Pinocembrin Attenuates Hypoxia/Reoxygenation-Induced Cardiomyocyte Injury by Regulating Autophagy Mediated by HIF-1α/BNIP3 Signaling Pathway

Objective:
To explore the mechanism of pinocembrin in reducing myocardial cell injury induced by hypoxia/reoxygenation by regulating autophagy mediated by hypoxia-inducible factor 1α (HIF-1α)/Bcl2/adenovirus E1B-interacting protein 3 (BNIP3) signaling pathway.
Methods:
Rat cardiomyocytes H9c2 were cultured in vitro, after establishing the H/R model, the mice were treated with 0, 25, 50, 100, 200 and 300 μg/mL pinocembrin for 24 hours, the cell viability of each treatment group was detected by MTT method, and the appropriate drug concentration was screened out. The H9c2 cells cultured in vitro were randomly separated into a control group, a model group, a low-dose pinocembrin group, a high-dose pinocembrin group, a HIF-1α siRNA negative control group, and a high-dose pinocembrin + HIF-1α siRNA group. Except for the control group, H/R models were established in the other groups, and then treated with pinocembrin, HIF-1α siRNA and their negative controls. The proliferation and apoptosis of cells in each group were detected by MTT method and flow cytometry respectively; enzyme-linked immunosorbent assay (ELISA) was used to measure the levels of inflammatory factors interleukin (IL)-17, IL-1β and IL-18 in H9c2 cells in each group; kits were used to measure the levels of oxidative stress factors catalase (CAT), glutathione peroxidase (GSH-Px), reactive oxygen species (ROS) and malondialdehyde (MDA) in H9c2 cells in each group; monodansylpentanediamine (MDC) fluorescence staining was used to detect the formation of autophagosomes in H9c2 cells in each group; Western blotting was used to detect the expression of autophagy and HIF-1α/BNIP3 pathway-related proteins in H9c2 cells in each group.
Results:
Compared with the control group, the cell viability, the relative content of autophagosomes, the levels of CAT and GSH-Px, LC3-II/LC3-I, and the protein expressions of P62, HIF-1α and BNIP3 in the model group decreased (P<0.05), the apoptosis rate, the levels of IL-17, IL-1β and IL-18 in cell culture supernatant, and the levels of cellular ROS and MDA increased (P<0.05). Compared with the model group, the cell viability, the relative content of autophagosomes, the levels of CAT and GSH-Px, LC3-II/LC3-I, and the protein expressions of P62, HIF-1α and BNIP3 in the low-dose and high-dose pinocembrin groups increased (P<0.05), the apoptosis rate, the levels of IL-17, IL-1β and IL-18 in cell culture supernatant, and the levels of cellular ROS and MDA decreased (P<0.05); compared with those in the low-dose pinocembrin group, the cell viability, the relative content of autophagosomes, the levels of CAT and GSH-Px, LC3-II/LC3-I, and the protein expressions of P62, HIF-1α and BNIP3 in the high-dose pinocembrin group further increased (P<0.05), the apoptosis rate, the levels of IL-17, IL-1β and IL-18 in cell culture supernatant, and the levels of cellular ROS and MDA further decreased (P<0.05); there was no obvious difference in each index of cells in HIF-1α siRNA negative control group (P>0.05). Compared with the high-dose pinocembrin group, the cell viability, the relative content of autophagosomes, the levels of CAT and GSH-Px, LC3-II/LC3-I, and the protein expressions of P62, HIF-1α and BNIP3 in the high-dose pinocembrin+HIF-1α siRNA group decreased (P<0.05), the apoptosis rate, the levels of IL-17, IL-1β and IL-18 in cell culture supernatant, and the levels of cellular ROS and MDA increased (P<0.05).
Conclusion:
Pinocembrin promotes autophagy by up-regulating the HIF-1α/BNIP3 pathway, thereby reducing inflammation and oxidative stress, inhibiting hypoxia/reoxygenation-induced cardiomyocyte apoptosis and reducing cell injury.

Effect of CircBIRC6 Regulation of miR-126-5p/JARID2 Axis on Cisplatin Resistance in Non-Small Cell Lung Cancer Cells

Objective:
To investigate the influence of circular RNA BIRC6 (CircBIRC6) on cisplatin (DDP) resistance of non-small cell lung cancer (NSCLC) cells (A549).
Methods:
RT-qPCR was used to detect the expression of CircBIRC6 in DDP-resistant and sensitive lung cancer tissues and cells. Human NSCLC cell line A549 and DDP-resistant cells A549/DDP were cultured in vitro. Effect of circ_0000615 on Proliferation Migration and Invasion of Osteosarcoma Cells by Targeting miR-142-3p

Objective:
To explore the effect of circ_0000615 on the proliferation, migration and invasion of osteosarcoma cells and its molecular mechanism.
Methods:
The osteosarcoma tissues and adjacent tissues of 60 patients with osteosarcoma were selected, and real-time fluorescent quantitative PCR (RT-qPCR) was used to detect the expression levels of circ_0000615 and miR-142-3p; the osteosarcoma cells U2OS were divided into si-circ_0000615 group, si-NC group, miR-142-3p group, miR-NC group, si-circ_0000615+anti-miR-142-3p group, si-circ_0000615+anti-miR-NC group; tetramethylazolium salt colorimetric method (MTT) to detect cell inhibition rate; clone formation experiment to detect cell clone number; Transwell method to detect U2OS cell migration and invasion numbers; Western blot method to detect protein expression; dual luciferase reporter experiment to detect the targeting relationship between circ_0000615 and miR-142-3p.
Results:
Compared with the adjacent tissues, the expression level of circ_0000615 in osteosarcoma tissue was increased, and the expression level of miR-142-3p was decreased (P<0.05). After silencing circ_0000615 or overexpressing miR-142-3p, the inhibition rate of U2OS cells was increased, the number of clones, migration and invasion of U2OS cells were decreased, the expression level of E-cadherin was increased, and the expression level of N-cadherin was decreased (P<0.05). circ_0000615 targets and regulates miR-142-3p; inhibition of miR-142-3p can reduce the inhibitory effect of circ_0000615 on U2OS cell proliferation, migration and invasion.
Conclusion:
Silencing circ_0000615 can inhibit the proliferation, migration and invasion of osteosarcoma cells through targeted regulation of miR-142-3p.

Effect of circPTK2 Regulation of miR-619-5p on Invasion and Migration of Esophageal Cancer Cells

Objective:
To investigate the effect of circPTK2 on the invasion and migration of esophageal cancer (EC) cells by regulating miR-619-5p.
Methods:
QRT-PCR was performed to measure the expressions of circPTK2 and miR-619-5p in EC tissues and cell lines (OE19, KYSE-450, KYSE-520, TE-13). TE-13 cells were randomly separated into control group, si-NC, si-circPTK2 group, miR-NC group, miR-619-5p group, si-circPTK2+anti-NC group and si-circPTK2+anti-miR-619- 5p group. Cell viability analysis was performed with Cell Counting Box-8 (CCK-8); the cell invasion and migration ability were measured by Transwell assay; the expression of epithelial-mesenchymal transition (EMT)-related proteins (E-cadherin, N-cadherin, Vimentin) was measureed by western blot. The interaction between circPTK2 and miR-619-5p was verified by dual-luciferase reporter gene assay and RNA pull-down experiment. The role of circPTK2 in vivo was investigated using xenograft experiments.
Results:
The expression of circPTK2 was up-regulated in EC tissues and cells, and the expression of miR-619-5p was down-regulated (P<0.05). Downregulation of circPTK2 expression or overexpression of miR-619-5p inhibited cell growth, invasion, migration and EMT (P<0.05). MiR-619-5p was a target of circPTK2. Inhibition of miR-619-5p reversed the effects of circPTK2 downregulation on EC cell proliferation, invasion, migration and EMT (P<0.05). In addition, inhibition of miR-619-5p expression could partially restore the effect of downregulation of circPTK2 on tumor growth (P<0.05).
Conclusion:
CircPTK2 promotes the growth, invasion and migration of EC cells through sponge adsorption of miR-619-5p, circPTK2 may become a potential therapeutic target for EC metastasis and is a promising biomarker for predicting EC cell metastasis.

