Relationship between Serum Leptin Free Fatty Acid and Adiponectin Levels and Insulin Resistance in Tibetan Patients with T2DM at Different Altitudes in Qinghai

Abstract:Objective: To investigate the relationship between levels of serum leptin, free fatty acid (FFA) and adiponectin and insulin resistance in Tibetan patients with type 2 diabetes mellitus (T2DM) at different altitudes in Qinghai. Methods: The medical records of 80 Tibetan patients with T2DM in the hospital were retrospectively analyzed from October 2022 to July 2024. According to the different altitudes of the patients, 41 cases were included in high altitude group ( ≥3000 meters), and 39 cases were enrolled as low altitude group (<3000 meters). Serum leptin, FFA, adiponectin and insulin resistance index (HOMA-IR) were detected. The differences in the above indicators were analyzed in the two groups. The correlation between serum leptin was explored by using Pearson correlation analysis., FFA and adiponectin and insulin resistance and to compare the correlation strength between indicators and HOMA-IR in the two groups. Results: The levels of serum leptin and serum FFA and HOMA-IR in high altitude group were higher than those in low altitude group (P<0.05), while the adiponectin level was lower than that in low altitude group (P<0.05). Serum leptin and FFA levels were positively correlated with HOMA-IR in both groups (P<0.05), and adiponectin level was negatively correlated with HOMA-IR in both groups (P<0.05). From the distribution of r value, the correlation effect in high altitude areas was stronger, nevertheless, there was no statistically significant difference between the groups (P>0.05). Conclusion: In Tibetan patients with T2DM in high altitude areas, serum leptin level and FFA level show high trends, while adiponectin level is relatively low, and the above indicators are closely related to insulin resistance in both high altitude and low altitude areas, no difference in the strength of association has been found between different altitude regions.
