primary osteoporosis chronic low back pain admitted from January 2016 to January 2017. Randomized control method was divided into control group and experimental group, the patients in the control group were treated with zoledronic acid injection alone, and the patients in the experimental group were treated with caltrate combined with zoledronic acid injection. The scores of daily functions, total effective rate of treatment and visual pain simulation score were compared between the two groups.
Results:
The total effective rate, daily function score and visual pain analogue scale of the control group were significantly better than those of the control group (P<0.05).
Conclusion:
The treatment of chronic lumbago and pain of primary osteoporosis with the combination of calcium D and zoledronic acid injection can effectively improve the treatment effect, daily function and relieve the pain symptoms of the patients.

Effects of Moxifloxacin Combined Seretide on Serum SAA TGF-β1 Copeptin, and sICAM-1 of AEC OPD

Objective:
To study the clinical efficacy of moxifloxacin combined seretide inhalation in acute exacerbation of chronic obstructive pulmonary disease (AECOPD), and its effects on serum amyloid A (SAA), transforming growth factor-β1 (TGF -β1), copeptin, soluble intercellular adhesion factor-1 (sICAM-1).
Methods:
A total of 102 AECOPD patients were enrolled of our hospital from December 2015 to December 2016. They were randomly divided into control group and experimental group (n = 51). The control group was treated with seretide combined Amoxicillin - clavulanic acid inhalation on the basis of routine treatment. The experimental group was treated with moxifloxacin combined seretide inhalation. To compare the changes of serum SAA, TGF-β1, copeptin and sICAM-1 in two groups, to observe the changes of serum CO

), and to detect lung function: (FEV1), vital capacity (FVC) and FEV1 / FVC were used to assess the quality of life and dyspnea of patients with COPD: Medical Research Council Dyspnoea Scale( mMRC), COPD Assessment Test( CAT), and adverse events.
Results:
After treatment, the effective rate (92.16%) in the experimental group was significantly higher than that in the control group (P <0.05). FEV1, FCV, FEV1 / FCV were improved after treatment, the difference was statistically significant (P <0.05), and the experimental group improved more than the control group (P <0.05). of the two groups was significantly higher than that of the control group (P <0.05)., and the experimental group was significantly lower than the control group (P <0.05), the difference was statistically significant (P <0.05), and the difference was statistically significant (P <0.05). Conclusion:
The clinical efficacy of moxifloxacin combined seretide inhalation on AECOPD patients can reduce serum SAA, TGF-β1, copeptin, sICAM-1 levels, improve arterial oxygen pressure, quality of life and lung function.

Effect of Conical Telescopic Denture on Masticatory Efficiency Plaque Index and Tooth Mobility in Patients with Periodontal Disease with Dentition Defect

Objective:
To explore the effects of conical telescopic denture on masticatory efficiency, plaque index (PLI) and tooth mobility in patients with periodontal disease and dentition defect.
Methods:
176 patients with periodontal disease and dentition defect were divided into group A (treated with conical telescopic denture, n=96) and group B (treated with traditional removable denture, n=80) according to different repair methods. The masticatory efficiency of the prosthesis was evaluated by sieving at 1 month, 6 months and 1 year after the repair. The PLI, gingival index (GI) and tooth mobility were evaluated, and the satisfaction degree was investigated.
Results:
The masticatory efficiency of two groups was poor before repair (P > 0.05). It was gradually improved in 1 year after repair, and the masticatory efficiency of group A was significantly better than that of group B at 1 month, 6 months and 1 year after repair (P < 0.05). Before repair, the PLI, GI and tooth mobility were similar in the two groups (P > 0.05). After repair, the PLI, GI and tooth mobility were significantly decreased, and the decrease in group A was significantly greater than that in group B (P < 0.05). In 1 year after repair, the satisfaction degree of comfort, chewing ability, phonological competence and retention ability of group A was significantly higher than that of group B (P < 0.05).
Conclusion:
Compared with the traditional removable denture, conical telescopic denture can effectively improve the masticatory efficiency, reduce PLI and tooth mobility, and improve the satisfaction of patients with periodontal disease and dentition defect.

Clinical Efficacy of Leflunomide Combined with Moderate Dose Prednisone in Treatment of IgA Nephropathy and Effects on Serum Levels of ICAM-1 MCP-1 and TGF-β

Methods:
96 patients with IgA nephropathy admitted to our hospital from October 2013 to May 2016 were randomly divided into control group and experimental group, with 48 cases in each group. All patients on admission were treated with conventional treatment. On the basis of this treatment, the control group received a large dose of prednisone, and the experimental group received leflunomide and a medium dose of prednisone.The clinical efficacy of the two groups before and after treatment was observed and compared. The levels of serum ICAM-1, MCP-1, TGF-β
1
, adverse reactions were observed in the two groups.
Results:
After treatment, the effective rate of the experimental group (95.83%) was higher than that of the control group (81.25%), and the difference was statistically significant (P<0.05). 3 months and 6 months after treatment, the levels of serum ICAM-1, MCP-1 and TGF-β
1
were significantly decreased in the two groups, and the experimental group was significantly lower than that of the control group. There was a significant difference in the interaction between the two groups, different points and between groups and at the same time (P<0.05). There was no significant difference in adverse reactions between the two groups (P>0.05).
Conclusion:
The effect of leflunomide and moderate dosage of prednisone in the treatment of IgA nephropathy is sure to reduce the level of serum ICAM-1, MCP-1 and TGF-β
1
, which is worthy of clinical application.

Efficacy Analysis of Masquelet and Ilizarov Technique in the Treatment of Adult Bone Defect of Long Bone of Lower Extremity

Objective:
To compare the clinical effect of Masquelet and Ilizarov technique in the treatment of adult bone defect of long bone with long bone.
Methods:
A retrospective analysis was made of 38 patients with long bone infection of the lower extremities from July 2014 to September 2017 with Masquelet and Ilizarov techniques, of which 16 were treated with Masquelet, and 21 were treated with Ilizarov. The complications, bone healing time, total weight bearing time, knee and ankle Iowa scores and SF-36 scores before and last follow-up were compared between the two groups.
Results:
In group 5~10cm, the bone healing time and weight bearing time in Ilizarov group were better than those in Masquelet group (P < 0.05). In bone defect group 10~15cm, Masquelet group had better bone healing time and weight-bearing time than P Technology (P < 0.05). The two methods made the patients' knee, ankle Iowa score, SF-36 score, and the last follow-up better than the preoperative.
Conclusion:
The treatment of Masquelet and Ilizarov have a good effect on the adult bone defect of long bone. Patients with more bone defects use Masquelet technology, and Ilizarov technology is used for patients with poor skin and soft tissue condition, severe infection, lack of autogenous bone and less bone defect.