The Ameliorating Effect of Ononin on Renal Injury in Rats with Type 2 Diabetic Nephropathy via AMPK/SIRT1/FoxO1 Pathway

Objective:
To investigate the ameliorating effect of Ononin on renal injury in rats with type 2 diabetic nephropathy via AMPK/SIRT1/FoxO1 pathway.
Methods:
The DN model of rat was prepared by intraperitoneal injection of streptozotocin. The rats in the low-dose Ononin group and the high-dose Ononin group were given Ononin by gavage (the doses were 50mg/kg and 200mg/kg, respectively), and the positive control group was given metformin (250mg/kg) by gavage, and the negative control group and model group were given the same amount of normal saline by gavage for 16 weeks. The serum levels of BUN, Scr and blood sugar were measured, and pathological examinations of rat kidneys were performed. At the same time, Western Blot method was used to detect the protein levels of SOD, GSH-PX, MDA, AMPK, SIRT1, FoxO1, FN and LC3B in kidney tissue.
Results:
In the model group, the glomerulus was abnormally enlarged, the mesangial cells proliferated, the mesangial matrix increased, and the interstitial fibrosis. After Ononin and metformin treatment, the glomerular morphology gradually returned to normal, The degree of inflammation and fibrosis transformation in the glomerulus were also improved in varying degrees. Compared with the negative control group, the levels of BUN, Scr, blood sugar, MDA, FoxO1 and FN in the model group, Ononin low-dose group, Ononin high-dose group and positive control group increased, SOD, GSH -PX, AMPK, SIRT1 and LC3B levels decreased(P<0.05). Compared with the model group, the levels of BUN, Scr, blood sugar, MDA, FoxO1 and FN in the low-dose Ononin group, high-dose Ononin group and the positive control group decreased, and SOD, GSH-PX, AMPK, SIRT1 and LC3B increased(P<0.05). Compared with the low-dose Ononin group, the levels of BUN, Scr, blood glucose, MDA, FoxO1 and FN in the high-dose Ononin group and the positive control group decreased, and the levels of SOD, GSH-PX, AMPK, SIRT1 and LC3B increased(P<0.05). There was no significant difference in indexes between the high-dose Ononin group and the positive control group(P>0.05).
Conclusion:
Ononin has a significant improvement effect on kidney injury in rats with type 2 diabetic nephropathy, and its mechanism may be related to the activation of AMPK/SIRT1/FoxO1 pathway by Ononin.

Influences of Resveratrol on Periodontal Tissue Healing and Sirt1/Nrf2 Signaling Pathway after Tooth Transplantation in Rats

Objective:
To observe the influences of resveratrol (RSV) on the periodontal tissue healing and Sirt1/Nrf2 signaling pathway after tooth transplantation in rats.
Methods:
48 male Lewis rats were randomly divided into control (Control) group, model (Model) group, low-dose RSV (L-RSV) group, and high-dose RSV (H-RSV) group. Hematoxylin-eosin (HE) staining and tartrate-resistant acid phosphatase (TRAP) staining were used to observe the pathological changes in periodontal tissues. Immunohistochemical staining was used to detect nuclear factors κB receptor activator ligand (RANKL), osteoprotegerin (OPG), runt-related transcription factor 2 (Runx2), osteocalcin (OCN), and other osteogenic differentiation-related factors. The root resorption was analyzed by scanning electron microscope. The expression levels of SIRT1 and Nrf2 proteins in periodontal tissue were detected by Western blot.
Results:
Compared with the control group, the number of TRAP-positive cells in the periodontal tissue of the model group increased; the expression of RANKL was increased, and the expressions of OPG, RUNX2, and OCN were decreased; the root resorption rate was increased; the protein levels of Sirt1 and Nrf2 were decreased (P<0.05). Compared with the model group, the number of TRAP-positive osteoclasts in the periodontal tissue of the L-RSV group and the H-RSV group decreased; the expression of RANKL was decreased, and the expressions of OPG, RUNX2, and OCN were increased; the root resorption rate was significantly decreased; Sirt1, Nrf2 protein levels increased (P<0.05).
Conclusion:
RSV can promote the healing of periodontal tissue after tooth transplantation in rats, and the promotion may be related to the activation of the Sirt1/Nrf2 signaling pathway.

Experimental Study of miR-106b Mediated RANKL/RANK/OPG Pathway Against Osteoporosis

Objective:
To explore the anti-osteoporosis effect of mirco ribonucleic acid 106b (miR-106b) mediated activator receptor of nuclear factor κB ligand (RANKL)/receptor activator of nuclear factor κB (RANK)/osteoprotegerin (OPG) pathway.
Methods:
Sixty Wistar rats were ovariectomized to prepare the osteoporosis rat model, and another 10 rats were only removed the fat around the ovary as the sham operation group. The 51 rats successfully modeled were randomly divided into model group, miR-106b mimics-NC group, miR-106b mimics group, miR-106b inhibitor-NC group and miR-106b inhibitor group.All groups except the model group were transfected with miR-106b mimic negative control, miR-106b mimic, miR-106b inhibitor negative control and miR-106b inhibitor respectively, and 10 rats were removed only the fat near the ovary as the sham operation group.X-ray bone densitometry was used to measure the bone mineral density of the rat femur; hematoxylin-eosin (HE) staining was used to observe the histopathological changes of the rat right femur; real-time fluorescence quantitative polymerase chain reaction (RT-qPCR) and immunoblotting (WB) were used to detect miR-106b expression, RANKL, RANK, OPG messenger ribonucleic acid (mRNA) expression and protein expression of the rat right femur respectively; dual luciferase reporter gene assay was performed to verify the targeting relationship between miR-106b and RANKL.
Results:
Bone mineral density, miR-106b expression, OPG mRNA and protein expression were increased (P<0.05), RNAKL mRNA and protein expression were decreased (P<0.05) in the right femoral tissue of the miR-106b mimics group compared to the model group and miR-106b mimics-NC group.Compared with the model group and miR-106b inhibitor-NC group, BMD, miR-106b expression, OPG mRNA and protein expression in the right femoral tissue of the miR-106b inhibitor group were reduced (P<0.05) and RNAKL mRNA and protein expression were increased (P<0.05).In the sham-operated group, a large number of bone trabeculae were closely arranged and of uniform thickness; in the model group, miR-106b mimics-NC group and miR-106b inhibitor-NC group, the number of bone cells was reduced, the arrangement was disordered and the gap between bone trabeculae was large; in the miR-106b mimics group, the number of bone cells was increased, the arrangement was tight and the thickness of bone trabeculae was increased; in the miR-106b inhibitor group showed a lack of osteoblasts, disorganized arrangement and large trabecular gaps. The dual luciferase reporter gene assay confirmed that miR-106b targets and regulates RANKL.
Conclusion:
Up-regulation of miR-106b expression inhibited RANKL expression, promoted OPG expression, increased femoral bone mineral density and reduced histopathological damage to the right femur in osteoporotic rats.

