Efficacy of Nasal Endoscopic Messerklinger Surgery on Fungal Rhinosinusitis and Its Effects on Serum Level of Inflammatory Factors of Patients

Objective:
To explore the efficacy of nasal endoscopic Messerklinger surgery on fungal rhinosinusitis (FRS) and its effects on serum level of inflammatory factors of patients.
Methods:
116 patients with FRS admitted to our hospital from June 2017 to June 2019 were selected as the analysis subjects,and they were divided into observation group and control group with the simple random grouping method,with 58 cases in each group.The patients in control group were treated with traditional Caldwell-Luc surgery,and the patients in observation group were treated with nasal endoscopic Messerklinger surgery.The clinical efficacy of the two groups was compared,and the incidence rates of postoperative complications (local infection,nasal mucosal redness,blood in the nose,facial numbness,toothache) were recorded.The levels of serum inflammatory factors [interleukin-6 (IL-6),interleukin-12 (IL-12),tumor necrosis factor-α (TNF-α)],eosinophil cationic protein (ECP) and total immunoglobulin E (TIgE) were observed in the two groups before surgery and at 1 month after surgery.
Results:
The total clinical effective rate in observation group was higher than that in control group (100.00% vs 93.10%) (P>0.05).The incidence rates of postoperative complications in observation group were significantly lower than those in control group (P<0.05).At 1 month after surgery,the levels of serum inflammatory factors IL-6,IL-12,TNF-α,ECP and TIgE in the two groups were significantly lower than those before surgery,and the levels in observation group were significantly lower than those in control group during the same period (P<0.05).
Conclusion:
Nasal endoscopic Messerklinger surgery has significant efficacy in treating FRS,and it can effectively reduce the incidence rates of postoperative complications,relieve the inflammation level,and reduce the serum ECP and TIgE levels.

Mechanisms of Bulleyaconitine A Relieving Pain in Rats with Neuropathic Pain by Regulating TRPV1 Receptor Function

Objective:
To investigate mechanisms of bulleyaconitine A relieving pain in rats with neuropathic pain by regulating the transient receptor potential vanilloid type 1(TRPV1) receptor function.
Methods:
100 SD rats were divided into control group,model group,dexamethasone group,low and high dosage groups of bulleyaconitine A;model group,dexamethasone group,low and high dosage groups of bulleyaconitine A to establish neuropathic pain model.The rats in the dexamethasone group (40mg/kg) and the low (40mg/kg) and high (80mg/kg) dosage groups of bulleyaconitine A were given intrathecal injection (Injection volume:1.0mL/100g) on the first day of modeling and continuously for 3 weeks.The control group and the model group received intrathecal injection for the same volume of physiology Brine.At the end of the experiment,the mechanical pain threshold,thermal pain threshold,and pain-weighted score were determined.The expression of transient receptor potential vanilloid type 1 (TRPV1) mRNA and protein in spinal cord tissue was determined by real-time fluorescence reverse transcription and immunohistochemistry.Determination of Fas,Caspase-3 and Caspase-6 in spinal cord tissue by immunosorbent assay.
Results:
Compared with the control group,the mechanical pain threshold and thermal pain threshold of the model group were decreased in the model group,and PIS score,TRPV1 mRNA and protein,Fas,Caspase-3,and Caspase-6 protein levels were increased (P<0.05).Compared with the model group,the mechanical pain threshold,thermal pain threshold increased,PIS score,TRPV1 mRNA and protein,Fas,Caspase-3,Caspase-6 protein levels were decreased in the dexamethasone group and low and high dosage groups of bulleyaconitine A,and the levels of PASP were decreased (P<0.05).The mechanical pain threshold and thermal pain threshold were higher in the high-dose group than those in the low-dose group (P<0.05).The PIS score,TRPV1 mRNA and protein,Fas,Caspase-3,and Caspase-6 protein levels were lower than those in the high-dose group than in the low-dose group (P<0.05);Compared with dexamethasone group,the mechanical pain threshold and thermal pain threshold were lower in the low and high dosage groups of bulleyaconitine A (P<0.05),PIS score,TRPV1 mRNA and protein,Fas,Caspase-3,Caspase-6 protein levels increased (P<0.05).
Conclusion:
The mechanism is related to the inhibition of TRPV1 receptor function and neuronal apoptosis of spinal cord by bulleyaconitine A,which can alleviate neuropathic pain in rats.

Correlation Between Preoperative Expressions of Serum E-Selectin ICAM-1 and MMP-2 and Prognosis of Patients After Lung Cancer Surgery

Objective:
To explore the correlation between preoperative expressions of serum E-selectin,intercellular adhesion molecule-1 (ICAM-1) and matrix metalloproteinase-2 (MMP-2) and prognosis of patients after lung cancer surgery.
Methods:
Ninety-five patients with lung cancer treated in our hospital from January 2015 to December 2016 were selected,and 80 healthy people for physical examination at the same period were selected as control group.The levels of serum E-selectin,ICAM-1 and MMP-2 were detected among patients with lung cancer before surgery,at 1 week after surgery and at 1 month after surgery and in control group on the day of physical examination.The patients were followed up for more than 3 years,and the survival status was recorded.Receiver operating characteristic curve (ROC) and area under the curve (AUC) were used to analyze the predictive value of preoperative serum E-selectin,ICAM-1 and MMP-2 on prognosis.Multivariate analysis affecting the prognosis of lung cancer patients was performed by COX proportional hazard regression model.Taking the median value of serum results as the cut-off point,the patients were divided into low expression group (loss than median value,47 cases) and high expression group (more than median value,48 cases),and the survival time was recorded,and Kaplan-Meier survival curve was plotted.
Results:
The levels of serum E-selectin,ICAM-1 and MMP-2 in lung cancer group before surgery,at 1 week after surgery and at 1 month after surgery showed a decreasing trend (P<0.05),and were higher than those in control group (P<0.05).The levels of preoperative serum E-selectin,ICAM-1 and MMP-2 in death group were higher than those in survival group (P<0.05).The AUC values of preoperative serum E-selectin,ICAM-1 and MMP-2 in predicting 3-year death were 0.869,0.830 and 0.808 respectively,and they were lower than that of combined detection of 0.972 (P<0.05).COX proportional hazard regression analysis results showed that preoperative serum E-selectin,ICAM-1 and MMP-2 were independent factors for prognosis of patients after lung cancer surgery (P<0.05).The 3-year survival rates of preoperative serum E-selectin,ICAM-1 and MMP-2 low expression groups were 21.28%,23.40% and 23.40%,which were higher than those of high expression groups with 10.42%,8.33% and 8.33% (P<0.05).
Conclusion:
Levels of serum E-selectin,ICAM-1 and MMP-2 in patients with lung cancer before and after surgery are higher than those in healthy people,and the three preoperative indexes are independent factors that affect the prognosis,and have a high predictive value on the 3-year survival of patients,and patients with low expressions of preoperative serum indexes have longer survival time and better prognosis.

The Relationship Between HSP70 TNF-α IL-4 and Humoral Immunity of Hypertensive Patients During Pregnancy

Objective:
To analyze the relationship between heat shock 70 KDa protein (HSP70),tumor necrosis factor-α(tnf-),interleukin-4(IL-4) and humoral immunity in hypertensive patients during pregnancy.
Methods:
108 patients with gestational hypertension treated in hospitals from December 2016 to June 2019 were selected as observation group,and 108 pregnant women in the same period,the control group.Observe the humoral immune indexes and serum HSP70,TNF-α,IL-4 differences between the two groups,and to analyze the correlation between HSP70,TNF-α,IL-4 and humoral immunity in hypertensive patients during pregnancy.
Results:
There was significant difference in humoral immunity between observation group and control group (P<0.05).There was significant difference in serum HSP70,TNF-α and IL-4 levels between the observation group and the control group (P<0.05).The correlation analysis showed that there was a close correlation between HSP70,TNF-α,IL-4 and humoral immunity in patients with gestational hypertension (all P<0.05).
Conclusion:
There is a positive correlation between humoral immune level and HSP70,TNF-α,IL-4 in hypertensive patients during pregnancy,which can screen high-risk pregnant women through HSP70,TNF-α,IL-4 examination,so as to give effective intervention measures as soon as possible to protect the health and safety of mother and child.