Efficacy of Renal Replacement Therapy in Patients with Sepsis combined with MODS Guided by Serum Cys C

Objective:
To investigate the reference value of serum cystatin C (Cys C) detection on the continuous renal replacement therapy time of the patients with sepsis combined multiple organ dysfunction syndrome (MODS).
Methods:
86 cases of sepsis combined with MODS were selected from June 2015 to August 2017 in our hospital with ICU. According to the time intervals between the time point of serum Cys C level began to increase and the time point of continuous renal replacement therapy began, they were divided into group A (<48h, 34 cases), group B (48~72h, 25 cases) and group C (>72h, 27 cases). The changes of 24h vital signs before and after continuous renal replacement therapy were recorded and compared, including heart rate, body temperature, APACHE II score and SOFA score; 48h cardiac index (CI), stroke output index (SVI), mean arterial pressure (MAP) and peripheral vascular resistance index (SVRI) levels were monitored before treatment and after treatment; venous blood and arterial blood samples were taken before treatment and 48h after treatment, the contents of alanine aminotransferase (ALT), total bilirubin (TBIL), blood urea nitrogen (BUN), serum creatinine (Scr), blood lactate and procalcitonin (PCT) were measured.
Results:
①Compared with before treatment, the 24h heart rate, body temperature, APACHE II score and SOFA score of the three groups were significantly lower (P<0.05) after continuous renal replacement therapy. Compared with group B and C, the heart rate, body temperature, APACHE II score and SOFA score in group A were significantly decreased (P<0.05). ②Compared with before treatment, the 48h CI, SVI and MAP increased significantly (P<0.05) and SVRI significantly decreased (P<0.05) in the three groups after continuous renal replacement therapy. Compared with group B and C, the increase of CI, SVI and MAP was more significant (P<0.05), and the decrease of SVRI was more significant (P<0.05) in group A. ③Compared with before treatment, ALT, TBIL, BUN, Scr, blood lactic acid and PCT decreased significantly in the three groups at 48h after continuous renal replacement therapy (P<0.05). Compared with group B and C, the decrease of ALT, TBIL, BUN, Scr, blood lactic acid and PCT in group A was more significant (P<0.05). ④The time of continuous renal replacement therapy and the time of ICU hospitalization in group A were significantly less than those in group B (P<0.05); the time of continuous renal replacement therapy and the time of ICU hospitalization in group B were significantly less than those in group C (P<0.05). The 4 weeks mortality rate in group A was lower than that in group B and group C, but the difference was not statistically significant (P>0.05).
Conclusion:
Continuous renal replacement therapy within 48h of serum Cys C level may better control the condition of septic patients with MODS.

Application of TDI-Tei Index in Evaluation of Right Ventricular Function in Patients with High Altitude Pulmonary Hypertension

Objective:
To analyze the relationship between Tei index and pulmonary artery pressure measured by pulsed Doppler imaging and tissue Doppler imaging, and to explore its value in evaluating right ventricular function in patients with pulmonary hypertension at high altitude.
Methods:
Sixty patients with pulmonary hypertension treated in our hospital from July 2016 to January 2018 were selected as study subjects. According to the degree of pulmonary arterial pressure, the patients were divided into mild, moderate and severe subgroups, 20 cases in each group, at the same time, normal subjects (control group, 30 cases) and healthy people (plain group, 30 cases), pulse wave Doppler imaging and Tissue Doppler imaging were used to measure the tricuspid annulus in each group. The ejection fraction, isovolumic systolic phase, isovolumic diastolic phase, the Tei index of each group of patients was calculated and the relationship between TDI-Tei index and pulmonary pressure was analyzed.
Results:
From plain to control group to mild to severe PAH group, the indexes of ICT and IRT increased gradually, ET index decreased gradually, but PW-Tei and TDI-Tei index increased gradually (P<0.05). The indexes of ET in TDI group were significantly higher than those of PW group, while the indexes of ICT and IRT were significantly lower than those in PW group. The final Tei index was significantly higher than that in PW group (P<0.05). Pearson correlation analysis of TDI-Tei, PW-Tei index and pulmonary artery systolic pressure showed that there was a positive correlation between TDI-Tei and PW-Tei index and pulmonary artery systolic pressure (r=0.968, P<0.01; r= 0.732, P<0.01).
Conclusion:
Both PW-Tei and TDI-Tei index can evaluate the right cardiac function of patients with pulmonary hypertension at high altitude, and the accuracy and sensitivity of TDI-Tei index are higher. It is worthy of clinical application.

Effect of Phacoemulsification and Lens Implantation on Central Corneal Thickness and OCT in Patients with High Myopia and Cataract

Objective:
To research the influence of ultrasonic emulsification sucked out and intraocular lens implantation on the visual quality, central corneal thickness and macular optical coherence tomography (OCT) of high myopia patients combined with cataract.
Methods:
112 cases of high myopia patients with cataract from March 2014 to March 2016 were selected and divided into the control group and the research group according to the lottery method, with 56 cases in each group. The control group was treated with extracapsular cataract extraction and intraocular lens implantation, the study group was treated with phacoemulsification and intraocular lens implantation. The clinical curative effect, changes of the luminosity, best corrected visual acuity, central corneal thickness, concave retinal thickness macular center before and after surgery, and postoperative complications were compared between two groups.
Results:
The total effective rate of research group was higher than that of the control group 94.64% vs 78.57%(P<0.05). Before the operation, the luminosity, best corrected visual acuity, central corneal thickness, macular center concave retinal thickness showed no significant difference between two groups (P>0.05). After surgery, the scattered luminosity of both groups were significantly lower than those before the surgery, which was lower in the research group than that of the control group (P<0.05); the best corrected visual acuity of both groups were rised than those before the surgery, which was higher in the research group than that of the control group (P<0.05). The central corneal thickness and macular center concave retinal thickness of control group were increased than those before surgery. The central corneal thickness and concave retinal thickness macular center of research group showed no difference statistical significance between two groups(P>0.05). The corneal edema, capsule membrane turbidity, aqueous humor turbidity and the incidence of high intraocular pressure of Complications in research group were significantly lower than those of the control group (P<0.05).
Conclusion:
Phacoemulsification and intraocular lens implantation are effective in the treatment of high myopia with cataract, which can improve the visual quality and alleviate the effect of the central corneal thickness and macular OCT.

The Role of Acoustic Impedance Test, DPOAE and ABR Test in Diagnosis and Treatment of AN-SOM in Children

Objective:
To explore the effects of acoustic immittance test, distortion product otoacoustic emissions (DPOAE) and auditory brainstem response (ABR) test in the diagnosis of acute non-suppurative otitis media (AN - SOM) in children.
Methods:
120 children with AN - SOM diagnosed in the department of otolaryngology in the hospital from February 2016 to December 2017 were enrolled in the study. There were 28 cases with bilateral acute earache seizures and 92 cases with single ear attack, a total of 148 ears. According to the presence of symptoms such as tinnitus, ear swelling and stuffiness, ear blockage and hearing loss, the ears were divided into 85 symptomatic ears and 63 asymptomatic ears. The rest 92 ears were normal ears. The acoustic immittance test, DPOAE and ABR test were performed.
Results:
The abnormal rates of acoustic immittance test, DPOAE and ABR for symptomatic ears were significantly higher than those of asymptomatic ears or normal ears (P < 0.05). The abnormal rates of 3 audiological examinations of asymptomatic ears were significantly higher than those of normal ears (P < 0.05). In the 3 audiological examinations, the percentages of 2 abnormal tests and 3 abnormal tests of symptomatic ears were significantly higher than those of asymptomatic ears or normal ears (P < 0.05). The tympanogram of symptomatic ears mainly was type B or type C while that of asymptomatic ears mainly was type As or type B and that of normal ears mainly was type A (P < 0.05). The hearing loss of symptomatic ears was mainly moderate or severe and that of asymptomatic ears mainly was mild or moderate but there was no hearing loss of normal ear (P < 0.05).
Conclusion:
Acoustic impedance test, DPOAE and ABR test can also be used as an auxiliary diagnosis method for AN-SOM, which helps to reduce the misdiagnosis and missed diagnosis of children with asymptomatic AN-SOM.