Chromosome Karyotypic Analysis of 167 Cases with Chronic Myeloid Leukemia

Objective:
To explore the significance of chromosomal karyotype changes at different stages in the diagnosis, treatment, progression and prognosis of chronic myeloid leukemia(CML).
Methods:
From July 2019 to Deceber 2021, 167 newly diagnosed CML patients were treated in The First Affiliated Hosptital of Air Force Military Medical University. Chromosome preparation was made using 24 hours short-term culture of bone marrow cells, and R-banding technique was used for chromosome karyotype analysis. The genetic characteristics and significance of the disease were observed.And an analysis was conducted of the differences between chromosomal variant translocations and additional chromosomal abnormalities and classical Ph+ CML in the diagnosis and treatment of the disease.
Results:
In the 167 cases of CML patients, 104 cases was male, 63 cases was female, and the age range was between 11 and 86. By analyzing the karyotype of 167 cases CML pateints, 7 cases (4.2%) were non mitotic, 154 cases were Ph chromosome positive (Ph+) and 6 cases were Ph chromosome negative (Ph-). In the 154 cases of Ph+ pateints, 126 cases were classical Ph+ and 28 cases were variant rearrangements or additional chromosome abnormalities, include +8, +Ph, -Y, der (17), del (3p), del (15q), del (11p), add (21q), add (2q) and so on, the majority were +8, +Ph, -Y, der (17).
Conclusion:
In the chromosome karyotypic analysis of CML, Ph chromosome was the genetic marker. The complexity of chromosome karyotype is closely related to CML treatment, progress and prognosis. The more complexity of the chromosomes, the worse prognosis of the patient.

Correlation Studies of Programmed Death Ligand 1 and EGFR Gene Mutations in Non-Small Cell Lung Carcinoma

Objective:
To find out the reason for poor immunotherapy effect of epidermal growth factor receptor (EGFR) mutant non small cell lung carcinoma (NSCLC) by analyzing the correlation between programmed cell death-ligand 1 (PD-L1) expression and EGFR gene mutation status, and to provide reference for precise diagnosis and treatment of EGFR-mutant NSCLC patients.
Methods:
A total of 308 NSCLC patients who underwent EGFR gene detection and PD-L1 immunohistochemical staining at the same time in the Department of Pathology of Tangshan Gongren Hospital from 2020 to 2021 were collected. The relationship between PD-L1 expression intensity and EGFR gene mutation rate, and the correlation between EGFR mutation status and PD-L1 expression intensity, were analyzed.
Results:
EGFR mutation rates were 71.30%, 57.55%, 50.00% and 44.12% (P=0.017, r=-0.176) in PD-L1 negative, weakly positive, moderately positive and strongly positive patients, respectively. The low and high expression rates of PD-L1 were 74.19% and 25.81% in the EGFR mutant NSCLC, and 62.30% and 37.70% in the wild type (P=0.027).
Conclusion:
There is a negative correlation between the expression intensity of PD-L1 and EGFR mutation rate. The higher the expression of PD-L1, the lower the mutation rate of EGFR. Patients with EGFR mutant NSCLC were more likely to have low expression of PD-L1.

Role of Dexmedetomidine in Perioperative Inflammation Inhibition and Prognosis in Elderly Hepatocellular Carcinoma

Objective:
To evaluate the role of dexmedetomidine in the perioperative inhibition of inflammation and early recovery in elderly patients undergoing hepatectomy.
Methods:
One hundred and fifty elderly patients, regardless of gender, with BMI 18.5 to 24.9 kg/m

, 65 to 80 years old, ASA grade II or III, and Child-pugh grade A or B, were proposed for elective hepatectomy. A random number table was used to simply randomly divide the enrolled cases into two parts: an experimental group and a control group, each with 75 cases. In the experimental group, dexmedetomidine 0.2 μg/(kg-h) was continuously pumped and maintained until 30 min before the end of surgery; in the control group, the same dose of 0.9% NaCl was pumped intravenously at the same rate. To investigate the time of minimally invasive operation for acute severe cholecystitis

Objective:
To investigate the timing of minimally invasive surgery for acute severe cholecystitis.
Methods:
Fifty five patients with acute severe cholecystitis who visited our hospital from January 2018 to December 2021 were selected as the study subjects, and the patients were divided into LC group (25 cases) and PTGD combined with LC group (30 cases) according to the different surgical timing of laparoscopic cholecystectomy (LC). The LC group all underwent LC, and the PTGD combined with LC group received LC before 5-8 weeks after initial ultrasound-guided percutaneous transhepatic cholecystocentesis catheter drainage (PTGD). Patient related clinical indicators were compared between the two groups.
Results:
there were no significant differences in age, gender, body mass index (BMI), and society of anesthesiologists (ASA) grade between the two groups (P>0.05), and the gallbladder wall thickness measured by preoperative transabdominal color ultrasound in the LC group was greater than that in the PTGD combined with LC group (P<0.001). Compared with the LC group, the PTGD combined with LC group had a shorter operation time, less intraoperative blood loss and intraoperative adhesion of the gallbladder trigone, a shorter time for the return of white blood cells and body temperature to normal, the time for the removal of abdominal drainage tube and the time for the first postoperative flatus (P<0.05); PTGD combined with LC group had postoperative alanine transaminase γ- The overall incidences of glutamyltransferase, C-reactive protein, procalcitonin, and postoperative complications were lower in the LC group, and the postoperative prealbumin and superoxide dismutase levels were higher in the LC group (all P<0.001). The incidence ratio of conversion to open was 0.00%: 3.33% in LC group and PTGD combined with LC group patients, and there was no statistical difference between groups (P>0.05).
Conclusions:
in patients with acute severe cholecystitis, LC performed after ultrasound-guided PTGD is a safe and feasible treatment to relieve the degree of intraoperative adhesion, promote early and rapid postoperative recovery, attenuate the inflammatory response of the gallbladder before and during surgery, improve liver function status, and also reduce the incidence of postoperative complications.

Correlation between miR-762 and sTREM-1 sFLT-1 in Acute Lung Injury

Objective:
To observe the expression of miR-762 in plasma of patients with acute lung injury and explore its relationship with potential value.
Methods:
A total of 60 patients with lung injury admitted to our hospital from September 2018 to September 2021 were collected and divided into medium low risk group and high risk group according to clinical symptoms and mMRC scores. Scores were analyzed with acute physiology and chronic health evaluation (APACHE Ⅱ). Human soluble myeloid cell trigger receptor-1 (sTREM-1) kit and human soluble FMS like tyrosine kinase 1 (sFlt-1) enzyme linked immunosorbent assay (ELISA) kit were used to detect the contents of sTREM-1, sFlt-1 in plasma; real time fluorescence quantitative polymerase chain reaction (RT-qPCR) was used to detect the expression of miR-762 in plasma. Receiver operating characteristic curve (ROC) was plotted to judge the diagnostic value and cut-off point of miR-762 in patients with lung injury.
Results:
The contents of sTREM-1 and sFlt-1, the relative expression of miR-762 of high-risk group and middle-low-risk group were all significantly higher than those of normal group. Compared with the middle-low-risk group, the contents of sTREM-1, sFlt-1 and the relative expression of miR-762 were substantially increased in the high-risk group in plasma. ROC curve showed that the relative expression of miR-762 was used to predict the area under curve (AUC) = 0.987, 95% confidence interval (CI): (0.9688 - 1.000), and the cut-off point was 1.210; AUC = 1.00, 95% CI: (1.000-1.000), with a cut-off point of 2.396. The scores of miR-762 and APACHE Ⅱ markedly increased, and the contents of sTREM-1 and sFlt-1 were positively correlated. The correlation coefficients (r) were 0.386, 0.321, 0.432.
Conclusion:
The high expression of miR-762 in plasma from patients with acute lung injury was related to the aggravation of lung injury, and it was a predictive marker of acute lung injury.