Expression of Mir-125b and PIGF in Villus Tissue and Serum of Women Suffering Early Abortion and Its Significance

Objective:
To study the expression and significance of microRNA-125b (mir-125b) and placental growth factor (PIGF) in villi and serum of women with early abortion.
Method:
105 cases of early spontaneous aborted women admitted to our hospital from July 2017 to October 2019 were selected as the research objects,and 105 cases of induced abortion in the same period were selected as the control group.Peripheral venous blood serum was collected and chorionic tissue was separated from all abortion tissues.Real-time quantitative PCR was used to detect the mRNA expression levels of miR-125b and PIGF in serum and villus tissue,and immunoblotting (WB) was used to detect the protein expression levels of PIGF in villus tissue.The serum PIGF level was detected by enzyme linked immunosorbent assay (ELISA),and the correlation of mir-125b and PIGF protein levels in serum and chorionic tissue was analyzed by Pearson method.The predictive value of serum mir-125b and PIGF levels in patients of spontaneous abortion in early pregnancy was analyzed by receiver operating characteristic (ROC).
Result:
Compared with the control group,the level of mir-125b in chorionic tissue of pregnant women in spontaneous abortion group increased significantly,and the level of PIGF mRNA and protein decreased significantly (P<0.05);The level of miR-125b in serum increased significantly and the level of PIGF decreased significantly;The difference was statistically significant (P<0.05).Pearson analysis showed that there was a negative correlation between mir-125b level and PIGF protein level in chorionic tissue,serum of spontaneous abortion pregnant women (r=-0.643,P<0.05).Serum levels of mir-125b and PIGF were 0.896 (95% CI:0.850-0.943) and 0.884 (95% CI:0.838-0.929) respectively for predicting the area under curve (AUC) of spontaneous abortion in early pregnancy.The sensitivity and specificity of mir-125b (>1.630) in predicting spontaneous abortion were 95.30% and 84.60%,respectively.The sensitivity and specificity of PIGF (<42.553 pg/mL) in predicting spontaneous abortion were 90.60% and 77.90%;the AUC,specificity and sensitivity of combined detection for predicting spontaneous abortion in early pregnancy were 0.954,92.30% and 89.60%,respectively.
Conclusion:
The levels of mir-125b and PIGF in chorionic tissues of women suffering early spontaneous abortion are up-regulated,and they are negatively correlated,which may have some predictive value for the occurrence of spontaneous abortion.

Expression and Significance of Linc00324 and Th17 Cytokines in Serum of Patients with Atopic Dermatitis

Objective:
To detect the expression of serum long non-coding RNA 00324 (Linc00324) and T helper cell 17(Th17) cytokine in patients with atopic dermatitis and to explore its role and relationship in the pathogenesis of atopic dermatitis.
Methods:
80 patients with atopic dermatitis confirmed by pathology in our hospital from October 2017 to November 2019 were selected as the study objects,which were divided into 26 patients with mild atopic dermatitis,24 patients with moderate atopic dermatitis,30 patients with severe atopic dermatitis,and 35 healthy people were selected as the control group.The expression of Linc00324 in serum was detected by real-time fluorescent quantitative PCR (qRT PCR).The levels of interleukin-4(IL-4) and interleukin-10(IL-10) in Th17 cells were detected by enzyme-linked immunosorbent assay (ELISA),and Pearson method was used to analyze the correlations between serum Linc00324 and the expression levels of Th17 cytokines IL-4,IL-10 in patients with atopic dermatitis.
Results:
The levels of serum Linc00324,Th17 cytokines IL-4 and IL-10 in patients with atopic dermatitis were significantly higher than those in the control group (P<0.05),the levels of serum Linc00324,Th17 cytokines IL-4 and IL-10 in severe group and moderate group were significantly higher than those in mild group (P<0.05),and the levels of serum Linc00324,Th17 cytokines IL-4 and IL-10 in severe group were significantly higher than those in moderate group (P<0.05).The expression of Linc00324 in serum of patients with atopic dermatitis was positively correlated with the levels of Th17 cytokines IL-4 and IL-10 (r = 0.516,r = 0.498,P<0.05).
Conclusion:
Linc00324 is highly expressed in the serum of patients with atopic dermatitis,the levels of Th17 cytokines IL-4 and IL-10 are significantly higher in patients with atopic dermatitis,which are closely related to the occurrence and development of atopic dermatitis.It is suggested that Linc00324 can be used as a potential biological index for the early diagnosis and assessment of atopic dermatitis with important clinical significance.

Methods:
A total of 256 neonates with hyperbilirubinemia admitted to the hospital from June 2018 to June 2019 were selected as the observation group.Another 100 healthy neonates born in the hospital during the same period were selected as the control group.According to the total bilirubin (TBIL) level,patients in the observation group were divided into the mild group (TBIL level between 221.00 and 256.50μmoL/L,130 cases),the moderate group (TBIL level between 256.50 and 342.00μmoL/L,92 cases) and the severe group (TBIL level higher than 342.00 μmoL/L,34 cases).The levels of serum NSE,UCB,β

-MG and BUN were compared among groups.Analysis was performed to analyze the correlation between serum NSE,UCB,β

Conclusion:

Intervention Effect of Cyclophosphamide on Lung Peroxidation Injury Induced by Paraquat in Rabbits

Objective:
To investigate the effect of cyclophosphamide on lung peroxidation injury of rabbits with paraquat poisoning.
Methods:
40 healthy rabbits were randomly divided into control group,model group,methylpolygonon group and cyclophosphamide+methylpolygonon group,with 10 cases in each group.Paraquat was given to the model group,methylpolygonon group and cyclophosphamide + methylpolygonon group.The methylprednisolone group was treated with methylprednisolone,the cyclophosphamide + methylprednisolone group was treated with cyclophosphamide and methylprednisolone,and the control group and model group were given normal saline.
Results:
After 1 day of treatment,the levels of TGF - β 1 and MDA in model group,methylprednisolone group and cyclophosphamide + methylprednisolone group were higher than those of control group,while serum SOD level was lower than that of control group (P<0.05).After 3 days,5 days and 7 days of treatment,the levels of serum TGF - β 1 and MDA in methylprednisolone group and cyclophosphamide + methylprednisolone group were lower than those of model group,while serum SOD level was higher than that of model group (P<0.05).The levels of serum TGF - β 1 and MDA in the cyclophosphamide + methylprednisolone group were lower than those in the methylprednisolone group after 3 days,5 days and 7 days of treatment,while the serum SOD level was higher than that of the methylprednisolone group (P<0.05).
Conclusion:
Cyclophosphamide can effectively reduce the expression of serum TGF-β
1
and MDA and increase the activity of SOD,so as to improve the peroxidation damage caused by paraquat.

Effect of Continuous bedside Hemofiltration on Serum AST cTnI AMS and Serum Inflammatory Factors in Patients with Acute Organophosphorus Pesticide Poisoning

Objective:
To explore the continuous bedside hemofiltration (continuous renal replacement therapy,CRRT) on levels of serum aspartate aminotransferase (AST),troponin I (cTnI),serum amylase (AMS) and serum inflammatory factors in patients with acute organophosphorus pesticide poisoning (AOPP).
Methods:
The clinical data of 132 AOPP patients admitted between January 2009 and December 2019 were retrospectively analyzed.The patients were divided into observation group (n=64) and control group (n=68) according to blood purification methods.Control group was treated with hemoperfusion (HP),and observation group was combined with CRRT on this basis.The treatment effects (case fatality rate) and recovery process (atropine dosage,atropinizationtime,awake time,cholinesterase recovery time,mechanical ventilation time,length of hospital stay) were compared between the two groups.The organ damage (AST,cTnI,AMS) and serum inflammatory factors [interleukin 6 (IL-6),tumor necrosis factor-α (TNF-α),hypersensitive C-reactive protein (hs-CRP)] were compared between the two groups before treatment and at 12h and 24h of treatment.The incidence rates of complications were recorded in the two groups.
Results:
①There was no statistically significant difference in case fatality rate between the two groups (P>0.05).The atropine dosage,atropinization time,awake time,cholinesterase recovery time,mechanical ventilation time and hospital stay in observation group were all less than those in control group (P<0.05).②Before treatment,at 12h of treatment and at 24h of treatment,the levels of AST,cTnI,AMS,IL-6,TNF-α and hs-CRP in the two groups showed a downward trend (P<0.05),and the AST,cTnI and AMS in observation group at each time point were lower than those in control group (P<0.05),and there were interaction effects between AST,cTnI and AMS and time among different treatment methods (P<0.05).③The incidence rate of rebound phenomenon in observation group was lower than that in control group (P<0.05),and there were no significant differences in the incidence rates of intermediate syndrome,lung infection,multiple organ dysfunction and gastrointestinal bleeding between the two groups (P>0.05).
Conclusion:
CRRT can improve the levels of AST,cTnI and AMS in AOPP patients,and it is beneficial to improving the efficacy of AOPP patients to a certain extent.

Concentrated Growth Factor Promotes Angiogenic Potential of Endothelial Cells in Vitro

Objective:
To study the effect of concentrated growth factor (CGF) on the apoptosis of MLO-Y4 osteocyte-like cells.
Methods:
CGF was prepared from the venous blood of healthy volunteers.The model of ischemia-reperfusion in vitro was duplicated and divided into control and experimental groups with different concentrations (including 1%CGFe,5% CGFe,10% CGFe).The expression levels of Bax,Bcl-2 and p53 mRNA were detected by real-time fluorescence quantitative polymerase chain reaction (qRT PCR) to evaluate the degree of MLO-Y4 osteocyte-like cells apoptosis.
Results:
CGF could significantly inhibit the increase of caspase-3 activity of MLO-Y4 osteocyte-like cells induced by ischemia-reperfusion,with the decrease of Bax,Bcl-2 and p53 mRNA levels.
Conclusion:
CGF can improve the apoptosis of human umbilical vein endothelial cells induced by ischemia-reperfusion.

