Study on Correlation between Coagulation Indexes and Homocysteine β2 Microglobulin and Occurrence of End-Stage Renal Disease in Patients with Hypertensive Nephropathy

Abstract:Objective: To analyze the correlation between coagulation indexes and homocysteine (Hcy), β2-microglobulin (β2-MG) and occurrence of end-stage renal disease (ESRD) in patients with hypertensive nephropathy (HN). Methods: A total of 138 patients with HN in the hospital from August 2019 to August 2021 were selected as the study group. According to the occurrence of ESRD after 3 years of follow-up, the above patients were divided into ESRD group (n=26) and non-ESRD group (n=112). In addition, 102 hypertensive patients without nephropathy who received examination in the hospital during the same period were included in the control group. The levels of coagulation indexes [D-dimer (D-D), fibrinogen (Fib), fibrinogen degradation product (FDP)], Hcy and β2-MG were measured among all study subjects. The correlation between D-D, Fib and FDP levels and Hcy and β2-MG levels in hypertensive nephropathy patients was analyzed by Pearson correlation, and the correlation between D-D, Fib and FDP levels and the occurrence of end-stage nephropathy in hypertensive nephropathy patients was analyzed by Logistic regression. Results: The levels of serum D-D, Fib, FDP, Hcy and β2-MG in the study group were higher than those in the control group (P<0.05). Serum D-D, Fib, FDP, Hcy and β2-MG levels were higher in the ESRD group than those in the non-ESRD group (P<0.05). Correlation analysis indicated that serum D-D, Fib and FDP levels were positively correlated with Hcy and β2-MG levels in patients with HN (r=0.771, 0.795; 0.568, 0.619; 0.545,0.591, P<0.05). The levels of D-D, Fib and FDP in hypertensive nephropathy patients were independent risk factors for end-stage nephropathy (P<0.05). Conclusion: The levels of D-D, Fib and FDP in hypertensive nephropathy patients were positively correlated with the levels of Hcy, β2-MG and end-stage nephropathy.