Correlation between Serum Chemokine, RNAKL and 25- Hydroxyvitamin D Levels and Disease Progression in Rheumatoid Arthritis

Objective:
To analyze the correlation of serum chemokine, RNAKL and 25- hydroxy vitamin D [25(OH)D] levels with disease progression in rheumatoid arthritis.
Methods:
289 patients with rheumatoid arthritis diagnosed and treated in our hospital from January 2019 to December 2021, and 100 healthy subjects who underwent physical examination in our hospital simultaneously, were included for the study. RA patients were divided into RA remission group and RA activity group using disease activity score 28-jointcount. Fasting serum levels of C-C chemokine ligand 20 (CCL20), RNAKL and 25(OH)D were determined in RA patients and healthy subjects.And DAS28 score was used as a variable of RA disease progression to analyze the correlation between the above indicators and RA disease progression.
Results:
Compared with the control group, the serum levels of CCL20 and RNAKL in RA patients were significantly increased, while the level of 25(OH)D was significantly decreased, the difference was statistically significant(P<0.05). Serum levels of CCL20 and RNAKL in RA activity group were significantly higher than those in RA remission group, the difference was statistically significant(P<0.05). Serum 25(OH)D levels in RA activity group were significantly lower than those in RA remission group, the difference was statistically significant(P<0.05). Using DAS28 score as a variable for RA disease progression, correlation analysis showed that serum CCL20 and RNAKL levels were positively correlated with the DAS28 score (P<0.05), while serum 25(OH)D levels were negatively correlated with the DAS28 score (P<0.05).
Conclusion:
Serum levels of CCL20, RNAKL and 25(OH)D are correlated with the progression of RA, and CCL20, RNAKL and 25(OH)D can be used as biological indicators to monitor disease activity and assist doctors to judge patients' conditions comprehensively.

Serum Omentin-1 Changes in Osteonecrosis of the Femoral Head

Objective:
To analyze the relationship between serum retinol-1 (Omentin-1) and non-traumatic osteonecrosis of the femoral head (NONFH).
Methods:
The study included 82 patients with NONFH as the patient group and 77 healthy subjects as the healthy group, and serum retinoid-1, total cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL), and low-density lipoprotein (LDL) levels were measured in both groups. Serum omentin-1 levels were compared between different etiologies, ARCO stages, and before and after collapse, and the correlation between serum omentin-1 levels and disease severity and lipid levels was analyzed to assess the value of serum omentin-1 concentrations in disease assessment using ROC curves.
Results:
The level of serum retinin-1 in NONFH was significantly lower than that in healthy controls (P<0.01), but there was no significant difference among patients with osteonecrosis of the femoral head (P>0.05). The level of serum retinin-1 in patients with femoral head collapse was significantly lower than that before collapse (P<0.01). The level of serum retinin-1 in patients with bilateral disease was lower than that in patients with unilateral disease, but the difference was not statistically significant (P>0.05). There was a significant difference in the level of serum retinol-1 among patients with different ARCO stages, and the level of serum retinin-1 was negatively correlated with ARCO stage, VAS score and Harris score, and negatively correlated with TC, TG and HDL. ROC curve analysis showed that the level of serum retinin-1 had a certain significance in the diagnosis of NONFH.
Conclusion:
The decrease of serum retinol-1 level can reflect the severity of NONFH disease and has certain significance for the clinical diagnosis and treatment of NONFH.

Evaluation of the Efficacy of Pressure-Regulated Volume-Control Ventilation Mode in Mechanical Ventilation Therapy of ARDS Patients

Objective:
To explore the application effect of pressure-regulated volume control in the treatment of acute respiratory distress syndrome.
Methods:

Effects of Auer Bodies on the Prognosis of Newly Diagnosed Non-M3 Acute Myeloid Leukemia Patients

Objective:
To analyze the effect of Auer bodies on the prognosis of newly diagnosed non-M3 acute myeloid leukemia (AML) patients.
Methods:
From January 2017 to January 2022, 124 newly diagnosed non-M3 AML patients in Chizhou People's Hospital were retrospectively selected as the research subjects, and the patients were divided into Auer body positive group and Auer body negative group according to the presence or absence of Auer bodies in bone marrow blast cells. In the negative group, the clinical characteristics such as age, gender, disease risk classification, laboratory indicators, and success rate of induction of remission were recorded. The clinical outcome and prognosis of the patients were followed up, and the Kaplan-Meier survival curve was plotted to compare the overall survival (OS) of the two groups of patients.
Results:
Among the 124 non-M3 AML patients, 54 were Auer body positive and 70 were Auer body negative. There was no difference between the two groups in age, sex, medical history, fever, bleeding, pain, anemia, and disease risk classification. There was statistical significance (P>0.05), and there was a statistically significant difference in the proportion of combined cardiovascular disease and digestive system disease (P<0.05); there was no significant difference in laboratory indicators WBC, PLT, Hb, and LDH between the two groups (P>0.05). There was no significant difference in the proportion of fusion genes (P>0.05). There was no significant difference in the complete remission rate and recurrence rate between the two groups, P>0.05, but the mortality rate of the Auer body positive group was lower than that of the Auer body negative group, and the difference was statistically significant (P<0.05); Kaplan-Meier survival curves showed no statistically significant difference in progression-free survival (DFS) between the two groups (P>0.05), but the OS of the Auer body positive group was higher than that of the Auer body negative group, and the difference was statistically significant (P<0.05).
Conclusion:
Auer bodies are closely related to the prognosis of newly diagnosed non-M3 AML patients. Patients with positive Auer bodies are less likely to have cardiovascular disease, are more likely to have digestive system symptoms, and have a lower mortality rate. The overall survival and prognosis are better than those with negative Auer bodies. Attention should be paid.

Objective:
To explore the clinical effect of non-invasive ventilator combined with high flow oxygen therapy on patients with acute exacerbation of COPD complicated with respiratory failure.
Methods:
The data of 133 patients with acute exacerbation of chronic obstructive pulmonary disease (COPD) and respiratory failure in the hospital were retrospectively analyzed. Among the patients, 66 cases treated with non-invasive ventilator-assisted ventilation were included in control group, and 67 cases given non-invasive ventilator-assisted ventilation and high-flow nasal cannula were enrolled as combined group. The efficacy, and clinical symptoms, blood gas indicators, serum biochemical factors and clinical treatment indicators before and after treatment were compared between the two groups.
Results:
The clinical efficacy of combined group was significantly higher than that of control group (P<0.05). The clinical symptoms of both groups after treatment were significantly reduced (P<0.05), and the SpO

, OI and PH were increased significantly (P<0.05), and the increases of combined group were significantly higher than those of control group (P<0.05). The PaCO

The mechanical ventilation time and ICU stay of combined group were significantly shorter than those of control group (P<0.05), while the intermittent treatment time was significantly longer than that of control group (P<0.05).
Conclusion:
Non-invasive ventilator combined with high-flow nasal cannula in the treatment of acute exacerbation of COPD with respiratory failure can significantly improve the clinical symptoms, enhance the respiratory quality, and reduce the body’s inflammation.