Effects of Telmisartan Bisoprolol Combined with Trimetazidine on Plasma BNP and Fg Levels and Cardiopulmonary Function in Patients with COPD Complicated with CHD

Objective:
To explore the effects of telmisartan,bisoprolol combined with trimetazidine on plasma B-type natriuretic peptide (BNP) and fibrinogen (Fg) levels and cardiopulmonary function in patients with chronic obstructive pulmonary disease (COPD) complicated with coronary heart disease (CHD).
Methods:
164 patients with COPD and CHD admitted to our hospital from March 2014 to March 2019 were selected,numbered according to the order of admission and randomly divided into study group and control group by sealed envelope method.82 cases in study group were given telmisartan + bisoprolol,and 82 cases in control group were treated with telmisartan,bisoprolol+trimetazidine.The changes of lung function indexes,cardiac function indexes,plasma BNP and Fg levels and serum inflammatory related indexes were compared between the two groups.
Results:
After treatment,the PEF,FVC and FEV1/FVC were significantly increased in the two groups (P<0.05),and the increases in study group were higher than those in control group (P<0.05).The LVEF in the two groups was significantly increased (P<0.05) while the LVEDD and LVESD were significantly decreased (P<0.05),and the changes in study group were greater than those in control group (P<0.05).The plasma BNP and Fg levels were significantly decreased in the two groups (P<0.05),and the decreases in study group were greater compared with those in control group.The serum levels of NF-κB,Chemerin,TGF-β and MMP-9 were significantly decreased in the two groups (P<0.05),and the decreases in study group were greater than those in control group (P<0.05).
Conclusion:
Telmisartan,bisoprolol combined with trimetazidine has exact efficacy in the treatment of COPD with CHD,and can significantly improve cardiopulmonary function,reduce plasma BNP and Fg levels,and inhibit synthetic release of serum NF-κB,Chemerin,TGF-β and MMP-9,and reduce the body’ s inflammatory damage.

Clinical Significance and Expression of Cx43 and p70S6K in Gastric Cancer

Objective:
To investigate the expression of connexin 43(Cx43) and p70S6 kinase (p70S6K) in gastric cancer tissues and their correlation with the clinicopathological parameters.
Methods:
The protein expression of Cx43 and p70S6K in gastric cancer and normal gastric mucosa was detected by Western blot assay.The mRNA expression of Cx43 and p70S6K genes was detected by quantitative real-time PCR.
Results:
At the protein level,the expression of Cx43 in gastric cancer tissues M(0.39,0.98) was lower than that in normal gastric mucosa tissues M(1.62,3.80),and the expression level of p70S6K M(0.94,1.55) was higher than that in normal gastric mucosa tissues M(0.54,1.02),and the differences were statistically significant (all P<0.01).At the gene level,the positive expression of Cx43 mRNA in gastric cancer tissues M(0.33,0.66)was lower than that in normal gastric mucosa tissues M(1.00,1.00),and the positive expression of p70S6K mRNA in gastric cancer tissues M(2.14,4.14) was significantly higher than that in normal gastric mucosa tissues M(1.00,1.00),and the differences were statistically significant (all P<0.01).No matter at the protein level or gene level,with the worse differentiation of gastric cancer,the higher the TNM stage,the deeper the infiltrating muscle layer,and the lower the positive expression rate of Cx43 in lymph node metastasis;on the contrary,the positive expression rate of p70S6K increased significantly (all P<0.05),regardless of gender and age (all P>0.05).There was a negative correlation between Cx43 and p70S6K protein expression in gastric cancer tissues (r=-0.746,P<0.01).There was a negative correlation between Cx43 and p70S6K mRNA expression (r=-0.759,P<0.01).
Conclusion:
The abnormal expression of Cx43 and p70S6K in gastric cancer may play an important role in the occurrence and development of gastric cancer.

Relationship between MGMT and Tiam1 Expression in Patients with Brain Metastases from NSCLC Treated with Concurrent Intensity-modulated Radiotherapy Combined with Temozolomide

Objective:
To analyze the relationship between the efficacy of concurrent radiotherapy combined with temozolomide and the expression of O6 methylguanine DNA methyltransferase (Mgmt) and invasion and metastasis inducing factor 1 (Tiam1) in patients with brain metastases from non-small cell lung cancer (NSCLS).
Methods:
60 NSCLC patients with brain metastasis were treated with simultaneous modulated accelerated radiotherapy and temozolomide,which of the expression of MGMT and Tiam1 in lung tissue was detected by immunohistochemistry and the relationship between the expression of MGMT and Tiam1 and the therapeutic effect and the survival time were analyzed.
Results:
The relief rate (RR) of 21 patients with MGMT (+) and Tiam1 (+) was 76.19%,the disease control rate (DCR) of which was 95.23%;The RR of 16 patients with MGMT (+) and Tiam1 (-) were 25.00%,the DCR of which was 81.25%;The RR of 17 patients with MGMT (-) and Tiam1 (-) were 88.24%,the DCR of which was 94.12%;The RR of 6 patients with MGMT (-) and Tiam1 (-) were 33.33%,and the DCR of which was 66.6% 7%.The follow-up time of 60 patients ranged from 1 to 45.35 months,the median follow-up time was 16.75 months and the median survival time was 13 months,where the 1-year,2-year and 3-year survival rates were 75.01%,53.86% and 25.31%,respectively.The median survival time of 21 patients with MGMT (+) and Tiam1 (+) was 13 months,where the 1-year,2-year and 3-year survival rates were 64.32%,45.15% and 20.33%,respectively.The median survival time of 16 patients with MGMT (+) and Tiam1 (-) was 6 months,where the 1-year,2-year and 3-year survival rates were 33.69%,20.01% and 0.00%,respectively.The median survival time of 17 patients with MGMT (-) and Tiam1 (+) was 15 months,where the 1-year,2-year and 3-year survival rates were 92.05%,74.12% and 40.09%,respectively.The median survival time of 6 patients with MGMT (-) and Tiam1 (-) was 10 months,where the 1-year,2-year and 3-year survival rates were 55.63%,47.35% and 18.96%,respectively.
Conclusion:
The different expression of MGMT and Tiam1 in lung cancer is related to the therapeutic effect of simultaneous modulated accelerated radiotherapy combined with temozolomide on NSCLS patients with brain metastasis.

Analysis of Early Hemodynamic Changes and Prognosis in Patients with Acute Paraquat Poisoning

Objective:
To analyze the characteristics and trends of hemodynamic changes in acute stage of paraquat poisoning,clarify the relationship between hemodynamic changes and prognosis,and provide clinical basis for cardiopulmonary bypass support.
Methods:
PQ poisoning patients older than 12 years old and poisoning time less than 24 hours admitted to the intensive care department of our hospital from January 1,2016 to December 31,2017 were selected as the observation objects.The patients were divided into early death group and early survival group according to whether the patients survived within 72 hours after poisoning.All patients took the same treatment strategy.The hemodynamic changes of the two groups were compared and analyzed.
Results:
The early survival group mainly suffered from moderate and severe poisoning,most patients died of respiratory failure;the early death group mainly suffered from fulminant poisoning,and most patients died of circulatory failure.Compared with the early death group,the early survival group had less toxicity,lower serum PQ concentration,lower SIPP score and lower serum lactic acid level (P<0.01);the early death group had higher initial serum creatinine than the survival group (P<0.05).However,there was no significant difference between the two groups in the time from poisoning to perfusion and from poisoning to the first blood purification (P>0.05).There was no significant difference in initial WBC,RBC,PLT,PH,PO

Conclusion:
The patients with PQ poisoning with high dose of poisoning may have "low-output and low-resistance"hemodynamic changes in acute phase,which are characterized by decreased cardiac output and decreased peripheral vascular resistance,leading to circulatory failure.Hemodynamic monitoring should be carried out as soon as possible,the use of vasoactive drugs should be adjusted according to the changes of hemodynamics,or cardiopulmonary bypass support should be implemented as soon as possible to improve organ perfusion.In order to prolong the survival time of patients and improve the rescue success rate of patients with severe and fulminant PQ poisoning.

Study on Ganoderma Lucidum Polysaccharide Reducing Apoptosis of Cardiomyocytes Induced by Doxorubicin

Objective:
To investigate the protective effects and mechanism of Ganoderma lucidum polysaccharides(GLPS)on doxorubicin (DOX)-induced injury of H9c2 cells.
Methods:
H9c2 cells were divided into control group,GLPs high dose group,DOX group,GLPs low and high dose + DOX group.Cell viability was detected by AV-PI method,and apoptosis related protein expression was detected by Western blot.
Results:
The apoptosis rate and the expression of apoptosis related proteins in the DOX group were significantly higher than those in the control group,but the apoptosis rate and the expression of Caspase-3,Caspase-8,cleared PARP and Bax in the GLPs low and high dose + DOX group were significantly lower than those in the DOX group,and the expression of Bcl-2 was significantly higher,and the Ganoderma lucidum polysaccharide was dose-dependent.
Conclusion:
Ganoderma lucidum polysaccharide can reduce the toxicity of doxorubicin induced cardiomyocytes,and its mechanism may be related to its inhibition of cardiomyocyte apoptosis.