Clinical Effect of Loading Dose Plus High Maintenance Dose of Atorvastatin in Treating ACS and the Influence on Serum Inflammatory Factors and Vascular Endothelial Function

Objective:
To explore the clinical curative effect of loading dose plus high maintenance dose of atorvastatin in treating acute coronary syndrome (ACS) and the influence on serum inflammatory factors and vascular endothelial function.
Methods:
96 patients with ACS treated by percutaneous coronary intervention (PCI) were equally divided into the experimental group and the control group by random number method. The two groups were treated with conventional dose (20mg/d) and loading dose plus high maintenance dose (80mg/d before operation, 40mg/d after operation, 20mg/d 1 month later) of atorvastatin, respectively. Serum inflammatory factors (hs-CRP, IL-6, TNF-α), vascular endothelial function (NO, ET-1) and yocardial injury markers (CK,CK-MB) were determined before and after treatment. The incidence rates of major adverse cardiovascular events in the two groups within 3 months after discharge were statistically analyzed.
Results:
Levels of serum hs-CRP, IL-6, TNF-α, ET-1, CK and CK-MB in the two groups decreased significantly in 1 month after treatment, while NO levels increased significantly, compared with those before operation (before medication). The decrease or increase in the experimental group were significantly greater than those in the control group (P < 0.05). The incidence rate of major adverse cardiovascular events in the experimental group was significantly lower than that in the control group within 3 months after discharge (8.33% vs 22.92%) (P < 0.05).
Conclusion:
The loading dose plus high maintenance dose of atorvastatin can significantly inhibit the release of inflammatory factors, improve the endothelial function, effectively protect the myocardium of patients with ACS, and significantly reduce the incidence rate of adverse cardiovascular events.

Changes of cTnI, IMA and CK-MB levels in Patients with Septic Cardiomyopathy and their Relationship between the levels and Prognosis

Objective:
To investigate changes in levels of cardiac troponin I (cTnI), ischemia modified albumin (IMA) and creatine kinase isoenzyme (CK-MB) in patients with septic cardiomyopathy and their relationship with the prognosis.
Methods:
The clinical data of 90 patients with septic cardiomyopathy who were admitted to the hospital from February 2011 to February 2016 were retrospectively analyzed. The patients were divided into the survival group and the death group according to the survival within 1 month, and the general datawere compared between the two groups. Levels of cTnI, IMA and CK-MB in the two groups were detected at different time points (the day of diagnosis, the6th and12thday). The correlation between levels of cTnI, IMA and CK-MB in patients with septic cardiomyopathy and the prognosis was analyzed.
Results:
51 cases survived and 39 died among the 90 patients within 1 month. The APACHE II score, levels of cTnI, IMA and CK-MB in the survival group were significantly lower than those in the death group (P < 0.05). Levels of cTnI, IMA and CK-MB in the survival group on the day of confirmed diagnosis, on the 6th and 12th days were significantly lower than those in the death group (P < 0.05). Logistic regression analysis showed that increased cTnI, IMA and CK-MB were high risk factors influencing the prognosis of patients with septic cardiomyopathy.
Conclusion:
Levels of cTnI, IMA and CK-MB are obviously higher in patients with septic cardiomyopathy. The increased cTnI, IMA and CK-MB have significant effect on the prognosis of patients with septic cardiomyopathy.

Predictive Value of Modified Level of Plasma Lactic Acid and Shock Index to Short-term Prognosis in Patients with ST-segment Elevation Myocardial Infarction

Objective:
To study the predictive value of level of plasma lactic acid (LA) and modified shock index (MSI) for 10-day outcome to short-term prognosis in patients with ST-segment elevation myocardial infarction (STEMI).
Methods:
STEMI patients (n=678) were chosen from the Department of Emergency in the Affiliated Hospital of Chengde Medical College from June 2015 to June 2017. According to the level of LA level and MSI, the patients were divided into higher group (LA≥2 mmoL/L) and normal group and (LA<2 mmoL/L ), and MSI≥1.3 group and MSI<1.3 group. The general data of the patients was recorded, and major cardiac adverse events (MACE) in 10 days was reviewed and the incidence of all-cause mortality was observed, and predictive value of two indexes to short-term prognosis was analyzed by using area under curve (AUC) of receiver operating characteristic curve (ROC).
Results:
The difference in systolic blood pressure, diastolic blood pressure, heart rate, grades Ⅲ and Ⅳ of Killip grading had statistical significance between LA higher and normal group (P<0.05). The incidence of fatal arrhythmias and cardiogenic shock and all-cause mortality were significantly LA higher group than those in LA normal group (P<0.05). The difference in systolic blood pressure, diastolic blood pressure, heart rate, grades Ⅲ and Ⅳ of Killip grading had statistical significance between MSI≥1.3 group and MSI<1.3 group. The incidence of fatal arrhythmias and cardiogenic shock and all-cause mortality were significantly higher in MSI≥1.3 group than those in MSI<1.3 group (P<0.05). The hospital mortality was 7.52%. AUC of LA was 0.823 and AUC of MSI was 0.789 ,which had statistical significance in prediction of short-term prognosis in STEM patients.
Conclusion:
LA and MSI all have some predictive value to short-term prognosis in STEM patients, and LA has higher predictive value.