Analysis of the Curative Effect of Combined PCI and IABP in the Treatment of Patients with Myocardial Infarction

Objective:
To explore the effect of aortic balloon counterpulsation (IABP) combined with percutaneous coronary intervention (PCI) in the treatment of acute myocardial infarction (AMI) patients on postoperative cardiac dysfunction.
Methods:
The general clinical data of 81 patients with AMI admitted to the hospital between August 2014 and April 2021 were retrospectively analyzed. Patients enrolled were grouped according to the treatment method. Those treated with PCI alone were included in the control group (n=39), and those treated with IABP combined PCI were included in the study group (n=42). The changes in cardiac function, hemodynamics, serum Brain natriuretic peptide (BNP), High-sensitivity troponin T (TNT-HS), Creatine kinase isozyme (CK-MB) levels were compared between the two groups. Adverse reactions were recorded.
Results:
After 1 week of treatment, TNT-HS, CK-MB, BNP, pulmonary wedge pressure (PCWP), left ventricular end diastolic diameter (LVEDd) and left ventricular end systolic diameter (LVESD) were significantly decreased in the two groups, and the invasive mean arterial pressure (MABP), invasive arterial systolic pressure (SBP) and left ventricular ejection fraction (LVEF) were increased, and the difference between the above indicators in the study group before and after treatment was significantly higher than that in the control group(P<0.05). The incidence rates of adverse cardiovascular events such as malignant arrhythmia, recurrent myocardial infarction, and heart failure in the study group were significantly lower than those in the control group, and the differences were statistically significant (P<0.05).
Conclusion:
Combined treatment of PCI and IABP for patients with AMI can effectively improve their hemodynamics, reduce BNP, CK-MB and TNT-HS levels, and effectively promote cardiac function recovery.

Distribution of Pathogenic Bacteria of Pulmonary Infection after Esophageal Cancer Surgery and Relationship with Serum TNF-α IL-1β and IL-8 of Patients

Objective:
To explore the distribution of pathogenic bacteria of pulmonary infection after esophageal cancer surgery and relationship with serum TNF-α, IL-1β and IL-8 of patients.
Methods:
98 patients with elective thoracoscopic radical resection of esophageal cancer in the hospital between January 2020 and January 2021 were selected as the research subjects. According to postoperative pulmonary infection, the patients were divided into the infected group (32 cases) and uninfected group (66 cases). According to the CURB-65 scoring standard for the pulmonary infection degree, the patients with pulmonary infection were classified into mild group (14 cases), moderate group (11 cases) and severe group (7 cases). The distribution status of pathogenic bacteria in infected group were analyzed. Postoperative peripheral blood samples were collected from all patients, and enzyme-linked immunosorbent assay was used to detect the levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and interleukin-8 (IL-8). Multivariate logistic regression analysis of risk factors for postoperative pulmonary infection in patients with esophageal cancer. Spearman rank correlation analysis was applied to analyze the correlation between the above inflammatory factors and CURB-65 score.
Results:
44 strains of pathogenic bacteria were isolated from 32 infected patients, including 26 strains (59.09%) of Gram-negative strains, 15 strains (34.09%) of Gram-positive strains and 3 strains (6.82%) of fungi. The proportions of patients with long-term smoking history, diabetes mellitus, tumors located in the upper/middle segment and CURB-65 score>2 in infected group were higher than those in uninfected group (P<0.05). The levels of serum TNF-α, IL-1β and IL-8 were higher in infected group than those in uninfected group (P<0.05). Serum levels of TNF-α, IL-1β and IL-8 were higher in severe group than those in mild group and moderate group, and were higher in moderate group than those in mild group (P<0.05). Multivariate Logistic regression analysis showed that long-term smoking history, diabetes mellitus, tumor located in the upper/middle segment, CURB-65 score>2 points, TNF-α≥7.44 μg/L, IL-1β≥38.21 μg/L and IL-8≥34.25 μg/L were risk factors for pulmonary infection in patients after esophageal cancer surgery (P<0.05). Serum TNF-α, IL-1β and IL-8 levels in patients with pulmonary infection were positively correlated with CURB-65 score (r=0.516, 0.589, 0.536, P<0.05).
Conclusion:
Serum TNF-α, IL-1β and IL-8 are highly expressed in patients with pulmonary infection after esophageal cancer surgery, and the levels of serum TNF-α, IL-1β and IL-8 are closely related to the severity of pulmonary infection. The occurrence and development of postoperative pulmonary infection in patients with esophageal cancer can be assessed clinically by detecting the levels of serum TNF-α, IL-1β and IL-8.

Clinical Evaluation of Transumbilical Single-Port Laparoscopic Surgery for Children with Cryptorchidism

Objective:
To compare the curative effect of transumbilical single-port laparoscopic surgery and traditional three-port laparoscopic surgery in children with cryptorchidism.
Methods:
A total of 60 pediatric patients with cryptorchidism admitted to our hospital from January 2020 to December 2021 were selected. Among them, 30 children were assigned to the single-hole group by umbilical single-hole laparoscopic surgery. During the operation, a 1cm incision was taken in the umbilical and a disposable four-channel trocar puncture device was placed open. The other 30 children were divided into the three-hole group by traditional three-hole laparoscopic surgery. During the operation, 0.5~1cm incision was made in the umbilical and left and right McIntosh points respectively, and disposable poke cards were inserted respectively. The instruments were operated into the abdominal cavity by three poke cards. The operative time, intraoperative blood loss, length of hospital stay, hospitalization cost, incidence of postoperative complications and aesthetic degree of postoperative incision were compared and analyzed between the two groups.
Results:
Sixty cases of children successfully completed the operation, the testicular sutured at the lower pole of the scrotum in a satisfactory position, spermatic cord vascular tension is not high. There were no significant differences in operation time, intraoperative blood loss and hospital stay between the single-well group and the three-well group (P>0.05). The hospitalization cost of single well group was higher than that of three well group, and the difference was statistically significant (P<0.05). The incision aesthetics of single hole group is better than that of three hole group.
Conclusion:
Transumbilical single-port laparoscopic surgery for the treatment of children cryptorchidism is safe, feasible, does not increase the operation time, beautiful incision without scar, should be popularized in clinical application.

Predictive Value of Integrated Neck Brain Ultrasound Combined with Red Blood Cell Distribution Width in Coronary Heart Disease

Objective:
To explore the diagnostic value of cervical cerebral integrated ultrasound combined with red blood cell distribution width in coronary heart disease.
Methods:
80 patients with coronary artery stenosis diagnosed by coronary angiography (CAG) in the Department of Cardiology of our hospital from January 2019 to December 2020 were selected as the observation group, and 80 patients without coronary artery stenosis diagnosed by coronary angiography were selected as the control group. The levels of transcranial Doppler ultrasound (TCD) detection parameters (mean flow velocity VM), transcranial vascular ultrasound detection parameters (IMT) and red blood cell distribution width (RDW) in each group were compared.
Results:
The mean flow velocity (VM), common carotid artery intima-media thickness (IMT) and RDW levels of MCA, ACA and PCA in the observation group were significantly higher than those in the control group (P<0.05). The area under the ROC curve of VM, IMT, RDW and combined detection for diagnosing coronary heart disease were 0.83, 0.81, 0.77 and 0.91, respectively. There were statistically significant differences in the area under the ROC curve of each diagnosis of coronary heart disease(P<0.05). When VM ≥ 2.03m/s, IMT ≥ 1.05mm and RDW ≥ 14.65% were present at the same time, the diagnostic sensitivity and specificity were 91.25% and 98.75%, respectively. The sensitivity and specificity of parallel combined detection were significantly better than that of general single test (P<0.05).
Conclusion:
The level of detection parameters determined by cervical cerebral integrated ultrasound combined with red blood cell distribution width is closely related to the occurrence of coronary heart disease. It has great diagnostic value for the diagnosis of coronary heart disease, and has great economic and social value.