Study on the Excellent and Good Rate of Treatment and Complications of the Minimally Invasive Percutaneous Internal Fixation (MIPPO) Combined with Locking Compression Plate (LCP) in the Treatment of Distal Tibial Fractures

Objective:
To analyze the excellent treatment rate and complications of MIPPO (Minimally Invasive Percutaneous Internal Fixation) + LCP (Locked Compression Plate) for treatment of distal tibial fractures.
Method:
72 patients with distal tibial fractures in our hospital from February 2018 to February 2020 were selected and divided into two groups by double-blind random sampling method (36 cases in each group).The control group was treated with ORIF (open reduction and internal fixation),and the experimental group was treated with MIPPO + LCP.The operation index,excellent and good rate of ankle joint function,incidence of complications and SF-36 score were compared between the two groups.
Results:
The incision length,operation time,hospitalization time,and fracture healing time of the experimental group were significantly shorter than that of the control group.The intraoperative bleeding volume of the experimental group was significantly lower than that of the control group,and the excellent rate of ankle joint function was significantly higher in the experimental group (94.44%) than in the control group (63.89 %) High,the total complication rate of the experimental group (2.78%) was significantly lower than that of the control group (25.00%),and the SF-36 scores of the experimental group were significantly higher than the control group at 1 month after operation,P<0.05 (all differences were statistically significant) Learning meaning).
Conclusion:
MIPPO + LCP operation can effectively improve the ankle function and reduce the incidence of complications in patients with distal tibial fractures.The operation time is shorter,the quality of life of the patients is higher,and the effect is significant.It is worth learning.

Observation on Clinical Effect on Single Open Door Laminoplasty with Arch Plate Fixation in the treatment of Cervical Spondylotic Myelopathy

Objective:
To compare the clinical efficacy of arch plate and lateral mass screw suspension fixation in posterior cervical open-door laminoplasty.
Methods:
86 patients with cervical spondylotic myelopathy treated by posterior open-door laminoplasty from January 2013 to February 2018 were retrospectively analyzed.The operation mode was arch plate fixation as the observation group (n=43),and the lateral mass screw suspension fixation was set as the control group (n=43).The patients in the two groups were followed up for 12 months,and the differences of clinical effects between the two methods were compared by observing the operation time,bleeding volume,JOA score,Pavlov value,incidence of axial symptoms and other indicators.
Results:
Compared with the control group,there was no significant difference in the average operation time and the average intraoperative blood loss in the observation group (P>0.05).The results of intra group comparison showed that JOA score and Pavlov value at different time points after operation were significantly higher than those before operation (P<0.05).There was no significant difference in JOA score and Pavlov value at 3 days,3 weeks,3 months and 12 months after operation in the observation group (P>0.05).The results of inter group comparison showed that JOA of observation group at 3 days,3 weeks,3 months and 12 months after operation was statistically significant.The score and Pavlov value of the observation group were higher than those of the control group,and the difference was statistically significant (P<0.05);the incidence of postoperative axial symptoms in the observation group was lower than that in the control group,and the difference was statistically significant (P<0.05).
Conclusion:
Arch plate fixation is used in posterior cervical spinal canal enlargement,which has high fixation strength and satisfactory clinical effect.It can reduce the occurrence of re closing and axial symptoms after operation,which is worthy of popularization and application.

Effect of Pedicle Screw Rod System in the Treatment of Pelvic Fractures and Anterior Sacral Iliac Joint Plate Fixation in the Treatment of Unstable Pelvic Fractures

Objective:
To investigate the effects of pedicle screw system in the treatment of pelvic fractures and anterior sacroiliac joint plate internal fixation in the treatment of unstable pelvic fractures.
Methods:
90 patients with pelvic fracture admitted to our hospital from August 2015 to December 2019 were selected as the research objects,and they were randomly divided into control group and observation group,45 cases in each group.The control group was treated with anterior plate fixation of sacroiliac joint,and the observation group was treated with pedicle screw rod system.The therapeutic effect and pelvic reduction of the two groups were compared under different treatment methods.
Results:
The operation time,blood loss,surgical incision,load and ambulation time in the observation group were lower than those in the control group,and the difference between the two groups was statistically significant (P<0.05).Repeated measurement results showed that the VAS scores of the two groups decreased at different time after treatment.The VAS scores of the observation group at 1,2 and 3 weeks after operation were lower than those of the control group (P<0.05).After treatment,the excellent and good rate of fracture reduction was 91.11% in the observation group and 75.56% in the control group,the difference was statistically significant (z = -3.847,χ

Study on Effect of Calcitriol Combined with Cinacacet in the Treatment of Secondary Hyperparathyroidism in Hemodialysis Patients

Objective:
To investigate the clinical effect and safety of application of calcitriol combined with cinacalcet in the treatment of secondary hyperparathyroidism in hemodialysis patients.
Methods:
The patients who were treated in our hospital between June 2016 and June 2019 were included.A total of 90 patients were included.They were divided into a random control group and a treatment group with 45 patients in each group.For patients in the control group,the main treatment was oral cinacalcet hydrochloride tablets.The starting dose was 25 mg/d,and the maximum dose was controlled at 75 mg/d.For patients in the treatment group,on the basis of receiving the treatment of the patients in the control group,oral calcitriol capsules were taken at the same time.The starting dose was 3 times a week,and then the dose was adjusted according to the patient's situation.Both groups of patients were treated for 3 months.The effects of the two groups of patients were analyzed,and the changes of blood calcium,blood phosphorus levels,parathyroid hormone,alkaline phosphatase,and parathyroid volume in the two groups of patients before and after treatment were compared.
Results:
The effective rate of the treatment group was 93.33%,and the effective rate of the control group was 77.77%.The effective rate of the treatment group was significantly higher than that of the control group (χ

=4.406,P=0.041).The blood calcium and blood phosphorus levels of the patients before and after treatment were compared,and there was no significant difference before treatment (P>0.05).After treatment,the blood phosphorus level of the treatment group changed more than the control group (P<0.05),while the blood calcium level of the treatment group changed less than the control group (P<0.05).The total parathyroid hormone and alkaline phosphatase of the two groups of patients were found to be no significant difference before treatment (P>0.05).After treatment,the total parathyroid hormone and alkaline phosphatase of the two groups of patients were reduced,(P<0.05).Compared with the control group,the patients in the treatment group changed more.After treatment,the parathyroid glands of the two groups of patients were significantly improved in length,width,thickness and volume,and the improvement effect in the treatment group was more significant (P<0.05).
Conclusion:
In the treatment of secondary hyperparathyroidism in hemodialysis patients,calcitriol combined with cinacalcet has a good effect,and it can also significantly reduce the volume of parathyroid glands and further control the occurrence of adverse reactions.

Effect of Ticagrelor on Platelet Function in Elderly Patients with AMI and Its Clinical Efficacy

Objective:
To observe the effect of ticagrelor combined with aspirin on platelet function in elderly patients with acute myocardial infarction (AMI).
Methods:
200 patients with acute myocardial infarction treated in our hospital from January 2015 to December 2016 were randomly divided into clopidogrel group (101 cases),ticagrelor group (99 cases),and clopidogrel group Loaded aspirin 300mg+ clopidogrel 600mg,the next day was changed to maintenance dose aspirin 100mg/d and clopidogrel 75mg/d;the ticagrelor group was given aspirin 300mg + ticagrelor 180mg,changed to the maintenance dose the next day Aspirin 100mg/d and ticagrelor180mg/d were tested for platelet reaction index (PRI) before and 1,6 and 12 months after administration,and the main adverse cardiovascular events (MACCE) and myocardial infarction thrombolytic therapy were observed ( TIMI) bleeding event.
Results:
99 patients in the clopidogrel group completed the follow-up,99 patients in the ticagrelor group completed the follow-up,5 patients in the clopidogrel group were aspirin intolerant,replaced with cilostazol,and 6 patients in the ticagrelor In the Luo group,aspirin intolerance was replaced by cilostazol.The patients in the two groups insisted on taking the drug,and no drug withdrawal due to allergy occurred.After receiving clopidogrel and ticagrelor patients,the difference in PRI was statistically significant (F = 13.169,P<0.001).Before treatment,1 month,6 months,and 12 months,The PRI differences were statistically significant (F = 21.574,P<0.001),and there were interactions between different groups and time.The differences between the two groups at the same time point were statistically significant (all P<0.001),different times within the same group between points,the trend of decreasing PRI (all P<0.001).The two groups of patients were followed up.After one year of treatment,MACCE occurred in 4% of patients in the ticagrelor group and in 13% of patients in the clopidogrel group.The difference between the two groups was statistically significant (P=0.01).During 1 year of follow-up,there was no statistically significant difference in the incidence of TIMI bleeding between the two groups,and ticagrelor did not increase the risk of bleeding.
Conclusion:
Patients with ticagrelor AMI can further reduce the MACCE event without increasing the risk of bleeding.

