Effect of Steel Wire Tension Band Fixation Combined with Total Hip Arthroplasty in the Treatment of Elderly Osteoporotic Femoral Intertrochanteric Fractures

Abstract：Objective: To explore the efficacy of steel wire tension band fixation combined with total hip arthroplasty (THA) in the treatment of elderly osteoporotic femoral intertrochanteric fractures. Methods: From January 2020 to January 2023, 102 elderly patients with osteoporotic femoral intertrochanteric fractures were randomly divided into THA treatment group and combined treatment group by lottery method, with 51 cases in each group. The THA treatment group was treated with THA, whereas the combined treatment group was given steel wire tension band fixation combined with THA. The surgical evaluation parameters (intraoperative blood loss, surgical time, bed rest time and fracture healing time), hip ranges of motions, clinical efficacy (Harris hip score) and complications were compared between groups of patients. Results: Compared with THA treatment group, the surgical time in combined treatment group was longer, and the bed rest time and fracture healing time were shortened, and the intraoperative blood loss was reduced, and the incidence rates of complications were decreased (P<0.05). At 4 months after surgery, the excellent and good rate of clinical efficacy in combined treatment group was higher than that in THA treatment group (P<0.05). The hip ranges of motions in the two groups were enlarged, and the differences of hip ranges of motions at 4 months after surgery and before surgery were greater in combined treatment than those in THA treatment group (P<0.05). Conclusion: Although steel wire tension band fixation combined with THA in the treatment of elderly osteoporotic femoral intertrochanteric fractures increases the surgical time of THA, it can effectively shorten the bed rest time and fracture healing time, reduce the intraoperative blood loss, increase the hip ranges of motions, and enhance the clinical efficacy, and has high safety.