Laparoscopic Radical Hysterectomy Plus Pelvic Lymph Node Dissection for Cervical Cancer and Effect on Bladder and Rectal Function

Objective:
To analyze the effects of laparoscopic pelvic nerve-sparing radical hysterectomy combined with pelvic lymphadenectomy in the treatment of cervical cancer and the effect on vesicorectal function.
Methods:
The clinical data of 140 patients with cervical cancer treated in our hospital from February 2014 to January 2016 were collected. According to the treatment of pelvic nerve during operation, the patients were divided into the retention group (treated by laparoscopic pelvic nerve-sparing radical hysterectomy and pelvic lymphadenectomy, n=64) and non-retention group (treated by radical hysterectomy and pelvic lymphadenectomy, n=76).The operation, postoperative recovery and recovery of vesicorectal function were compared between the two groups. Urodynamic test was performed in the two groups before operation and 3 months after operation. The bladder compliance, the maximum urine flow rate, maximum bladder capacity and maximum detrusor systolic pressure were measured. The incidence of postoperative complications was statistically analyzed. All patients were followed up for 1 year, and the improvement of quality of life (QOL) was evaluated by the Functional Assessment of Cancer Therapy-Cervix (FACT-Cx).
Results:
①There were no significant differences between the two groups in operation time, intraoperative blood loss, number of dissected lymph nodes, the length of parametrial resection and the length of vaginal resection (P>0.05). ②The postoperative anal exhaust time, indwelling catheter time and hospital stay of the retention group were shorter than those of the non-retention group (P<0.05). ③The incidence rates of urinary symptoms and complications were significantly lower in the retention group than in the non-retention group(P<0.05).④After operation, the bladder compliance, the maximum urine flow rate and maximum detrusor systolic pressure were decreased while the maximum bladder capacity was increased in the two groups (P<0.05). After operation, the bladder compliance, the maximum urine flow rate and maximum detrusor systolic pressure in the retention group were higher than those in the non-retention group (P<0.05), and the scores of quality of life were higher than those in the non-retention group (P<0.05).
Conclusion:
The application of laparoscopic pelvic nerve-sparing radical hysterectomy combined with pelvic lymphadenectomy in the treatment of cervical cancer can protect the vesicorectal function in patients and improve the quality of life of patients.

Effect of Folic Acid Combined with Atorvastatin on Carotid Artery Intima-media Thickness in Patients with Type H Hypertension

Objective:
To study the effect of folic acid combined with atorvastatin on carotid artery intima-media thickness in patients with Hypertension.
Methods:
A total of 144 H-type hypertensive patients admitted to our hospital from January 2016 to July 2017 were enrolled in this study. All patients underwent no antihypertensive drugs in the recent 1 month and were randomly divided into observation group and control group. Group folic acid combined with atorvastatin treatment, the control group was treated with atorvastatin, 6 months follow-up, Hcy levels in patients with bilateral carotid artery IMT were measured.
Results:
After treatment, the systolic blood pressure (138.71±6.93mmHg vs 141.77±10.24mmHg) and diastolic blood pressure (90.82±7.12mmHg vs 94.72±6.55mmHg) were significantly different between the two groups (P<0.05), and the observation group was at the level of Hcy ( 9.82±3.11umol/L vs 12.32±3.18umol/L) and IMT (1.14±0.11mm vs 1.29±0.15mm) were significantly better than the control group, P<0.05, and the difference was statistically significant.
Conclusion:
Folic acid combined with atorvastatin has significant clinical efficacy in reducing Hcy level and decreasing IMT thickness of common carotid artery in patients with H type hypertension, which is worthy of clinical promotion.

Ultrasonographic Findings and causes of Misdiagnosis for Patients with Acute Abdomen Diseases

Objective:
To analyze the ultrasonic findings of acute abdominal diseases and the causes of misdiagnosis.
Methods:
100 cases of patients with acute abdominal diseases treated in our hospital from April 2012 to May 2017 were selected and examined with color Doppler ultrasonography. According to the features of ultrasonic images, the types of acute abdominal diseases were diagnosed, and the results were compared with the diagnosis results of operation and pathology. The accuracy and misdiagnosis rate of ultrasonic diagnosis of acute abdominal diseases, and the causes of misdiagnosis were analyzed.
Results:
According to the sites, size, echo and blood flow images of lesions displayed by ultrasound, it was found that the ultrasonic findings of patients with acute abdominal diseases caused by different causes were obvious. The postoperative pathological diagnosis of 100 patients with acute abdominal diseases showed 19 cases of urinary calculi, 18 cases of hepatic stones and gallstones, 15 cases of acute appendicitis, 11 cases of acute cholecystitis, 10 cases of acute pancreatitis, 10 cases of intestinal obstruction, 12 cases of intestinal perforation, 5 cases of ectopic pregnancy and ovarian torsion, and 1 case of abdominal large vascular disease. The accuracy rate of ultrasonic diagnosis was 84.00%, and the misdiagnosis rate was 16.00%. Among the misdiagnosed patients, there were 2 cases of urinary calculi misdiagnosed as chronic colitis and intestinal obstruction, 1 case of gallstone misdiagnosed as intestinal polyps, 1 case of acute appendicitis misdiagnosed as urinary calculi, 2 cases of acute cholecystitis misdiagnosed as gallstones and cholestasis, 1 case of acute pancreatitis misdiagnosed as intestinal fistula, 1 case of intestinal obstruction and 1 case of intestinal perforation misdiagnosed as acute appendicitis, 2 cases of ectopic pregnancy misdiagnosed as purulent appendicitis and early pregnancy, 3 cases of ovarian torsion misdiagnosed as purulent appendicitis, appendix perforation and early pregnancy, and 1 case of abdominal large vascular disease misdiagnosed as ectopic pregnancy. The causes of misdiagnosis mainly includes poor ultrasonic findings, non obvious signs, atypical ultrasonic features or similar diseases.
Conclusion:
The accuracy rate of Doppler ultrasound is high in the diagnosis of acute abdominal diseases. However, poor ultrasonic findings, non obvious signs and atypical ultrasonic features may cause misdiagnosis. Other ways can be used for differential diagnosis.

Value Analysis of Brain Natriuretic Peptide Combined with D-D in Diagnosis and Treatment of Patients with Acute Exacerbation of Interstitial Lung Disease

Objective:
To study and analyze the diagnostic value of brain natriuretic peptide combined with D- two (D-D) in patients with acute exacerbation of interstitial lung disease.
Methods:
From June 2015 to August 2016, 50 patients with acute exacerbation of interstitial lung disease were treated in our hospital. The death group was divided into 28 cases in the death group and 22 in the survival group. Brain natriuretic peptide and D-D levels were detected in all patients. Baseline data, brain natriuretic peptide and D-D levels, clinical symptoms and signs, serum IL-6 and C reactive protein levels were compared between the two groups.
Results:
There was no significant difference in the sex, weight, age and past medical history between the death group and the survival group (P > 0.05). The brain natriuretic peptide and D-D level in the death group were (744.6 + 248.2) ng/ml and (4245.5 + 1584.2) pg/ml respectively, which were all higher than those of the survival group (32.5 + 10.3) ng/ml and (425.3 + 242.5) pg/ml, all of which were significantly different (P < 0.05). Death group of patients with dyspnea, hemoptysis, fever, chest pain, syncope, lower extremity edema and lung wet incidence rale were 82.14% (23/28), 28.57% (8/28), 67.86% (19/28), 42.86% (12/28), 39.29% (11/28), 50.00% (14/28), 100.00% (28/28), were higher than in survival group 50.00% (11/22), 4.55% (1/22), 18.18% (4/22), 13.64% (3/22), 13.64% (3/22), 18.18% (4/22), 54.55% (12/28), all had significant differences (all P < 0.05). Death patients serum IL - 6 and C - reactive protein levels respectively was (13.1±2.6)ng/L, (137.8±11.6) mg/L, were higher than in survival group (6.5±3.5 )ng/L, (9.6±6.1) mg/L, all have significant differences (all P < 0.05).
Conclusions:
In clinical work, the brain natriuretic peptide and D-D level can be used to predict the acute exacerbation of interstitial lung disease, and can be known as an effective indicator of the prognosis of patients with acute exacerbation of interstitial lung disease.