Analysis on Effect of Micro-implant Anchorage in the Treatment of Orthodontics

Objective:
To analyze the effect of micro-implant anchorage on the efficacy and compliance of orthodontics.
Method:
180 patients with orthodontic treatment in our hospital from January 2017 to January 2019 were selected as the research objects and randomly divided into observation group and control group,with 90 cases in each group.The observation group was treated with micro implant anchorage,while the control group was treated with non-implant anchorage.The model,oral health related quality of life (OHRQoL) scale score,adverse reactions and patient compliance were compared between the two groups before and after treatment.
Results:
Compared with the model overbite,model coverage,canine width and first molar width of the two groups,the improvement degree of the observation group was more significant (P<0.05).The changes of upper alveolar angle,upper and lower alveolar seat angle,Sn angle of upper central incisor and upper lower central incisor angle of the two groups were compared.The changes of each index in the observation group were more significant than those in the control group (P<0.05).In this paper,t-test was used to compare the scores of OHRQoL scale between the two groups from the aspects of physiological pain,functional limitation,physiological dysfunction,social ability limitation,psychological disorder,limb movement disability and inner communication ability.The results showed that the score of the observation group was significantly higher than that of the control group (P<0.05).At the same time,the difference in the incidence of adverse reactions before and after treatment was compared by means of chi square test.The results showed that the observation group was significantly lower than the control group (χ

Comparison of the Curative Effect between FURSL and MPCNL on 2-3cm of Kidney Stones

Objective:
To compare the curative effect and safety between flexible ureteroscopic lithotripsy (FURSL) and mini-percutaneous nephrolithotomy (MPCNL) in the treatment of 2-3 cm of kidney stones.
Methods:
A total of 160 patients with 2-3cm of kidney stones who were admitted to Urology Department of Dongguan People's Hospital between January 2018 and January 2019 were randomly divided into FURSL group (n=80) and MPCNL group (n=80).The preoperative,intraoperative and postoperative indexes such as changes in renal function and infectious indexes,pain visual analogue score (VAS),and incidence of complications were compared between the two groups.
Results:
There were no statistical differences in preoperative basic data between the two groups.The intraoperative blood loss,hospitalization time,hospitalization costs,decreased value of hemoglobin and one-stage stone clearance rate of FURSL group were less/shorter/lower than those of MPCNL group (P<0.05),while the operation time was longer than MPCNL group (P<0.05).There was no significant difference in one-stage stone clearance rate between the two groups (P>0.05).BUN,Scr and PCT in both groups were significantly increased at 2 days after operation (P<0.05).Besides,BUN,PCT and their differences before and after treatment in FURSL group at 2 days after operation were lower than those in MPCNL group.However,there was no significant difference in Scr between the two groups (P>0.05).VAS scores of FURSL group were lower than those of MPCNL group at 1 day and 2 days after operation (P<0.05).The incidence of complications in FURSL group was lower than that in MPCNL group within 3 months after operation (P<0.05).
Conclusion:
Both fursl and MPCNL are safe and effective in the treatment of 2-3 cm renal calculi.However,fursl is a natural lumen technique with no surgical wound.It has the advantages of small trauma,quick recovery,small damage to renal function,light pain and low risk of complications.However,for 2-3 cm renal calculi,the operation time of fursl is longer than that of MPCNL,and the stone clearance rate of fursl is lower than that of MPCNL,so the operation method should be selected according to the patient's condition.

Relationship between Serum Uric Acid and Serum PA Levels and SLE Activity and Kidney Damage

Objective:
To investigate the relationship between serum uric acid and serum prealbumin (PA) levels and the degree of activity and kidney damage in systemic lupus erythematosus (SLE).
Methods:
66 SLE patients diagnosed and treated in our hospital from January 2016 to December 2016 were retrospectively selected as the study subjects.According to the SLEDAI score,they were divided into the mildly active group (25),the moderately active group (21),and the severely active group (20).The differences in clinical data,serum uric acid and serum PA among the three groups were compared,and the relationship between serum uric acid and PA with SLE activity and kidney damage was analyzed by Logistic regression.

Serum uric acid level SLE mild activity group<moderate activity group<severe activity group,serum PASLE mild activity group>moderate activity group>severe activity group,the differences between the groups were statistically significant (P<0.05).Pearson's correlation results showed that serum uric acid was positively correlated with SLEDAI score (P<0.01),and serum PA was negatively correlated with SLEDAI score (P<0.01).Among the other 66 SLE patients,46 had LN,of which 56.52% (26/46) had normal renal function and 43.48% (20/46) had abnormal renal function.Single-factor results showed that the differences of Hb,TG,eGFR,Scr and serum uric acid in the normal and abnormal renal function groups were statistically significant (P<0.05),while the differences of serum PA level between the two groups were not statistically significant (P>0.05).Binary Logistic regression analysis showed that Hb,TG,eGFR and serum uric acid were all related influencing factors for renal damage in patients with LN (B=-3.126,3.623,-4.143,4.464,P<0.05) .
Conclusion:
Serum uric acid and serum PA are closely related to the activity level of SLE,both of which can be used as indicators of SLE activity.Serum uric acid was a related factor of renal damage,but no correlation between renal damage and serum PA was found.

Value of High-Frequency Ultrasound of Wrist and Finger Joint in the Diagnosis and Differential Diagnosis of Rheumatoid Arthritis

Objective:
To investigate the value of high-frequency ultrasound (HFUS) of wrist and finger joint in the diagnosis and differential diagnosis of rheumatoid arthritis (RA) and hand osteoarthritis(OA).And to analyze the value of semi-quantitative ultrasound score in the assessment of disease activity of RA.
Methods:
54 patients with RA were enrolled,and 30 patients with hand OA were selected as control group.Wrist joints,metacarpophalangeal and proximal interphalangeal joints were examined by high-frequency ultrasound.The thickness of synovial membranes,synovial blood flow,bone erosion and tenosynovitis were measured.Semi-quantitative ultrasound scores of the above four indexes were calculated.And laboratory characteristics were carefully recorded,including ESR,CRP,RF,anti-CCP and DAS28 scores in the RA group.The correlation between ultrasound scores and clinical indicators were analyzed.
Results:
RA group was significantly more than OA group with synovial hyperplasia,synovial blood flow,bone erosion and tenosynovitis (P<0.01).There were significant positive correlation between semi-quantitative ultrasound score and DAS28 score (r=0.550,P<0.05).
Conclusion:
High-frequency ultrasound was valuable in the diagnosis and differential diagnosis of RA and hand OA.Semi-quantitative ultrasound score can reflect the disease activity in RA.

Analysis on Correlation of Leukoaraiosis with Cognitive Impairment and Functional Outcome in Patients with Acute Ischemic Stroke

Objective:
To study the relationship between leukoaraiosis and cognitive function/functional outcome in the patients with acute ischemic stroke.
Methods:
A total of 100 patients with acute ischemic stroke were selected from the neurology ward of the First Affiliated Hospital of Harbin Medical University.The cognitive function was evaluated by MMSE,MOCA and HAMD,and the National Institute of health stroke was used.The functional outcomes were evaluated by scale (NIHSS),modified Rankin Scale (MRS) and ability of daily living (BI).Based on Fazekas method,leukoaraiosis was divided into periventricular hyperintensities (PVH) and deep white matter matter hyperintensities (dwmh),and divided into 4 grades (0-3 grades).The correlation between cognitive function score and functional outcome score and the degree of leukoaraiosis was analyzed.
Results:
The age of patients in the PVH positive groups was higher than that in the control group,and the more severe the degree of leukoaraiosis was,the older they were,and the difference was statistically significant (P<0.05).The score of MMSE and MoCA in the PVH and DWMH positive groups were lower than those in the control group,and the more severe the degree of leukoaraiosis was,the lower the score of MMSE and MoCA were,and the difference was statistically significant (P<0.05).The score of NIHSS and mRs in the PVH and DWMH positive groups were higher than those in the control group,and the more severe the degree of leukoaraiosis was,the higher the score of MMSE and MoCA were,and the difference was statistically significant (P<0.05).The score of BI in the PVH and DWMH positive groups were lower than those in the control group,and the more severe the degree of leukoaraiosis was,the lower the score of BI was,and the difference was statistically significant (P<0.05).PVH and DWMH were negatively correlated with MMSE and MoCA of cognitive function score,while they were positively correlated with NIHSS and mRs of functional outcome score.Furthermore,compared with PVH,DWMH was significantly correlated with cognitive function and functional outcome.
Conclusion:
With the aggravation of the degree of leukoaraiosis,the cognitive function score decreased and the functional prognosis score increased,suggesting that the aggravation of the degree of leukoaraiosis could be used as a predictor of cognitive dysfunction and adverse functional prognosis in patients with acute ischemic stroke.Furthermore,DWMH was associated greater with cognitive function and functional outcome than PVH.