Clinical Application of Extracorporeal Shock Wave Combined with Eccentric Exercise Training in Patients with Patellofemoral Pain Syndrome

Objective:
To investigate the clinical effect of extracorporeal shock wave combined with eccentric exercise training on patients with patellofemoral pain syndrome.
Methods:
A total of 124 patients with patellofemoral pain syndrome admitted to our hospital from January 2019 to December 2021 were selected for research, and they were divided into the experimental group and the control group by a simple random method using a random number table, 62 cases in each group. Both groups received conventional rehabilitation training, and extracorporeal shock wave treatment was given to the control group, and extracorporeal shock wave combined with centrifugal exercise training was given to the test group. After 8 weeks of treatment, the pain visual analog scale (VAS) and Kujala Patellofemoral Score (KPS) were compared between the two groups before and after treatment, and the following parameters in X-ray were analyzed: distal femur valgus angle (distal femur valgus angle) , DFVA), proximal tibial varus angle (PTVA), femoral angle (FA), tibia angle (TA), tibiofemoral angle (TFA), Insall- Salvati index (ISR), ditch angle (SA), lateral patella angle (LPA), congruence angle (CA), patellofemoral index (PI), and Berg balance scale.
Results:
The difference of VAS scores in the experimental group before and after treatment was greater than that in the control group (P<0.05); the difference in KPS scores in the experimental group before and after treatment was greater than that in the control group (P<0.05); the values of ISR, SA, PA and PI in the experimental group after treatment were lower than those in the control group group (P<0.05); TFA in the experimental group was higher than that in the control group after treatment (P<0.05), and there was no significant difference in DFVA, PTVA, FA, TA, and CA values between the two groups before and after treatment (P>0.05).
Conclusion:
Extracorporeal shock wave combined with eccentric exercise training can effectively relieve pain in patients with patellofemoral pain syndrome in the short term, and also has a significant effect in improving knee joint function, but further research is needed to evaluate its therapeutic effect from various aspects.

The Efficacy of Binocular Vision Adjustment Training on Myopia in Children and the Effect on Visual Function

Objective:
To explore the effect of binocular vision adjustment training on children's myopia and visual function.
Methods:
A total of 180 myopic children who were admitted to our hospital from November 2019 to November 2021 were prospectively selected as the research subjects. They were divided into groups A and B by simple random coin toss method. Finally, 98 cases were included into group A and 82 cases in group B. Children in group B wore single vision glasses, and children in group A received binocular vision adjustment training on this basis, and both groups were treated for 6 months. The clinical efficacy of the two groups of children was compared, and the differences in visual acuity (mean refraction, pooled near point value, regulated pool to regulation ratio) and binocular regulation function (regulation amplitude, regulation sensitivity, regulation response) between the two groups before and after 6 months of treatment were compared. The American Academy of Ophthalmology questionnaire was used to evaluate the quality of life of the children before treatment and after 6 months of treatment.
Results:
The total effective rate of treatment for children in group A was 90.82% (89/98) significantly higher than that of group B, 79.27% (65/82), P<0.05; After 6 months of treatment, the mean refractive error and pooling near point values decreased in both groups and the difference before and after treatment was greater in group A than in group B. The ratio of adjusted pooling to adjustment increased and the difference before and after treatment was greater in group A than in group B. P<0.05; After 6 months of treatment, the amplitude of regulation, the sensitivity of regulation, and the level of regulation response increased in both groups and the difference before and after treatment was greater in group A than in group B. P<0.05; After 6 months of treatment, the quality of life scores of children in both groups improved and the difference before and after treatment was greater in group A than in group B. P<0.05.
Conclusion:
Binocular vision adjustment training for myopic children has a significant effect, can significantly improve children's vision, adjust visual function, and effectively improve their quality of life, which is worthy of clinical reference.

Effect of Breast-Conserving Surgery Combined with Mastoscopic Axillary Lymph Node Dissection on Short-Term and Long-Term Efficacy in Patients with Early Breast Cancer

Objective:
To analyze the effect of breast-conserving surgery (BCS) combined with mastoscopic axillary lymph node dissection (MALND) on short-term and long-term efficay in patients with early breast cancer (BC).
Methods:
A total of 98 BC patients treated between January 2017 and December 2018 were selected, and divided into observation group (BCS combined with MALND) and control group (BCS combined with conventional lymph node dissection) according to the random number table method, with 49 cases in each group. Intraoperative and postoperative indicators, upper limb activities, social fitness, short-term and long-term survival, and complications were compared between the two groups.
Results:
The operation time and hospital stay of observation group were shorter than those of the control group (P<0.05). The intraoperative blood loss and postoperative drainage volume of observation group were less than those of the control group (P<0.05). There was no significant difference in upper limb activities between the two groups before operation (P>0.05). The ranges of internal rotation, external rotation, adduction, abduction, abduction, anteflexion and retroflexion were reduced at 6 months after operation (P<0.05), and the difference in the ranges of observation group were greater than those of the control group (P<0.05). There was no significant difference in social fitness between the two groups before operation (P>0.05). The Social Adaptation Questionnaire (SAQ) scores and Functional Assessment of Cancer Therapy-Breast (FACT-B) scores were significantly increased at 6 months after operation (P<0.05), and the scores difference in the observation group had higher than the control group (P<0.05). All subjects were followed up, survival analysis showed that the median survival time of observation group and control group was 38 months and 37 months, respectively. There was no statistically significant difference in overall survival between the two groups (P>0.05). The total incidence of complications in the observation group was lower than that in the control group (P<0.05).
Conclusion:
BCS combined with MALND for treating early BC can shorten the operation time and hospital stay, and promote recovery. It also can improve upper limb activities and the patients’ social fitness after surgery, with few complications.

Clinical Application of Neoadjuvant Chemotherapy Combined with Breast Conserving Surgery in Patients with Early-Stage Breast Cancer

Objective:
To investigate the clinical effect of neoadjuvant chemotherapy combined with breast conserving surgery on patients with early-stage breast cancer.
Methods:
A total of 90 patients with early-stage breast cancer in our hospital from January 2018 to December 2021 were selected and randomly divided into the control group of 45 cases and the observation group of 45 cases according to the random number table method. The control group was treated with breast conserving surgery only, and the observation group was treated with neoadjuvant chemotherapy combined with breast-conserving surgery. The short-term efficacy, surgical indicators and quality of life were compared between the two groups; Her-2 and Ki67, tumor markers [carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125) and Carbohydrate antigen 153 (CA153)] were different before and after treatment.
Results:
The overall effective rate in the observation group (91.11%) was higher than that in the control group (75.56%) (P<0.05). The observation group had less intraoperative bleeding than the control group, fewer complications than the control group, and shorter operative time than the control group (P<0.05). The KPS scores of early breast cancer patients in the 2 groups were higher after treatment than before treatment (P<0.05); the magnitude of difference before and after treatment was higher in the observation group than in the control group (P<0.05).Serum Her-2 and Ki67 levels were lower in the 2 groups than before treatment (P<0.05); the difference in the observation group was higher than that in the control group before and after treatment (P<0.05). Serum CEA, CA125 and CA153 levels were lower in the 2 groups than before treatment (P<0.05); the difference in the observation group was higher than that in the control group before and after treatment (P<0.05).
Conclusion:
Neoadjuvant chemotherapy combined with breast-conserving surgery is effective in the near term for patients with early-stage breast cancer, significantly improving the quality of survival, reducing Her-2 and Ki67 levels, and lowering CEA, CA125 and CA153 levels.