Clinical Analysis of Nasal Septal Perforation Repair under Nasal Endoscope for Nasal Septal Perforation

Objective:
To analyze the clinical effect of nasal endoscopic perforation repair on patients with nasal septum perforation.
Methods:
60 patients with nasal septum perforation admitted to our hospital from May 2015 to May 2017 were randomly divided into control group (n = 30) and observation group (n = 30) according to the random number table method. Thirty patients in the control group were treated by conventional repair surgery. Thirty patients in the observation group were treated with endoscopic nasal septal mucosal flap repair. The clinical effects of the two groups were compared.
Results:
The success rate of repair in the observation group was 93.33% (28/30), while that in the control group was 73.33% (22/30), with significant difference (χ

Efficacy Analysis of two Different Surgical Treatments for Idiopathic Macular Hole

Objective:
To compare the effect of vitrectomy, internal boundary membrane (ILM) avulsion and overturn coverage combined with air filling (cover group) and vitrectomy, internal boundary membrane simple stripping combined with air filling (stripping group) for the treatment of idiopathic macular hole (IMH).
Methods:
For patients with IMH, the diameter of the foramen was smaller than 400 mu m, and the total diameter of the eyes was 30 eyes and the diameter of >400 hole was 42 >400. The best corrected visual acuity, the closure of hiatus, the rate of closure of the foramen and the complications were observed before and after operation.
Results:
In the patients with MH diameter less than 400 u m, the visual acuity of the two groups increased, the difference was not statistically significant (P=0.98). All the patients were closed after the operation, and there was no statistical difference between the two groups (P>0.05). The visual acuity of the two groups in the two groups was higher than that of the cover group (P=0.04), and the closure rate in the stripping group was higher than that of the cover group (P=0.022). There were no serious complications after operation in the two groups.
Conclusion:
Vitrectomy, internal boundary membrane cover combined with air filling can effectively treat IMH, but for patients with larger MH diameter and IV stage with split hole stage, it is suggested that inert gas should be filled to improve the success rate.

Effect of Kyphoplasty at Different Time Points on Osteoporotic Vertebral Compression Fractures

Objective:
To evaluate the clinical effect of PKP at different time on osteoporotic vertebral compression fractures.
Methods:
60 patients of osteoporosis vertebral compression fracture treated by PKP within 3 weeks after injury, another parallel 60 patients were treated by beyond 3 weeks. VAS score, ODI score, kyphotic angle and C7/SFD were recorded before and afer treatment.
Results:
There were no obvious difference of improvement of VAS and ODI between two groups, improvement of kyphotic angle and C7/SFD of group A were better than those of group B after treatment.
Conclusions:
PKP at different time can both improve symptoms and enhance the quality of life quickly; the earlier treatment, the more advantage in correcting the local fracture deformities.

Effect and of Batroxobin Combined with Ginkgo Biloba Injection on Sudden Deafness and Influence on Coagulation Function

Objective:
To explore the clinical efficacy and the effect of the injection of baotase combined with ginkgo biloba in patients with sudden deafness.
Methods:
A total of 143 patients with sudden deafness were randomly divided into control group (n = 71) and treatment group (n = 72). Both groups of patients were treated with ginkgo biloba injection. The treatment group was based on the basic therapy combined with the treatment of balthase, continuous treatment for 2 weeks. After treatment, 2 groups of clinical efficacy were calculated. To detect and compare the changes of the pure acoustic threshold and coagulation function of the two groups before and after treatment. The adverse reactions during the treatment of 2 groups were calculated.
Results:
The total effective rate of treatment group was 90.28%, significantly higher than 71.83% in the control group (P<0.01). Compared with the treatment before treatment, the pure acoustic threshold of the two groups was significantly reduced after treatment. The treatment group was significantly lower than the control group (P<0.01); After treatment, both PT and APTT were significantly shortened after treatment (P <0.05 or P<0.01). The treatment group was significantly shorter than the control group (P < 0.01); The serum FIB levels were significantly lower in the 2 groups after treatment, and the treatment group was significantly lower than the control group (P<0.01). The serum d-d levels were significantly higher in 2 groups (P <0.05 or P< 0.01), and the treatment group was significantly higher than the control group (P<0.01). The incidence of adverse reactions in the treatment group was 5.56%, which was significantly lower than that in the control group (P < 0.05).
Conclusion:
The combination of Batroxobin and ginkgo leaf injection can effectively improve hearing and coagulation function in patients with sudden deafness, and reduce the related adverse reactions effectively. The curative effect is significantly better than that of ginkgo leaf injection.

The control group were treated with transurethral resection of bladder tumor en bloc resection, application of laser assisted study group patients after transurethral resection of the bladder tumor en bloc resection. Postoperative operative time, intraoperative blood loss and time of indwelling catheter were compared between the two groups, including operative time, postoperative complications, postoperative recurrence rate and survival rate.
Results:
The operative time, intraoperative blood loss, indwelling catheter time and hospitalization time in the control group were significantly higher than those in the study group (P<0.05). The incidence and recurrence rate of postoperative complications in the control group were significantly higher than those in the study group (P<0.05). The study group survival rate was significantly higher than the control group (P<0.05).
Conclusion:
The application of laser assisted force in bladder tumor resection for bladder tumor can significantly improve the clinical effect. The incidence of complications is significantly reduced, the survival time is obviously improved and the risk of recurrence is low. It is less traumatic and worthy of further promotion in clinical.

Clinical Efficacy of Metoprolol Combined with Dopamine in Children with Severe Pneumonia Complicated with Heart Failure

Objective:
To investigate the clinical efficacy of metoprolol combined with dopamine in the treatment of severe pneumonia complicated with heart failure.
Methods:
140 cases of children with severe pneumonia combined with heart failure were selected from June 2015 to December 2016 and randomly divided into the control group and the observation group, 70 cases in each group. Basic supportive therapy and anti-infection treatment, correction of hypovolemia were given to both groups of patients, then equal amount of dopamine was given to both groups via intravenous drip, the observation group was given oral metoprolol treatment on the basis of control group, all the treatment lasted for 7 days. The total effective rate, changes of heart rate, breathing rate and blood oxygen saturation, CK-MB, LDH levels before and after the treatment, improvement time of clinical symptoms and signs were compared between two groups of patients.
Results:
After treatment, the total efficiency of observation group (92.86%) was significantly higher than that of the control group (74.29%) (P=0.00); the heart rate, respiratory rate, serum CK-MB, LDH levels after treatment were significantly lower than those before treatment, which were significantly lower in the observation group than those of the control group (P<0.05); the blood oxygen saturation of both groups after treatment were significantly higher than those before treatment, which was significantly higher in the observation group than that of the control group (P<0.05); the improvement time of symptoms and signs in observation group were shorter than those of the control group(P<0.05).
Conclusion:
Metoprolol combined with dopamine could effectively improve the symptoms and signs, shorten the time of clinical symptoms and signs in the treatment of severe pneumonia with heart failure in children.