Effects of Paroxib Sodium Combined with Dexmedetomidine on Stress Hormone Levels and OAA/S Sedation Scores After Hip Replacement in Elderly patients

Objective:
Analysis of the effect of paroxib sodium combined with dexmedetomidine on stress hormone levels and OAA/S sedation scores after hip replacement in the elderly.
Methods:
From June 2018 to February 2020,184 elderly patients who underwent unilateral hip replacement were divided into observation group and control group by random number table method.Dexmedetomidine was administered in the control group and paroxib sodium combined with dexmedetomidine was administered in the observation group.The VAS scores of the two groups were observed at different time after operation.At the same time,the levels of postoperative stress index (superoxide dismutase- SOD、 malondialdehyde- MDA) and OAA/S sedation scores were compared between the two groups.Finally,the incidence of adverse reactions between the two groups was counted.
Results:
The interaction effects between time points,between groups,between time points and groups of VAS scores in the two groups were statistically significant (P<0.05).The 6 h VAS、12h VAS、24h VAS scores of observation group and control group were significantly lower than those of control group (P<0.05).There was significant difference in the SOD、MDA、OAA/S sedation score between the observation group and the control group after 1 d (P<0.05).Adverse reaction rate in observation group was significantly lower than that in control group (P<0.05).
Conclusion:
The application of paroxib sodium combined with dexmedetomidine for advanced analgesia in elderly patients with hip arthroplasty can effectively reduce the postoperative pain score thus improve the postoperative stress response status of patients,and help to improve the sedation effect,which is worth popularizing.

Objective:
To investigate the effect of oxymatrine on intestinal mucosal barrier damage and HMGB1 level in patients with severe pancreatitis.
Method:
A total of 120 patients with severe pancreatitis were selected as the research object,and were randomly vided into observation group and control group.The control group was given basic treatment,while the observation group was treated with Oxymatrine on the basis of the control group.The white blood cell recovery time,inflammatory factor level and intestinal mucosal barrier damage index level were observed.
Results:
The total effective rate of observation group was 93.33%,which was significantly higher than that of control group,P<0.05;The abdominal pain relief time (3.58±1.02) d,first defecation time (3.09±0.68) d,blood amylase recovery time (4.58±1.45) d and white blood cell recovery time (5.05±1.33) d in the observation group were significantly lower,P<0.05;After treatment,the levels of TNF-α(887.65±133.24)ng/L,IL-6(64.75±22.31)ng/L,IL-1(388.65±83.06)ng/L and HMGB1(32.43±23.67)μg/mL in the observation group were significantly lower,P<0.05;After treatment,the levels of serum DAO (8.16±2.04) U/L,serum D-lactic acid (6.51±0.53) mg/L and botulinum toxin (16.42±6.87) EU/L in the observation group were significantly lower,P<0.05.
Conclusion:
Oxymatrine combined with basic treatment for severe pancreatitis has significant clinical effect,can significantly reduce the level of inflammatory factors,improve the intestinal mucosal barrier function of patients,and is worthy of clinical application.

Effects of Early Continuous Blood Purification on PCT and CRP Levels in Patients with Sepsis

Objective:
To explore the effects of early continuous blood purification (CBP) on inflammatory mediators and coagulation function in patients with sepsis.
Methods:
A total of 110 patients with sepsis admitted to our hospital from January 2019 to December 2019 were selected for the study and divided into treatment group (55 cases) and control group (55 cases) with simple random grouping and random number table methods.Control group was given routine treatment,and treatment group was given CBP on the basis of control group,and the two groups were treated for 1 w.Before and after treatment,the organ function indexes [lactic acid,creatinine,bilirubin],inflammatory mediators [tumor necrosis factor-α (TNF-α),serum procalcitonin (PCT) and C-reactive protein (CRP)] and coagulation function [prothrombin time (PT),thrombin time (TT),and activated partial thromboplastin time (APTT)] were compared between the two groups.Acute Physiology and Chronic Health Evaluation (APACHE II) and Sepsis-Related Organ Failure Assessment (SOFA) were used to assess the disease severity in the two groups,and the clinical outcomes and prognosis were compared between the two groups.
Results:
The decreases of lactic acid,creatinine and bilirubin in treatment group before and after treatment were greater than those in control group (P<0.05).The decreases of TNF-α,PCT and CRP in treatment group before and after treatment were greater than those in control group (P<0.05).The increases of PT,TT and APTT in treatment group before and after treatment were greater than those in control group (P<0.05).The decreases of scores of APACHE II and SOFA in treatment group before and after treatment were greater than those in control group (P<0.05).The ICU stay and ventilator ventilation time in treatment group were significantly shorter than those in control group,and the mortality rate within 28d and incidence rate of MODS were significantly lower than those in control group (P<0.05).
Conclusion:
CBP can significantly improve the prognosis of patients by regulating the inflammatory mediators levels and coagulation function in the treatment of sepsis.

Effect of Azithromycin Combined with Yangyin Qingfei Decoction on the Expression of IL-1 and TNF-αin Serum of Children with Bronchial Pneumonia

Objective:
To study the curative effect of Azithromycin combined with Yangyinqingfei Decoction on the serum interleukin-1beta (IL-1 beta) and tumor necrosis factor alpha (TNF- alpha) expressions of children with bronchial pneumonia.
Methods:
96 cases of children with bronchial pneumonia in our hospital from October 2015 to December 2016 were randomly divided into the control group and the observation group with 48 cases in each group.The control group was administered azithromycin treatment,and the observation group was treated by oral Yangyinqingfei decoction on the basis of control group.The total effective rate,disappearance time of clinical symptoms and signs and changes of serum IL-1 and beta TNF- alpha levels before and after treatment were compared between two groups.
Results:
After treatment,the total efficiency of observation group (93.75%) was significantly higher than the control group (77.08%) (P=0.02);The disappearance time of fever,cough,pulmonary rales,pulmonary shadow and hospital stay of observation group were significantly shorter than those of the control group(P<0.01);The serum IL-1 beta and TNF- levels of observation group were significantly lower than those of the control group(P<0.01).
Conclusion:
the Azithromycin combined with Yangyinqingfei Decoction could enhance the clinical efficacy in the treatment of children with bronchial pneumonia,which was able to promote the recovery of the symptoms and signs,decrease the serum IL-1 beta and TNF- levels.

Effects of Enhanced External Counter-Pulsation on Sleep Quality of Non-Hospitalized Chronic Cardiac Failure Patients

Objective:
To explore the effect of enhanced external counter-pulsation on sleep quality of non-hospitalized patients with chronic heart failure.
Methods:
In the selection period,80 patients with chronic heart failure and insomnia were randomized into the control group and the experimental group,40 cases in each.The control group was treated with conventional therapy,and the experimental group was given enhanced external counter-pulsation therapy based on conventional therapy.On the day of discharge and 36 hours after enhanced external counter-pulsation,the sleep quality of the two groups were compared.
Results:
After enhanced external counter-pulsation treatment,the insomnia symptoms of the experimental group were significantly improved,and the scores were better than those of the control group,indicating that enhanced external counter-pulsation treatment is effective.
Conclusion:
Enhanced external counter-pulsation has a positive effect on improving the sleep quality of patients with chronic heart failure.It is worthy of further research and promotion.

Diagnostic Accuracy of MiRNA-21 MiRNA-155 and MiRNA-195 for Breast Cancer: A Meta-Analysis and Indirect Comparison

Objective:
To conduct a meta-analysis to determine the diagnostic accuracy of three miRNAs ( miRNA-21,miRNA-155and miRNA-195) in breast cancer (BC),and to evaluate the effectiveness of three miRNAs with common reference-based indirect comparison meta-analysis.
Methods:
We searched the following databases such as PubMed,EMbase,Medline,The Cochrane Library,Chinese Biological Medicine Database (CBM),China National Knowledge Infrastructure (CNKI),WANFANG Database,Chinese Weipu Database (VIP) from inception to october 2019.The screening,data extraction and quality assessment were conducted in accordance with the inclusion and exclusion criteria by two reviewers independently.Quality assessment was performed using a second version of the Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2).Meta-Disc 1.4 and Stata 12.0 software were used to perform Meta analysis.Pooled sensitivity,pooled specificity,pooled positive likelihood ratio,pooled negative likelihood ratio,and pooled diagnostic odds ratio were used to assess the diagnosis efficiency.The summary receiver operating characteristic (SROC) curve was plotted and the area under the ROC curve (AUC) was calculated using RevMan 5.3.Stability was evaluated by sensitivity analysis.Deeks'funnel was used to investigate the publication bias.Relative diagnostic odds ratio (RDOR) was used to perform the indirect comparison meta-analyses of three miRNAs in BC by R software.
Results:
A total of seventeen documents were included,including a total of 1 576 BC cases,and 987 control cases.A fixed-effect model was used for pooling the studies of miRNA-21 and miRNA-195.The pooled sensitivity,pooled specificity,pooled positive likelihood ratio,pooled negative likelihood ratio,pooled diagnostic odds ratio and the AUC of SROC curve of miRNA in the diagnosis of BC were 0.73 95% confidence interval(CI)(0.69~0.77),0.86(95% CI 0.83~0.89),4.99(95% CI 3.86-6.46),0.34(95% CI 0.29~0.39),15.18(95% CI 10.79~21.37) and 0.885 respectively,and as for miRNA-195 those were 0.76(95% CI 0.72~0.80),0.89(95% CI 0.85-0.93),6.59(95% CI 4.77~9.12),0.27(95% CI 0.23~0.33),28.41(95% CI 17.98~44.89) and 0.910,and as a random effect model was used for miRNA-155 those were 0.81(95% CI 0.78~0.84),0.85(95% CI 0.80-0.89),4.90(95% CI 2.33~10.31),0.16(95% CI 0.07~0.33),44.22(95% CI 11.74~166.53) and 0.933.The results of indirect comparison showed that the diagnostic accuracy has no significant difference among the three miRNAs in diagnosing BC.The Deek's funnel plot asymmetry test showed no significant publication bias.
Conclusion:
MiRNA-155 was better than miRNA-21 and miRNA-195 in diagnosing BC.