Effects of Cervical Discectomy Combined with Subtotal Vertebrectomy and Bone Graft Internal Fixation on Clinical Efficacy, Neurological Function and Pain in the Treatment of Patients with Multilevel Cervical Spondylotic Myelopathy

Objective:
To explore the effects of cervical discectomy combined with subtotal vertebrectomy and bone graft internal fixation on clinical efficacy, neurological function and pain in patients with multilevel cervical spondylotic myelopathy (MCSM).
Methods:
A total of 103 patients with MCSM were selected. Among them, 46 cases with posterior open-door spinal canaloplasty and mini-titanium plate internal fixation were included in control group, and 57 cases with cervical discectomy combined with subtotal vertebrectomy and bone graft internal fixation were included in observation group. The surgical situation and hospital stay, clinical efficacy, spinal cord nerve function [Japanese Orthopedics Association (JOA) score], Neck Disability Index (NDI), pain [Visual Analogue Scale (VAS)], cervical range of motion and complications were compared between the two groups.
Results:
Compared with control group, the surgical time was prolonged, intraoperative blood loss was reduced, and hospital stay was shortened in observation group (P<0.05). The total effective rate of treatment was 71.93% (41/57) in observation group and 69.56% (32/46) in control group respectively (P>0.05). At 6 months after surgery, comparison of differences of JOA score, NDI score and pain VAS score before and after surgery showed no statistical significance between the groups (P>0.05), and the difference of cervical range of motion before and after surgery in observation group was greater than that in control group (P<0.05). The incidence rate of postoperative complications of 5.26% in observation group was lower than 19.56% in control group (P<0.05).
Conclusion:
Cervical discectomy combined with subtotal vertebrectomy and bone graft internal fixation has short-term efficacy on patients with MCSM, and it can effectively improve the spinal cord nerve function and pain, and it is beneficial to maintaining the cervical range of motion and reducing the postoperative complications.

Influence of Pre-Intravenous Injection of Esketamine on Postoperative Pain Effect and Complications in Patients Undergoing Laparoscopic Cholecystectomy

Objective:
To observe the influence of pre-intravenous injection of esketamine on postoperative pain effect and complications in patients undergoing laparoscopic cholecystectomy (LC).
Methods:
A total of 102 patients undergoing elective LC in the hospital between January 2021 and January 2022 were selected as the observation subjects, and they were numbered according to the order of admission and divided into the esketamine group (odd number) and the routine anesthesia group (even number) by the odd-even number method, with 51 cases in each group. Results:
The intraoperative propofol dosage and remifentanil dosage in the esketamine group were significantly less or lower than those in the routine anesthesia group (P<0.05), and the laryngeal mask removal time and postoperative awakening time were significantly shorter than those in the routine anesthesia group (P<0.05). MAP at T1, T3 and T4 in both groups were significantly lower than those at T0 (P<0.05), and the MAP at T1, T3 and T4 in the esketamine group was significantly higher than that in the routine anesthesia group (P<0.05). Effect of Spermatic Vein Ligation on Total Antioxidant Capacity of Sperms in Patients with Varicocele

Objective:
To evaluate the effect of spermatic vein ligation on the total antioxidant capacity (TAC) of semen in patients with varicocele.
Methods:
Forty infertile male patients with varicocele and 40 healthy controls (control group) were admitted to our hospital for primary or secondary infertility. Patients with varicocele were divided into two groups: nonobvious palpation (GI) varicocele group (16 cases) and obvious palpation (GⅡ-Ⅲ) varicocele group (24 topics). All patients underwent microscopical ligation of the spermatic vein. The semen TAC level and sperm parameters of all patients were evaluated. The preoperative sperm quantity, sperm motility, sperm morphology, and semen TAC level were compared with the same parameters 3 ~ 6 months after spermatic vein ligation and the control group.
Results:
After spermatic vein ligation, the sperm count (preoperative 18.01±1.88, postoperative 24.20±7.60, P=0.040), motile sperm (preoperative 17.90±8.90, postoperative 23.90±7.90, P=0.001), NF (10.20±3.02 before operation, 19.30±7.05 after operation, P<0.001) and semen TAC level (2.52±0.28 before operation, 3.72±0.20 after operation, P=0.031) were higher than those before operation, and the difference was statistically significant (P<0.05); and compared with the control group after operation, sperm count (varicocele patients 24.20±7.60, control group 29.40±8.30, P=0.156), sperm motility (varicocele patients 23.90±7.90, control group 23.90±7.90) 30.50±8.00, P=0.465), NF (varicocele patients 19.30±7.05, control group 23.90±7.90, P=0.085) and semen TAC level (varicocele patients 3.72±0.20, control group 2.78±0.20, P=0.666), the difference was not statistically significant (P>0.05). However, according to the evaluation of the patients' varicocele grade, only patients with GII-III varicocele had NF before and after surgery (6.16±2.50 preoperatively, 10.00±6.41 postoperatively, P=0.040), TAC (preoperatively 6.16±2.50, P=0.040). 2.36±0.23, postoperative 2.66±0.15, P=0.040) level difference was statistically significant (P<0.05). When the patients were grouped according to semen parameters, there was a statistically significant difference in TAC levels in the oligospermia (OAT) group (2.46 ± 0.34 before surgery, 2.80 ± 0.19 after surgery, P=0.000).
Conclusion:
Spermatic vein ligation significantly improves the total antioxidant capacity of semen in patients with GII-III varicocele.

Application of Ultra-High Resolution Subtraction CT Angiography in the Follow-Up after Intracranial Aneurysm Treatment

Objective:
To study the application effects of ultra-high resolution subtraction computed tomography (CT) angiography (CTA) in the follow-up after treatment for intracranial aneurysms.
Methods:
The clinical data of 98 patients with intracranial aneurysms who were treated in the hospital from June 2020 to January 2022 were retrospectively analyzed. The patients underwent ultra-high resolution subtraction CTA and digital subtraction angiography (DSA) review at 3 months after surgery. Taking DSA as the "gold standard" to analyze the diagnostic value of ultra-high resolution subtraction CTA in diagnosing aneurysm occlusion, residual aneurysm neck, parent artery and intracranial macrovascular patency and presence or absence of postoperative cerebral vasospasm (CVS) and new aneurysm in patients with intracranial aneurysms after treatment.
Results:
The sensitivity, specificity and accuracy of ultra-high resolution subtraction CTA were 97.87% (92/94), 75.00% (3/4) and 96.94% (95/98) in the diagnosis of aneurysm occlusion in patients with intracranial aneurysms after treatment, were 95.70% (89/91), 85.71% (6/7) and 96.94% (95/98) in diagnosing residual aneurysm neck, were 98.89% (89/90), 62.50% (5/8) and 95.92 (94/98) in diagnosing the parent artery and intracranial macrovascular patency in patients with intracranial aneurysms after treatment, were 62.50% (5/8), 98.89% (89/90) and 95.92% (94/98) in diagnosing the presence or absence of CVS, and were 50.00% (1/2), 98.96% (95/96) and 97.96% (96/98) in the diagnosis of presence or absence of new aneurysm after treatment.
Conclusion:
Ultra-high resolution subtraction CTA has high diagnostic accuracy in diagnosing tumors and blood vessels in patients with intracranial aneurysms, and it is helpful for postoperative follow-up diagnosis of tumor occlusion and postoperative complications, and can provide a basis for judging clinical follow-up and help clinical formulation of follow-up treatment regimen.