Comparison of Clinical Efficacy and Quality of Life between Adult Plasma Adenoidectomy and Dynamic Adenoidectomy

Objective:
To compare the clinical efficacy and the quality of life of two kinds of surgical methods in the treatment of adult patients with adenoid ablation and dynamic adenoidectomy.
Methods:
60 cases of adult adenoidal hypertrophy patients were divided into 2 groups, plasma group and power systems group, the two groups were compared, the cost of surgery, intraoperative bleeding, postoperative bleeding volume, postoperative pain score, postoperative life quality score.
Results:
The plasma group had shorter operative time, less bleeding in the operation, less postoperative bleeding and less postoperative pain, but the operation cost was higher and the postoperative vitality was higher (P<0.05, the difference was statistically significant). There were no significant differences in the two groups of emotional function, social function, mental health, general health status, physiological function, physiological function and so on (P>0.05, the difference was not statistically significant).
Conclusion:
Plasma adenoid ablation is shorter than the power system, with less bleeding, less postoperative bleeding and less postoperative pain, but the cost of surgery is higher, which is worthy of clinical promotion.

Effects of Subclinical Hypothyroidism on Pregnancy and Outcome

Objective:
To analyze the sub-clinical hypothyroidism (SCH) on pregnancy and pregnancy outcomes.
Methods:
93 pregnant women (untreated group) and 42 pregnant women (treatment group) with SCH treated by L-T4 replacement therapy were enrolled in this study from October 2016 to October 2017 in our hospital. At the same time, Select the same period in our hospital for examination of 131 cases of thyroid function in pregnant women as the research object, comparative analysis of three groups of patients with pregnancy outcome.
Results:
(1) Compared with untreated group, the incidence of premature delivery, preeclampsia, fetal growth restriction, placental abruption, gestational hypertension and neonatal body weight were significantly lower in the treatment group than in the untreated group. There was a statistical difference (P<0.05); (2) In the untreated group and the control group, the incidences of premature delivery, preeclampsia, fetal growth restriction, and placental abruption were significantly lower in the control group than in the untreated group. Differences (P<0.05); (3) There was no statistical difference in incidence of preterm labor, preeclampsia, pregnancy-induced hypertension, placental abruption, neonatal weight loss, and fetal growth restriction between the treatment group and the control group. (P>0.05).
Conclusion:
SCH will lead to adverse pregnancy outcomes pregnant women, as soon as possible to give effective treatment can effectively improve pregnant women's pregnancy outcomes, reduce the incidence of complications.

Study on the Relationship Among Neutrophil Lymphocyte Ratio, Coronary Artery Disease and Gensini Score

Objective:
To investigate the relationship between neutrophil lymphocyte ratio,coronary artery disease and Gensini score.
Methods:
Patients were divided into coronary artery disease (CAD) group (123 cases) and Non-CAD group (60 cases) according to coronary angiography (CAG) results.The baseline data and blood biochemical data of all patients were collected.NLR and Gensini score were calculated and the relationship of them were analyzed.
Results:
The NLR were higher in CAD group (3.55±1.56) compared with Non-CAD group (2.41±0.90) (P<0.001) .Multivariate Logistic regression analysis showed that NLR (OR = 2.752, 95% CI:1.729-4.38) was an independent risk factors of CAD.The Pearson regression analysis showed that it was a positive correlation between Gensini score and NLR (r =0.426, P < 0.001).
Conclusion:
The level of NLR was closely related to CAD and correlated with the Gensini score representing the degree of coronary stenosis.

Methods:
107 cases of patients with type 2 diabetes hospitalized in affiliated hospital of chengde medical university were enrolled as objects of the study, the levels of vitamin D、MDA and SOD in the blood were detected.
Results:
The levels of vitamin D and SOD of contral group, microalbuminuria group and clinical proteinuria group were decreased,while the levels of MDA were on the rise, the differences were statistically significant(P<0.05)。 The levels of vitamin D and MDA were negtively correlated,and the levels of vitamin D and SOD were positively correlated.
Conclusion:
The levels of vitamin D and oxidative stress in type 2 diabetic nephropathy were negtively correlated.

Evaluation of Safety and Efficacy of CT Perfusion Imaging in Intravenous Thrombolytic Therapy for Acute Ischemic Stroke

Objective:
To explore the guiding role of CT perfusion imaging in the safety and efficacy of intravenous thrombolytic therapy for acute ischemic stroke.
Methods:
75 patients with acute middle cerebral artery ischemic stroke (6h) admitted in our hospital from October 2016 to December 2017 were selected as the subjects. The CBF ratio of 35 cases underwent CTP examination ranged from 0.20 to 0.35, suggesting reversible ischemia penumbra. The remaining 40 cases were examined by CTP without ischemic penumbra. The two groups were treated with rt-PA thrombolytic therapy. The bleeding conversion rate, mortality rate, NIHSS score and Barthel index score of 24h, 7d, 30d and 90d after thrombolysis in two groups were compared.
Results:
There were 1 case of hemorrhage conversion within 48 hours after thrombolytic therapy in IP group and 4 cases of hemorrhage conversion within 48 hours after thrombolytic therapy in no IP group,there were significant differences (P<0.05).There were no deaths in IP group after thrombolytic therapy, and there were 1 deaths in 14d after thrombolytic therapy in group IP, no significant difference (P=0.095).NIHSS scores and Barthel index scores after 24h, 7d, 30d and 90d after treatment in CTP group were better to control group and the difference was statistically significant (P<0.05).
Conclusion:
In the thrombolytic therapy of patients with acute ischemic stroke, according to the results of CTP examination, it is important to predict the risk of secondary hemorrhage after thrombolytic therapy, which is of great significance to the improvement and prognosis of neural function defects.

Effect of Valproate Combined with Lamotrigine on TNF- and IL-2 in Peripheral Blood of Patients with Primary Epilepsy

Objective:
To investigate the effect of combined use of sodium valproate and lamotrigine on peripheral blood levels of TNF and IL in patients with primary epilepsy and its efficacy and safety.
Methods:
The observation cases were 150 patients diagnosed as primary epilepsy in our hospital from March 2014 to March 2017. All the patients in the study were randomly divided into control group And observation group, with 75 cases in each group. The two groups were administered with valproate sodium alone and valproate combined with lamotrigine, and were followed up. The time of electroencephalogram (EEG) improvement, the proportion of abnormal discharge, the frequency of postoperative seizure, the score of SES, the levels of TNF-α and IL-2 in peripheral blood and the adverse reactions were compared between the two groups.
Results:
In terms of curative effect, the results of EEG improvement time and abnormal discharge proportion in the observation group were significantly lower than those in the control group (P <0.05; the frequency of seizures in the observation group at 3 months and 6 months was significantly less than that in the control group (P <0.05 =; The mean score of SES self-esteem in observation group was significantly higher than that in control group (P <0.05); the level of TNF-α and IL-2 in peripheral blood of the combination group was significantly lower than that of the control group (P <0.05) The incidence of adverse reactions in the observation group was significantly lower than the control group, higher safety.
Conclusion:
Sodium valproate in combination with lamotrigine has a good and stable efficacy in the treatment of epilepsy, in reducing blood levels of TNF-α and IL- 2 levels at the same time, the safety of its use is higher, the incidence of adverse reactions was significantly reduced.This method is effective and safe, it is worth promoting.