Efficacy of External Counter-Pulsation Combined with Nicorandil on Coronary Microcirculation Disorders and Its Effectson Vascular Endothelial Function

Objective:
To explore the efficacy of external counter-pulsation (ECP) combined with nicorandil on coronary microcirculation disorders and its effects on vascular endothelial function.
Methods:
From April 2017 to March 2019,240 patients with coronary microcirculation disorders and angina pectoris in our hospital were selected as the research objects,and they were divided into conventional treatment group (control group),ECP treatment group (ECP group),nicorandil treatment group (nicorandil group) and ECP combined with nicorandil treatment group (combined group) according to the simple random grouping method,with 60 cases in each group.The clinical efficacy of angina pectoris and clinical efficacy of electrocardiogram were recorded among the four groups after 6 months of treatment,and the positive rate of treadmill exercise test,incidence rate of abnormal wall motion,6-minute walking distance (6MWT),myocardial contrast echocardiography indexes and cardiopulmonary function were compared among the four groups before treatment and after 6 months of treatment.
Results:
There were no statistically significant differences in the clinical efficacy of angina pectoris and clinical efficacy of electrocardiogram between nicorandil group and ECP group (P>0.05).The clinical efficacy of angina pectoris and clinical efficacy of electrocardiogram in combined group were significantly higher than those in nicorandil group,ECP group and control group (P<0.05),and the clinical efficacy of angina pectoris and clinical efficacy of electrocardiogram in nicorandil group and ECP group were higher than those in control group (P<0.05).6 months after treatment,the positive rate of treadmill exercise test and the incidence rate of abnormal wall motion in the four groups were lower than those before treatment (P<0.05),and the two indexes in combined group were lower than those in nicorandil group,ECP group and control group (P<0.05),and the two indexes in nicorandil group and ECP group were lower than those in control group (P<0.05).The 6MWT and A value,β value and MBF in the four groups were higher than those before treatment (P<0.05),and the increases in combined group were greater than those in nicorandil group,ECP group and control group (P<0.05).And the increases in nicorandil group and ECP group were greater than those in control group (P<0.05).There were no significant differences in the PVO

Expressions and Clinical Significances of UBE2S and CNB1 in Glioma

Objective:
To study the expression and clinical significance of cyclin B1 (CCNB1) and ubiquitin binding enzyme E2S (UBE2S) in glioma.
Methods:
106 cases of glioma tissue specimens from January 2015 to April 2019 were selected and brain glial tissue specimens from 114 patients with intracranial hemorrhage or trauma undergoing intracranial decompression were taken as control group.The expression levels of CCNB1 and UBE2S in glial tissue were measured by immunoblotting (WB).The relationships between the expression levels and clinicopathological features of glioma were analyzed.All patients were followed up.The effects of the expressions of both on the prognosis of patients with glioma were explored.
Results:
The expression levels of CCNB1 and UBE2S in low-grade glioma group and high-grade glioma group were significantly higher than those in the control group (P<0.05),and the expression levels of CCNB1 and UBE2S in high-grade glioma group was significantly higher than that in low-grade glioma group (P<0.05).The expression levels of CCNB1 and UBE2S in glioma tissues were not correlated with age,sex and pathological type (P>0.05),but were related to the degree of tissue differentiation,peritumoral edema and infiltration (P<0.05).Pearson analysis showed that the expression levels of CCNB1 and UBE2S in glioma tissues were positively correlated (r = 0.460,P<0.05).The median survival time of patients with high expressions of CCNB1 and UBE2S were significantly lower than those with low expressions of CCNB1 and UBE2S (P<0.05).COX multivariate analysis showed that low tissue differentiation,high expression of CCNB1 and UBE2S were independent risk factors for poor prognosis of glioma patients (P<0.05).
Conclusion:
The expression levels of CCNB1 and UBE2S in glioma tissues are significantly increased,which is related to the degree of differentiation,peritumoral edema,infiltration and survival time of glioma tissues,and might be a clinical index for evaluating the prognosis of glioma patients.

Correlation of Plasma Hcy, MMP-9 and ApoE Levels with Brain SWI Imaging Features in Patients with Stroke

Objective:
To explore the correlation between plasma homocysteine (Hcy),serum matrix metalloproteinase-9 (MMP-9) and apolipoprotein (ApoE) levels and head susceptibility-weighted imaging (SWI) features in stroke patients.
Methods:
96 stroke patients admitted to the hospital between January 2017 and January 2019 were selected as the observation group.Meanwhile,44 healthy people were selected as the control group.Head SWI examination and plasma Hcy,MMP-9 and ApoE detection were performed.The number and location of bleeding displayed by head SWI images,Hcy,MMP-9 and ApoE levels were compared between the two groups.
Results:
The SWI-MRI examination results showed that the sensitivity of SWI sequence was significantly higher than that of DWI,T1WI and T2WI (P<0.05).Plasma Hcy,ApoE and MMP-9 levels were significantly higher in observation group than control group (P<0.05),and significantly higher in patients with large infarction than those with middle and small infarction (P<0.05).In the observation group,plasma Hcy,MMP-9 and ApoE levels were positively correlated with the number and volume of stroke lesions and the number of cerebral cortical lesions (P<0.05).The expression level of ApoE was positively correlated with the number of deep white matter lesions (P<0.05).Logistics regression analysis showed that plasma Hcy,MMP-9 and ApoE levels were significantly correlated with the diagnostic results of SWI (P<0.05).
Conclusion:
The expression levels of Hcy,MMP-9 and ApoE are positively correlated with the number and volume of stroke lesions and cortical lesions displayed by SWI.The expression level of ApoE is positively correlated with white matter lesions.Hcy,MMP-9 and ApoE levels are independent risk factors of stroke development.

76 cases of DOR in vitro fertilization embryo transfer from January 2017 to December 2018 were selected and divided into 41 cases in group A (micro stimulation and antagonist program) and 35 cases in group B (double stimulation program) according to the patient's voluntary principle.Group B was divided into follicular phase group and luteal phase group to compare the results of different stimulation schemes.
Results:
Compared with the follicular group,the number of oocytes,the amount of ovulation,the time of ovulation and the number of high-quality embryos in group A were significantly increased.And the content of luteinizing hormone on the trigger day was significantly decreased (P<0.05).Compared with group A,the dosage,time,progesterone and estradiol of luteal phase group were significantly increased,while luteinizing hormone content was significantly decreased (P<0.05).Compared with the follicular group,the number of oocytes obtained,the amount of ovulation induction,the time of ovulation induction,the number of embryos available,the number of high-quality embryos and the progesterone value on the trigger day in the luteal phase group were significantly higher,and the luteinizing hormone content on the trigger day was significantly decreased (P<0.05).The available embryo rate of group B was significantly higher than that of group A (P<0.05).
Conclusion:
In the clinical application of DOR patients,the luteal phase of the double stimulation program can achieve the same ovulation results as the micro stimulation+antagonist program,and the implementation of the double stimulation program in the same menstrual cycle can reduce the treatment time of in vitro fertilization embryo transfer,which is feasible and reliable for the treatment of DOR patients.