Construction of Influencing Factors Associated with Thrombotic Microangiopathy Secondary to Systemic Lupus Erythematosus and Its Predictive Model

Objective:
To investigate the influencing factors associated with thrombotic microangiopathy (TMA) secondary to systemic lupus erythematosus (SLE) and its prediction model.
Methods:
One hundred and fifty-six patients with TMA secondary to SLE in our hospital from January 2018 to January 2022 were selected as the study group, and 156 patients with SLE in the same period were included as the control group according to the 1∶1 principle. The clinical characteristics and laboratory indexes of the two groups were counted to construct a logistic regression model for thrombotic microangiopathy secondary to SLE, and to establish the scoring criteria for TMA secondary to SLE based on logistic regression model analysis to test its reliability.
Results:
The duration of SLE, SLEDAI score and peripheral blood Hcy and CD154 levels in the study group were higher than those in the control group, and peripheral blood PLT and ADAMTS-13 levels were lower than those in the control group (P<0.05); SLEDAI score (OR: 16.291) and peripheral blood Hcy (OR: 15.175), PLT (OR: 0.670), CD154 (OR: 8.851) and ADAMTS-13 (OR: 0.533) are influencing factors for TMA secondary to SLE ((P<0.05). The logistic regression model had an AUC of 0.917, a sensitivity of 92.95%, and a specificity of 71.15% for predicting TMA secondary to SLE. Referring to the beta values of the logistic regression model and its variable types, the SLE secondary TMA rating scale was developed, which had a Cronbach's alpha coefficient of 0.840, correlation coefficients between the factors and between the factors and the total scale score ranging from 0.590 to 0.783, and an exploratory factor analysis factor loading of 0.665.
Conclusion:
The SLEDAI score and peripheral blood levels of Hcy, PLT, CD154 and ADAMTS-13 are factors influencing the development of TMA secondary to SLE, so the construction of a line graph prediction model can help predict the risk of TMA and guide clinical management.

Analysis of Preoperative Blood Lipid Levels and Coronary Artery Disease Syntax Score in Postmenopausal Women with ACS Undergoing PCI

Objective:
To investigate the status of preoperative blood lipid levels and the Syntax score of coronary artery lesions in postmenopausal women with acute coronary syndrome (ACS) patients undergoing coronary intervention, so as to provide a favorable basis for clinical treatment.
Methods:
A total of 126 postmenopausal female ACS patients who underwent interventional therapy in our hospital from June 2020 to June 2021 were selected and divided into control group (34 cases of acute ST-segment elevation myocardial infarction) and observation group (92 cases of acute coronary syndrome with non-ST segment elevation), the changes of preoperative blood lipid levels in the two groups of patients were analyzed, and the Syntax score of coronary artery lesions in the two groups of patients was also analyzed.
Results:
There were significant differences in menopause age, history of hypertension, diabetes history, smoking history, TC and TG indexes in the observation group (P<0.05). There was no significant difference in HDL-C, LDL-C level stratification and Syntax score between the two groups (P>0.05). In the control group, the level of LP(a) in severe lesions was higher than that in mild and moderate lesions, and the level of LP(a) in moderate lesions was higher than that in mild lesions, and the differences were statistically significant (P<0.05). However, in the observation group, the levels of LDL-C, LP(a) and TG in the severe lesions were higher than those in the mild and moderate lesions, and the levels of LDL-C, LP(a) and TG in the moderate lesions were higher than those in the mild lesions. Statistical significance (P<0.05). Apolipoprotein B/Apolipoprotein Al (APOB/APOAI) in the severe lesion was higher than that in the mild and moderate lesions, and the difference was statistically significant (P<0.05).
Conclusion:
In the treatment of postmenopausal women with acute coronary syndrome by coronary intervention, the more important blood lipid indicators are APOB/APOAI, LP(a), LDL-C, and TG, which should be further studied in clinical practice to provide an effective basis for treatment.

Analysis of Bacterial Population Spectrum and Drug Sensitivity in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Objective:
To analyze of bacterial population spectrum and drug sensitivity in patients with chronic obstructive pulmonary disease (COPD).
Methods:
The sputum culture data of 212 patients with COPD were retrospectively analyzed between January 2018 and June 2022. The distribution characteristics and drug susceptibility test results of pathogens in sputum culture were analyzed.
Results:
There were 212 strains of pathogens isolated from sputum culture specimens, including 111 (52.36%) strains of Gram-negative bacteria, 9 (4.24%) strains of Gram-positive bacteria and 92 (43.40%) strains of fungi. In Gram-negative bacteria, proportion of Pseudomonas aeruginosa was the highest (23.11%), followed by Escherichia coli (8.02%). In Gram-positive bacteria, proportion of Streptococcus pneumoniae was the highest (1.89%). In fungi, proportion of Candida albicans was the highest (40.09%). The results of drug sensitivity test showed that Pseudomonas aeruginosa was 100% sensitive to colistin, and the resistance rate to other antibiotics was 6.12% ~ 28.57%; Escherichia coli was 100% sensitive to meropenem and tigecycline, and 94.12% sensitive to imipenem and ertapenem; Klebsiella was 100% sensitive to amikacin, piperacillin tazobactam, cefoperazone sulbactam, imipenem, meropenem, tigecycline and ertapenem, and the resistance rate to other antibiotics was 11.76% ~ 29.41%.
Conclusion:
The pathogenic bacteria of COPD are mainly Gram-negative bacteria, mainly Pseudomonas aeruginosa and Escherichia coli. It is recommended to refer to the results of drug sensitivity test.

Analysis of the Effects of Insulin and Zoledronic Acid Combination Therapy on Bone Metabolism and Islet Beta Cells in Patients with T2DM Combined with OP

Objective:
To investigate the effect of insulin and zoledronic acid (ZOL) combination therapy on bone metabolism and pancreatic islet β cells in patients with type 2 diabetes mellitus (T2DM) complicated with osteoporosis (OP).
Methods:
A total of 106 patients with T2DM and OP diagnosed and treated from January 2020 to August 2021 were selected as the research subjects, and divided into control group (treated with ZOL combined with acarbose tablets) and observation group (treated with ZOL combined with insulin), 53 cases in each. Alkaline phosphatase (ALP), bone-specific alkaline phosphatase (BAP), osteoprotegerin (OPG), insulin secretion index (HOMA-β), and insulin resistance index (HOMA-IR) were compared; adverse reactions, including Nausea/vomiting, headache, dizziness, bloating/abdominal pain, and rash were observed.
Results:
Before treatment, there was no significant difference in ALP, BAP, OPG, HOMA-β, HOMA-IR between the two groups (P>0.05); after treatment, ALP, BAP, OPG, HOMA-β in the observation group were higher than in the control group, and HOMA-IR was lower than in the control group (P<0.05). In the observation group, there were 2 cases of nausea/vomiting, 1 case of dizziness and 1 case of rash; 3 cases of nausea/vomiting, 1 case of headache and abdominal distension/abdominal pain, and 2 cases of dizziness in the control group; there was no statistically significant difference in the comparison of adverse reactions between the two groups (P>0.05).
Conclusion:
Insulin combined with ZOL has a significant effect in the treatment of T2DM complicated with OP, which is beneficial to improve bone metabolism and islet β-cell function, with high safety and no serious adverse reactions.