Effect of Sequential Therapy with Azithromycin Combined Shuanghuanglian Oral Liquid on the T Lymphocyte Subsets, Serum Inflammatory Cytokines Levels of Children with Mycoplasma Pneumonia

Objective:
To explore the effect of sequential therapy with azithromycin combined with shuanghuanglian oral liquid on the T lymphocyte subsets, serum interleukin-6 (IL-6), tumor necrosis factor (TNF)-α and high sensitivity C-reactive protein (hs-CRP) levels of children with mycoplasma pneumonia.
Methods:
172 cases of children with mycoplasma pneumonia in our hospital from March 2015 to December 2016 were selected. According to simple random method, all the patients were divided into two groups with 86 cases in each group. Sequential therapy with azithromycin were given to both groups and shuanghuanglian oral liquid was given to the observation group additionally. The clinical effect, T lymphocyte subsets, serum IL-6, TNF-α and hs-CRP levels before and after treatment were compared between two groups.
Results:
The total effective rate of observation group was 96.51%, which was significantly higher than that of the control group (88.37%) (P<0.05). The T lymphocyte subsets CD3+, CD4+ ratio, CD4+/CD8+ of both groups of children after treatment were significantly higher than those before treatment(P<0.01), the CD8+, serum IL-6, TNF-α and hs-CRP levels were significantly lower than those before treatment, and the improvement of the indicators mentioned above in the observation group were more obvious than those of the control group. Between groups, statistically significant at different times within the group (P<0.01). No significant difference was found in the incidence of adverse reactions between two groupsP>0.05).
Conclusion:
Azithromycin sequential therapy combined with Shuanghuanglian oral liquid could effectively improve the immune function, reduce inflammatory response with high safety for the children with mycoplasma pneumonia.

Effect of the Treatment of Chronic Periodontitis and Expressionl of IL-17 and IL-35 in Serum and Gingival Crevicular Fluid

Objective:
To investigate the effect of the combination of IL-17 and IL-35 in the treatment of chronic periodontitis and the expression of serum and gingival crevicular fluid.
Methods:
130 patients of chronic periodontitis received therapy from Jan 2014 to Dec 2016 in our hospital were selected as the research object. All the patients were randomly divided into observation group and control group, each group of 65 cases. The control group was given tinidazole treatment, the observation group was treated with periocline ointment on this basis. After 4 weeks of treatment, the clinical efficacy, gingival index (GI), plaque index (PLI), periodontal attachment (PAL), periodontal pocket depth (PPD), tooth mobility (MD), Interleukin-17 (IL-17), interleukin-35 (IL-35) and adverse reactions were compared. Results: After treatment, the observation group, the total effective rate was higher than the control group (92.31% vs 73.85%, P<0.05); The GI, PLI, PAL, PPD and MD in the observation group were significantly lower than those in the control group(P<0.05); The levels of IL-17 and IL-35 in gingival crevicular fluid in observation group were lower than those in control group(351.64±41.73 vs 394.35±46.17, 100.44±7.19 vs 143.25±9.64, P<0.05); The levels of IL-17 and IL-35 in the observation group were lower than those in the control groupf[(16.05±1.07)vs(20.64±1.83)ng/L,(100.44±7.19) vs (258.17±16.94)ng/ml](P<0.05); The total incidence of adverse reactions in the observation group was lower than that in the control group (6.15% vs 18.46%, P<0.05).
Conclusion:
It can effectively relieve the clinical symptoms, improve the expression of IL-17 and IL-35 in gingival crevicular fluid, improve the immune function of the body, and have less adverse reactions. It is worth popularizing.

Results:
The application of Rabeprazole sodium enteric-coated tablets in outpatients department was more appropriate, but there were still some unreasonableness, such as dosage of outpatient prescription was more than 7 days, indications and usage were not appropriate.
Conclusion:
Rabeprazole sodium enteric-coated tablets has high clinical value, social benefit and economic benefit, but it is necessary to pay attention to the indications and usage in the course of use.

Objective:
To investigate the effects of atomization inhalation of salbutamol and budesonide on pulmonary function in children with bronchiolitis.
Methods:
A total of 220 children with bronchiolitis hospitalized in two families of Langfang People's Hospital from November 2013 to November 2016 were selected and divided into the observation group and the control group, with 110 cases in each group. The clinical indicators, clinical pulmonary function and curative effect of the two groups were compared.
Results:
After treatment, the observation group was relieved of asthma, cough disappeared, lung rale disappeared and hospitalization time, compared with the control group, there were significant differences (P<0.05). Before treatment, there was no significant difference in pulmonary function between the two groups (P > 0.05).After treatment, the Ti /Te(%)、VPEF /VE(%)、TPTEF /TE(%)、Vt /kg(ml/kg) in the observation group was significantly increased with the control group (P<0.05). After treatment in the observation group and the control group, the remarkable effect was (30.91%VS 21.82%); the effective was (56.36%VS 50.00%); the invalid was (12.73%VS 28.18%); the total effective rate was (87.27%VS 71.82%), there was significant difference(χ

=10.208,P=0.013). No adverse reaction was found in all two groups. The liver and kidney function and routine hematuria were normal after treatment.
Conclusion:
3% hypertonic saline atomization combined with salbutamol and budesonide is effective in the treatment of bronchiolitis, significantly improving the lung function of the children, and the safety is better.

Change and Significance of Levels of Serum Th1/Th2 Cytokines and AFP before and after TACE in Advanced PHC Patients

Objective:
To explore the levels changes of serum Th1/Th2 cytokines and AFP in patients with advanced hepatocellular carcinoma (PHC) before and after transcatheter arterial chemoembolization (TACE) and their clinical significance.
Methods:
95 cases of PHC patients hospitalized in our hospital were divided into the TACE group (n=58, given TACE treatment) and the venous group (n=37, given intravenous chemotherapy). The lesion reduction degree and the levels of serum Th1/Th2 cytokines and AFP before and after TACE were compared between the two groups.
Results:
The total reduction rate of lesion in the TACE group was significantly higher than that in the venous group (P<0.05). The serum AFP level, the AFP positive rate and the levels of serum interleukin-4 (IL-4) and interleukin-10 (IL-10) were significantly decreased after treatment in the two groups (P<0.05) while the levels of interferon-γ (IFN-γ) and interleukin-2 (IL-2) were significantly increased (P<0.05). The levels changes of serum AFP and Th1/Th2 cytokines in the TACE group were higher than those in the venous group (P<0.05). The positive expression rate of IL-10 in the TACE group was significantly lower than that in the venous group (P<0.05).
Conclusions:
TACE treatment can effectively control the disease condition of advanced PHC and regulate Th1/Th2 balance disorders, and the levels of serum Th1/Th2 cytokines and AFP can be used as important indicators for efficacy evaluation.