A Complication Analysis of Further Pregnancy of Women at Advanced Maternal Age with Scar Uterus

Objective:
To analyze the complications of further pregnancy at advance age of women with scar uterus,and to provide a theoretical basis for the timing of re-pregnancy and the choice of delivery mode for primipara.
Methods:
We selected 210 cases of scar uterus multiparae in our hospital from January 2017 to December 2018,of which 105 cases were in the advanced age group,age ≥35 years,and 105 cases in the control group,age <35 years.Data of pregnancy complications and perinatal complications were collected in detail.
Results:
1.Comparison of the general items:The advanced age group has larger data of age,pregnancy times,abortion/curettage times and the interval from the last cesarean section than those of the control group,and the differences were significant statistical (P<0.01). 2.Comparison of pregnancy complications:The incidence of gestational diabetes mellitus,preeclampsia,premature rupture of membranes and uterine leiomyoma in the advanced age group was significantly higher than that in the control group,and the difference was statistically significant (P<0.05);In preterm labor,placenta previa,placental abruption,hydramnios,oligohydramnios,HELLP syndrome,gestational hypothyroidism,the advanced age group had higher data than the control group,but the difference is not statistically significant (P>0.05).3.Comparison of perioperative complications:The intraoperative bleeding volume,operating time and perioperative complications in the advanced age group were higher than those in the control group (P<0.01).The advanced age group had higher data of postpartum hemorrhage,uterine contraction fatigue,placenta accreta,placental adhesion,pelvic adhesion,intraoperative secondary injury,blood transfusion,postoperative fever,postoperative intestinal obstruction,and poor wound healing,but there is no statistical difference (P>0.05). 4.Comparison of fetal and neonatal adverse events:Both gestational weeks and neonatal birth weight in the advanced age group were less than those in the control group,but there was no significant difference (P>0.05);The incidence of perinatal adverse events in the advanced age group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).And the proportion of neonatal asphyxia and transference to NICU was significantly higher(P<0.05).
Conclusion:
The incidence of complications of gestational diabetes mellitus,preeclampsia,premature rupture of membranes and hysteromyoma of advanced age group was 2.1,2.8,2.4 and 4.8 times higher than that of control group,and the incidence of perioperative complications and perinatal complications of advanced age group is higher at that.Therefore,it suggests that early pregnancy is meaningful in avoiding the high risk of advanced age pregnancy.At the same time,couples with more than one child requirement should choose vaginal delivery at first labor to reduce the cesarean section caused by social factors,and thus reduce complications of the perinatal and pregnant women.

Correlation Between Serum TGF-β1 and CTGF and Myocardial Remodeling in Children with Congenital Heart Disease

Objective:
To analyze the correlation between serum transforming growth factor-β1 (TGF-β1) and connective tissue growth factor (CTGF) and myocardial remodeling in children with congenital heart disease (CHD).
Methods:
The clinical data of 186 children patients with CHD from May 2017 to December 2019 in our hospital were analyzed retrospectively.The children patients were divided into CHD heart failure group and CHD non-heart failure group according to the occurrence of heart failure.Serum TGF-β1 and CTGF levels were compared between the two groups.According to the New York Heart Disease Association (NYHA) heart function grading,the children patients in CHD heart failure group were divided into NYHA Ⅱ group,NYHA Ⅲ group and NYHA Ⅵ group,and the myocardial remodeling indexes [LADI,LVEDDI,LVEDVI,LVMI] and levels of serum TGF-β1 and CTGF were compared among the three groups,and Pearson correlation analysis was used to evaluate the correlation between serum TGF-β1 and CTGF levels and myocardial remodeling.
Results:
Among 186 children patients with CHD,129 cases with heart failure were included in CHD heart failure group,and 57 cases without heart failure were included in CHD non-heart failure group.There were no statistically significant differences between the two groups in the baseline data such as gender,age and etiology (P>0.05).Serum TGF-β1 and CTGF levels in CHD heart failure group were significantly higher than those in CHD non-heart failure group (P<0.05).Among 129 children patients with CHD and heart failure,39 cases with NYHA grade Ⅱ were included in NYHA Ⅱ group,and 56 case with NYHA grade Ⅲ were included in NYHA Ⅲ group,and 34 cases with NYHA grade VI were included in NYHA VI group.Comparison of LADI,LVEDDI,LVEDVI,LVMI and serum TGF-β1 and CTGF levels showed that NYHA Ⅱ grouP<NYHA Ⅲ grouP<NYHA Ⅵ group (P<0.05).Pearson correlation analysis found that serum TGF-β1 and CTGF levels were positively correlated with LADI,LVEDDI,LVEDVI and LVMI (P<0.05).
Conclusion:
Serum TGF-β1 and CTGF are closely related to the myocardial remodeling in children with CHD,and they have certain application value in evaluating the status of myocardial remodeling in children patients.

Effect of Electroacupuncture on Mayo Elbow Function Score and Serum Inflammatory Factors in Patients with Elbow Joint Stiffness

Objective:
To investigate the effect of electroacupuncture on Mayo Elbow function score and serum inflammatory factors in patients with elbow joint stiffness.
Methods:
From January 2018 to October 2019,300 patients after arthrolysis of elbow stiffness were selected and divided into the control group (150 cases) and the observation group (150 cases) according to the random number table method.The cases in the control group were given routine rehabilitation training treatment;and the cases in the observation group were given electric acupuncture treatment after arthrolysis of elbow stiffness on the basis of the control group.Before and after treatment,the levels of inflammatory factors [Interleukin-6(IL-6),Interleukin-1(IL-1),Tumor Necrosis Factor-α(TNF-α)] were measured by enzyme linked immunosorbent assay(ELISA).The Mayo Elbow function score,effects and side effects were compared between two groups.
Results:
The treatment efficiency of the observation group was higher than that of the control group (100.00% vs 96.00%) (χ

=6.122,P=0.013).There was no difference in elbow pain,range of motion,stability,activity ability and Mayo total score between the two groups before treatment.There is no statistical significance (22.47±4.28 vs 22.53±4.31,t=0.121,P=0.904),(8.96±2.15 vs 9.03±2.17,t=0.281,P=0.779),(5.14±1.08 vs 5.09±1.01,t=0.414,P=0.679),(17.69±4.25 vs 17.71±4.26,t=0.041,P=0.968),(54.26±8.32 vs 54.36±8.37,t=0.104,P=0.917).Compared with the control group,the observation group's elbow pain,range of activity,activity capacity and Mayo total score were higher after treatment (39.96±7.28 vs 34.31±6.95,t=6.875,P<0.001),(17.98±4.03 vs 13.86±3.52,t=9.430,P<0.001),(25.63±4.52 vs 22.03±4.31,t=7.060,P<0.001),(93.59±17.79 vs 80.05±16.69,t=6.798,P<0.001),but there was no difference in stability scores between the two groups (9.98±2.16 vs 9.85±2.07,t = 0.532,P=0.595);There was no statistically significant difference in preoperative level comparison[(11.82±2.41) ng/L vs (11.76±2.39) ng/L,t=0.217,P=0.829],[(137.83±21.41) ng/L vs (139.47±19.52) ng/L,t=0.693,P=0.489],[(47.83±11.10) ng/L vs (48.02±10.69) ng/L,t=0.151,P=0.880],while after treatment,compared with the control group,the levels of IL-6,IL-1 and TNF-α in the observation group were lower than those in the control group[ (9.52±1.03) ng/L vs (10.48±1.49) ng/L,t=6.491,P<0.001],[ (60.31±12.48) ng/L vs (97.19±11.81) ng/L,t=27.281,P<0.001],[ (7.88±1.36) ng/L vs (23.84±4.65) ng/L,t=40.346,P<0.001];there was no significant difference in the incidence of adverse reactions between the observation group and the control group (2.00% vs 6.67%) (χ2=2.895,P=0.089).
Conclusion:
The application of electroacupuncture intervention can significantly improve the Mayo function score of elbow joint,reduce the postoperative inflammatory response,and has a better clinical effect on the functional rehabilitation after arthrolysis of elbow joint stiffness,and has a better safety,which is worthy of clinical promotion.

Study on the Distribution and Correlation of Syndrome Elements in Patients with Primary Insomnia

Objective:
To explore the distribution rules of traditional Chinese medicine syndrome elements in patients with primary insomnia,thereby providing ideas and methods for clinical diagnosis and treatment.
Methods:
The clinical data of 120 patients with primary insomnia in this area were collected by retrospective research method.The factors such as age,gender,course of disease,chief complaint and symptoms,syndrome were statistically analyzed.And the clinical syndrome elements and pattern combination rules of primary insomnia were obtained.The correlation between different syndrome elements and gender was discussed.
Results:
The age distribution of 120 patients was mainly middle-aged and elderly,with 43 cases (35.8%) in 51-60 years old group,and 53 cases (44.2%) in disease course >24 months group.Among the different syndrome element combinations of primary insomnia,the number of patients with double syndrome element combination (87 cases) was more than that of three syndrome element combination (33 cases),among which the number of liver stagnation and heat transforming fire syndrome (26 cases,21.7%) of double syndrome element combination was more than other combination.Fire heat (83 cases),Qi Stagnation (63 cases) and phlegm turbidity (48 cases) were more common in the disease nature syndrome elements,heart (57 cases),liver (63 cases) and spleen (26 cases) were more common in disease location syndrome elements;in 34 male patients,the top three syndrome elements were fire heat (24 cases),phlegm turbidity (20 cases),Qi Stagnation (16 cases).In 86 female patients,the top three syndrome factors were fire heat (59 cases),Qi Stagnation (47 cases) and Qi deficiency (32 cases).Among them,the number of cases of qi deficiency and phlegm turbidity between male and female was statistically significant (P<0.05).
Conclusion:
The main disease location syndrome elements involved in primary insomnia are:liver,gallbladder,heart,spleen,stomach and kidney;the main disease nature syndrome elements are:fire heat,qi stagnation,Qi deficiency,phlegm turbidity,blood deficiency,yin deficiency,and the pathogenesis of insomnia is mostly double syndrome elements.