Effect of Mobile Management Platform on TCM Symptom Score in Type 2 Diabetes Mellitus

Objective:
To evaluate the effect of mobile management platform on TCM symptom score in type 2 diabetes mellitus (T2DM).
Methods:
140 T2DM patients admitted into the endocrinology department of Tongzhou district Chinese Medicine Hospital of Beijing from January 2015 to May 2016 were selected, and they were randomly divided into two groups, 70 cases in each group. Control group was given routine blood glucose therapy; intervention group was given management in mobile management platform based on control group. The clinical efficacy of two groups were compared. The changes of TCM symptom scores before intervention and after intervention for 3 months were observed.
Results:
The total effective rate of intervention group was 92.86% (65/70), which was higher than 77.14% (54/70) of control group (P<0.05); The total effective rate of qi-yin deficiency in intervention group was higher than that in other differentiation (P<0.05); After intervention for 3 months, the TCM symptom score of intervention group was (10.84±2.32) points, which was lower than (15.97±3.95)points of control group (P<0.05).
Conclusion:
Mobile management platform in T2DM patients can decrease the TCM symptom score, control the level of blood glucose, help medical staff closely monitor blood glucose of patients, develop the management ability of medical staff.

Results:
Total efficiency of the treatment group was 86.67%, total efficiency of the control group was 68.33%, the difference was statistically significant (P <0.05); LH, T, after treatment of two groups were reduced compared with before treatment, the differences were statistically significant (P <0.05), FSH, E2 were increased compared with before treatment, the differences were statistically significant (P <0.05), T, E2 and LH / FSH ratio after treatment of two groups had statistically significant (P <0.05); BBT recovery rate after treatment of the treatment group was 68.33%, recovery rate of the control group was 36.67% (P<0.05); Menstrual cycle, dizziness, tinnitus, waist and knee pain, loss of libido, etc of two groups were statistically significant (P <0.05); Total efficiency of TCM symptoms of the treatment group was 78.33% (47 cases), total efficiency of the control group was 55.00% (33 cases) (P <0.05).
Conclusion:
In this study, the method of invigorating kidney and removing stasis and activating blood circulation is a great method to make the body achieve the balance of yin and Yang, the flow of Qi and blood, and the coordination of the function of the viscera. It can obviously improve the clinical symptoms and TCM Syndromes of the patients. It is worth popularizing in clinical practice.

Effect of Outpatient Health Education Management on Promoting Self-management of Elderly Patients with Asthma

Objective:
To explore the effects of outpatient health education management on promoting self-management of elderly patients with asthma.
Methods:
280 elderly patients with asthma admitted to our clinic from June 2016 to June 2017 were randomly divided into two groups. The control group received routine outpatient procedures while the management group received outpatient health education management. The interventions were compared between the two groups After nearly 6 months of living conditions, self-management ability, quality of life and disease prognosis.
Results:
The FEV1%> 80% (62.14%) of the patients in the management group were significantly higher than those in the control group (27.86%) in the last 6 months, 15% in the last 6 months, 5% in the last 6 months (2.86%) and tobacco exposure rate (6.43%) were significantly lower than those in the control group (35.71%, 17.14%, 13.57%, 26.43%). After 6 months of intervention, the self-management ability score 7.43), total quality of life (193.78 ± 22.60), ACT score (21.40 ± 1.66) and FEV1 level (2.90 ± 0.72) were significantly higher than the control group (78.11 ± 7.85, 172.75 ± 20.97, 19.69 ± 1.40, 2.10 ± 0.68), there was a significant difference (P <0.01).
Conclusion:
Outpatient health education management has a significant effect on promoting self-management of senile asthma patients.

Analysis of Early Comprehensive Nursing Measures for Elderly Patients with Pulmonary Infection and Heart Failure

Objective:
To investigate the early comprehensive nursing measures of elderly patients with pulmonary infection and heart failure.
Methods:
156 patients with pulmonary infection and heart failure from January 2015 to December 2016 were selected as the subjects. Random numbers were used as the grouping principle to divide them into two groups: control group and study group example. The control group was treated with routine nursing. On the basis of the control group, the study group took the early comprehensive nursing measures immediately after the diagnosis. The changes of clinical symptoms and signs, the improvement of cardiac function after nursing intervention and the quality of life at 3 months were compared between the two groups.
Results:
The rales of the study group disappeared, the sputum disappeared or decreased, the body temperature returned to normal, the time of sputum culture and the peripheral white blood cell count returned to normal time were lower than the control group (P<0.05). The effective rate of improvement of cardiac function was 71.79% in the study group, which was higher than that in the control group 55.13%(P<0.05). The social function, body pain, body function, mental health, vitality, affective role and overall health score of the SF-36 scale were higher in the study group than in the control group at 3 months (P<0.05).
Conclusion:
Early diagnosis and treatment of elderly patients with pulmonary infection and heart failure is satisfactory and worthy of clinical promotion.

Status and Drug Resistance of Multidrug-resistant Bacteria in a Three Grade A Hospital from 2014 to 2016

Objective:
To analyze the infection current status and drug resistance of multiple drug resistant organism (MDRO) in a grade A tertiary hospital from 2014 to 2016.
Methods:
Samples of urine, sputum, blood and secretions were collected from hospitalized patients of a grade A tertiary hospital from 2014 to 2016, and their multi-drug resistant strains were isolated and the susceptibility test was given.
Results:
There were 2365 strains of MDRO, 1002 strains (42.37%) of sputum, 484 strains (20.47%) of urine, 295 strains (12.47%) of secretions and 250 strains (10.57%) of blood among 53364 examined samples. There were 805 strains (34.04%) in 2014, 750 strains (31.71%) in 2015 and 810 strains (34.25%) in 2016. And there were mainly ECO with 1323 strains (55.94%). The distribution rate of MDRO was the highest in ICU ward and 453 strains (19.15%) were detected. Staphylococcus aureus was highly resistant to penicillin, oxacillin and ampicillin and was sensitive to vancomycin, teicoplanin and linezolid. Escherichia coli was highly resistant to ampicillin, ciprofloxacin and levofloxacin and was sensitive to the imipenem, amikacin and piperacillin/tazobactam. Klebsiella pneumoniae was highly resistant to ciprofloxacin and cotrimoxazole and was sensitive to imipenem and piperacillin/tazobactam. Acinetobacter baumannii was highly resistant to the ceftriaxone, ciprofloxacin and imipenem. Pseudomonas aeruginosa was highly resistant to the cefuroxime and ceftriaxone and was more sensitive to ampicillin.
Conclusions:
The MDRO detection rate is higher in the hospital, and we should strictly control the clinical abuse of antimicrobial drugs in departments, and strengthen the monitoring of MDRO.
